I've noticed several reoccurring themes in McMillen's more popular games:
1) have a "dark world" ( harder versions of previous levels)
2) have some form or iteration of Hell
3) have at least one weird/ disturbing enemy (lil' chads in SMB, fuckin' everything in The Binding of Isaac, and most of the enemies in The End is Nigh, *PARTICULARLY THE FUCKING MOTHER LIKE HOLY-* )
4) Rockets. Just, put them in somehow.
And that's it! Unless I'm missing something...
...Ah, yes!
5) The main character must, and I mean MUST, be adorable, but have a very morbid aura around them.

6) Guppy HAS to show up somewhere
7) Have "- God" Achievement for 100% the game (Or like 5 for TBoI)
8) Have an ending credits sequence showcasing all of the enemies of the game
9) Have a Bonus song on its OST with vocals (Power of Meat, He's the Number One, and The End is Nigh Bonus)

10) Have tumors in one way or another (collect them in TEIN and fight them in TBOI, sadly no tumors in smb '^' )

I'm not sure if this is correct, but my understanding is this: Ruin and Acceptance are completely different songs in the game files, but Anguish, Gloom, and Blight are actually a track laid on top of the original song of the area. Since they were made this way, they aren't in the OST. It's also possible it was done to prevent spoilers from being in the OST, as people might too easily connect the songs to the other ones and the order of areas.
