    BEST video I have seen and I trust her. I almost paid for PRF and am so glad I saw this beforehand. Also, I hyperpigment easily and I am happy I didn't try microneedling under the eye. Both of which I've been debating on trying.

    What an informative and concise roundup! Could you do one just like this for the barcode/smokers lines around the lips? Between Botox (lip flip), filler, PRF, ablative Lasers , microneedling, actives and lately Microcoring, it can be hard to know what works and what’s BEST to treat these buggers! Thanks, Doc!

    I enjoy watching your videos, and appreciate your expertise. Micro needling has been a GAME Changer for my face and especially neck. It has completely rid my under eyes and neck of crapiness, uneven texture and my face easily looks 5/10 years younger from the stimulated collagen. done correctly and consistently, microneedling works.

      Some are trying to be cheap not thinking long term or sometimes the doctors will do it for a discount rate or free if they promote their business.

      Because they have way too much money. The general population can only afford to do a few treatments here and there so it remains natural looking. Celebrities who have millions of dollars and millions of people looking at them and commenting on their appearance and rich friends who also have tons of treatments done that they recommend to them. They can afford all of the “latest and greatest” treatments and are often quite demanding. Cosmetic Doctors are a business just like any other. Even if they don’t think the patient doesn’t really need what they are asking for they know that if they don’t provide the treatment the celebrity client will find a doctor that will.

    Extremely beneficial information! I had deep hollows and the plastic surgeon used Restylane. I don't like the fact that the filler is a little lumpy and somewhat visible to me. He didn't use a canula. Laser was never discussed sadly. Would a chemical peel be effective for collagen stimulation under the eyes if the cost of a laser treatment is prohibitive?     I tried Botox under the eye per recommendation at a medi spa. I looked awful! It got rid of my crows feet but over emphasized my fine lines under my eyes and looked like a frown line. I had gone back and expressed my concern. She said there was nothing she could do about it and blepharoplasty was my option to correct the issue. Longest 3 months of my life until I looked normal again.

    Q switch lasers are good under eye dark circles, no idea how carbon peel would work and PRP is a Chefs Kiss. But CO2 Fractional Laser can cause a bit of waxy skin. But great video thank you.

    Thank you as always for another extremely valuable & informative video Dr. Kappel. I always learn so much from you!! I had my appointment with a new Dermatologist b/c I wanted to talk about the possibility of getting Sculptra. Turns out when I got there I ended up seeing his Nurse Practitioner. They supposedly misunderstood me when I made my appointment & thought I just wanted a skin check. He was a very nice young man but did no cosmetic procedures. They worked on skin cancer, etc. at this particular location. At least he was honest. They have another location in a town about an hour away where they focus on cosmetic procedures. 🙌🙌

    Just the video I've been waiting for! Thanks for covering PRP 💜 I just did round one of Excel V laser for rosacea but now I wonder if that would also work for the dark circles that recently appeared under my eyes. I will be doing a Thermafrax treatment in the winter. I also have an upcoming appointment for filler but I'm hesitant even though the dermatologist I'm seeing is an expert injector. I'm wondering if the filler will be unnecessary and it's something I could pass on.

    I’m 28 and I’ve had several clear and brilliants and have loved them! However, they don’t go close enough to my under eyes to address that area. They kind of avoid it.. is this normal?

    Going for a Fraxel eye consultation tomorrow at Chicago Cosmetic & Dermatology with Dr. Carolyn Jacob. Maybe you know of her? What is the downtime for Fraxel & Thermage for the under-eye and upper eyelids? Plan to do full-face Thermfrax this fall. Also, I see other well know dermatologists continue to say Thermage is an annual procedure and you are the only one I ever hear say 3-5 years. I am 38, trying for a second baby and trying to get a head before I turn 40.

    Have you heard of the prf Ez filler? It’s like bio filler. If so what are your thoughts? You’re correct PRP did nothing for me but I’m too scared to get tear through (HA) the ez filler is supposed to be safer but I don’t want to waste my time and money x❤

    Dr. Kappel - you mention often that your prefer Thermage vs RF Microneedling (Morpheus, SylfirmX, etc.). However, isn't the energy approach the same? I ask because every cosmetic dermatologist in my state that I have found says they no longer do Thermage because newer technologies offer the same RF tightening but with added benefits. I am focused on lower face / jawline tightening and underwhelming results with lipo, Accutite, and a few Morpheus and SylfirmX treatments - the last 3 all providing strong RF tightening like Thermage. What am I misunderstanding?

    Hi Dr Kappel, would you say lasers can work as a substitute to fillers in cases of volume loss? i can clearly see the outline of the orbital bone where it connects with the cheekbone and Id like to "fill that void" for lack of better words but am a bit fearful of fillers.       @Sofwarriors PRF does not last. Plasma products have an affinity to be intravascular so it get circulated out of the area injected 2 weeks later.

    Hey, what about prf for tear troughs if you have an autoimmune disease (I have psoriatic arthritis)? I wanted tear trough fillers for anatomical hollows that are worsening as I approach 40. I visited a dermatologist about tear trough filler and she advised (and I researched) that a hyperactive immune system can react to cross linked hyaluronic acid used in fillers. In my research it looks like prf may be a safer alternative?

      Just an FYI. I know everyone is different. I have autoimmune issues too, RA and Sjogrens, and was very worried about potential complications. The plastic surgeon who did my tear troughs said I would be ok using hyaluronic acid and I actually did fine. I’ve had Restylane for my eyes and Juvaderm for my lips. Good luck!

    I live in Tampa bay and we are so limited on Dermatologists and Plastic Surgeons that do all the lasers. Most will do the surgical services and then have their PAs do the lasers. One Medspa has their medical aesthetician do the Fraxel/Clear and brilliant in South Tampa. I feel like that should be illegal

      In some states yes but laser training is a highly complex subspecialty depending on the devices being used. It’s like letting them perform surgery.

    Hi Stefani. From my understanding you noted that the results for under eye treatment with Thermage typically lasts up to a year and five years? Can you tell me exactly how long the results last? I am not sure if it is a year or up to five years, I assume it depends on the patient.

    Does one need to have the eye shields put in for laser under eyes and on eyelids ? I want to make sure whoever does mine does it right . Thank you ! I I’ve always been afraid of fillers, because I thought they would stretch the skin as well. To hear a doctor say it confirms it. Thank you. Now I just need to find a good laser technician in my area.

    I do some of my treatments in Mexico because I live in a border town so it is much more affordable. I have been going to the same provider for about 15 years. Is thermage the same as radio frequency? And what do you think about periocular Morpheus 8, which is what she recently recommended?

    I would love to hear, what is the dose/percentage of Tretinoin you would maximally prescribe for lower eyelids (of course not applied too close to the lash line). I was burned by co2 Laser and have indented scars on my lower eyelids for more than a year now

    Hi Dr. Would fraxel help with a forehead scar? I have had it all my life from an injury as a child. It was dent-like in appearance up until a year ago when I had filler done by a dr to see if it would fill it in. It worked but now looks more scar like with a small white line and make the skin look even uneven in texture and tone. Would love to know the best treatment to pursue. Many thanks in advance

      There are many skincare options for scars. Hop over to skinbeautifulrx she has a RUclips channel and online store and sells almost every brand of medical skincare. She has a big scar on her forehead and has a video about what she used to have it faded! She’s an Asthetician.

      Really grateful to have this channel! I’m someone who’s has the med-spa treatments with either results that were only good for a few months…or no results at all. And they still charge thousands of dollars. I LOVE your expertise on lasers, and this gives me comfort that I can do some tightening in a non-invasive way. I’m turning 42 in a few days and STRESSING OUT, lol.

    How do you even find someone who will do lasers on the skin around your eyes? Everyone ive tried wont get that close to eye area.

      Its always best to find a laser specialist who completed a fellowship in laser medicine. To get near the eye , a protective eye shield needs to be placed in the eye and a special laser tip needs to be used. Many either don't have the appropriate training or don't have the specific instruments for the eyes.

    I got 6 PRP treatments for my under eyes and they did absolutely NOTHING. What a waste of money
