    Heartbreaking, doc. This poor dude. I remember one of your videos early in Zions career where you observed his walking motion and knee inversion. I can't believe how correct you were in the observation of his injury concerns. Super disappointing, Zion could be so, so dominant

      There’s some evidence that people with chronic ankle instability walk more on the outside of their feet. I wonder about something like that with Zion but it’s too hard to tell from afar

    The fact that one ankle sprain like this can cause multiple after is so true , I’ve had about 14 sprains in my life as an athlete and it brought me to have bad sleep thinking about that so I decided to stop playing my sport and concentrate on lifting

      @@lanceberry9688 I wish. Too many sprains for me in my youth, now there's nothing left to rehab. I roll my ankles at least a dozen times a day just from instability.

    Thanks Doctor, insightful and well-explained in relatively simple terms for most of us who don't understand the basic physiology. My son had this problem playing soccer, rolled his ankle badly and then continued to have ankle sprains despite intensive physiotherapy for months. In the end, he had to give up the game he loved, but found another sport to enjoy.

      I wouldn’t be as worried about the ankle long term if he is doing the Steph Curry type protocol to keep the ankles in top shape. Sometimes it takes these tweaks to get those things started, which is why this really is a big moment for them to make sure prehab is implemented

      Part of it has to do with the rapid muscle build. PJF performance predicted it. I wish he would get with PJF Performance or some better trainers

      @@olivermichael7152 Yeah, unfortunately, the NBA still doesn't necessarily always hire the best or most qualified trainers. The fact that chiropractors are on NBA teams, which is literally just pure pseudoscience is evidence of this.

    I feel like the way he walks has so much to do with this. He leans side to side so much when he walks putting all that muscle memory movement on the outside of the foot. Possibly forced his foot to fold in like it did more than it would’ve for most players due to him pushing off the sides of his feet so much.

    When I was a kid, my ankles rolled a lot when I’d run or walk around the house barefoot, especially when turning corners. I think my body unconsciously over-compensated by making me walk on the insides of my feet which messed up so many things. I didn’t realise that was happening until my senior year in a strength and conditioning elective gym class. I was having really bad shin splints, which I’d experienced before when I was more active as a kid. I talked with my gym teacher about it and he suggested new shoe types and the way I land on my feet when I’m running, and we figured out part of the problem. So nowadays I am really conscious of what part of my foot I’m walking on, it definitely helps my legs feel better when standing or walking. I had no idea the chronic issues could have been happening to me. My mom and I just said we had “loose ankles.” Thank you for educating me 😊 ❤

    I had an injury like this prior to my senior HS season. My toe hooked into a defenders shoe top and had an inversion similar to this. The orthopedist said it was a type 2 almost high ankle sprain/strain. Also said if there was a bit more force I likely would have had an open fibula fractures. The separation of the syndesmosis is what generates most of the pain and also affects the mobility/spring after the injury. I stopped wearing ‘basketball shoes’ and instead opted for running shoes afterward and never had an issue again.

    This ankle sprain reminds me a ton of what I went through in high school. I had to wear an ankle brace plus taping for four years. I would twist my ankle almost every game for a year. I had to work very hard at learning how to roll with it to prevent further injury as well as countless hours in the gym strengthening and improving balance. After four years I have been fine for years of sports.

      @@PatrickRami oh man. I did calf raises both feet then one foot as it got stronger, stair way climbs, box jumps, tons of stretches, the alphabet thing where you go through making your foot write the alphabet. Used the half ball things to stand on at first, then did squats on them, would do shoulder raises with one arm while standing on the half balls. This is a progression as the leg gets more stabilized. Jumping jacks, jump rope. Ladder on the ground work, shuffles, high knees. Basically anything out there for increasing leg strength and stabilization. I did hot and ice bath alternating for keeping inflammation out. I did most exercises without ankle supports on and worked very slow to start and increased work load and speed as I went. When I played sports I always used tape and an ankle brace to stabilize for the four years I continued to work. Eventually it got to the point I didn’t twist it any more while playing, which is when I started trying it out slowly in sports without the brace.

    I had this issue as a kid. I was a heavier than average teen but didn't let it stop me playing sports. But boi playing basketball my ankles would always roll and twice in life I came down off rebounds on a rolled ankle which would pop a couple times during the process lol I never went to the Dr as a kid wish I had.

    That is the number one way I sprain my ankle and I've even had a few breaks from that mechanism. As a kid, I would sprain my ankle at least once a month or more unless I broke it and was in a cast for six weeks. I have broken my ankle four times in my life, twice on each side, and I can definitely verify how it leads to an unstable ankle.
    I haven't sprained my ankle in a decade now though and I'm fairly certain it's because I started doing yoga. I remember feeling pain in all those muscles after my sprains and I noticed that yoga works those muscles. After several years of doing yoga, I noticed that I had stopped spraining my ankle. I was going to school full-time with a difficult degree so it got to the point where I was only doing yoga for regular exercise, but I did walk to school everyday and I would often take big hikes in the mountains if I had the time on the weekend. Never did get another sprain.

      @@Skerboysweet I fucked them up a lot because I did stunts from getting home from school until my parents made me come inside and I absolutely pushed the limit. I didn't know it at the time but I have a genetic ligament disorder that makes my ligaments more likely to sprains, although I guess I strengthened them with yoga because it's not like I was a young person when I discovered yoga.

      @@waff6ix A lot of the common gym exercises work the larger muscles and more muscle fibers at one time when sometimes joint stability requires one to strengthen the smaller muscles and smaller groups of muscle fibers to make for stable joints. I'm quite a bit older now and I don't know what to do for arthritic joints other than to warm them up and put some weed lotion on them when they get all inflamed. I'll figure it out if I live long enough.

      @@BrianSuttererMD Isn't there research suggesting that wearing ankle braces compromises your overall ankle strength and knee protection? I can understand wearing them in games, but it's probably best to avoid them when training, correct?

      @@myjciskate4 yeah, but if your body is made of glass and you're a bulky player like Zion, the added support might be more beneficial than the risk of long term weakening of the joint. Yeah, I'd imagine it would be better to train. Without the brace to build strength.

    I've had about 4-5 right ankle sprains, but each time wasn't the same, maybe about 2 were bad but overall I kept stretching it, icing it immediately, casting it and making sure to balance after all that when I could, I played basketball during these times

    I probably sprained my ankle at least 3 times without noticing it. Never got proper treatment as I thought it was just pain I had to deal with.
    This was until one day I completely blew my ankle out during a run as I had a complete inversion roll. Of course this happened in 2020, where I couldn't get treatment either. I got surgery after an MRI revealed severe permanent damage to multiple ligaments and some of the muscles in the area. The surgery happened a year and a half ago and I'm still not back to being able to do the activities I once did. I'm also only 21 for reference.
    Use this as a tale of caution and listen to what the doc says. Take care of those ankles.

    Hey Dr. Sutterer, another great short video explaining to us normal people about the inner workings of our anatomy. I've sprained my ankle more then a few times from basketball, both to be exact but I've NEVER gotten them really looked at besides a few times since I wasn't a professional athlete. I now deal with a weird pain sometimes in my left ankle but the worst is the itch that feels like it's being caused by a ligament or a nerve. Any comment that would help would be great, thank you in advance & keep pushing out great content!

    I had issues with my ankle especially playing soccer and my foot would just roll even just jogging on the field. Had to get surgery because my tissue that held my perineal tendon was very loose and my tendon would just move out of place when I flexed my foot. I learned the hard way, stabilizing starts at the hip. I worked my hips like crazy and now have much better balance.

    Do you have to specialize in something specifically you know orthopedic surgery or just sports medicine in general? I'd rather hear it from the horse's mouth then watch a RUclips video or go to reddit!

    I’ve had issues with ankle sprains in the past and I found a lot of success with getting into yoga and mma to help strengthen my core and hips. Really helped stabilize my body when I played basketball

    Hey Brain, do you think you could do a video about pseudoscience in professional sports leagues? I understand that there's always been a bit of a problem with pseudoscience creeping in to major sports leagues for a while (mostly because the money can often attract those types of people), but it's not something that often doesn't get talked about a lot, unfortunately. I think that's be a really cool and educational video.

    Based on one of the first videos Brian did on Zion evaluating his gate, it really puts this injury into better context. You could see the way he extended his foot, it was almost turned inward before it ever hit the ground...and without another players foot to disrupt the landing, it just rolled on it's own. I really hope this isn't just a constant thing with him due to his bio-anatomy, but it seems like that's exactly what it is...

      no he was bumped before so that pushed his leg off balance and he did not have enough time to correct it. This video is not entirely correct cause it does not focus on how the bump changed everything. If you extend your foot out on an explosive drive to the hoop and it gets bumped how the hell are you going to have enough time to correct that? its like someone pulling a chair out from under you. Dont go blaming Zions body for this one.

    Great stuff. And considering damaged ligament/collagen tissues can sometimes take up to a year to truly heal, the risk of CAI will go way up.

    Hi Dr. Sutterer I really hope you see my comment. I am a 46 year old, 6'0 180 pound skinny leg guy. I sprained my right ankle in late August. I did this by missing the last step on a set of stairs and twisted my ankle. My right ankle twisted inward just like Zion did and immediately my ankle was swollen. I've had two X-rays and I have no fractures but I am still in some pain. I was told to wrap it and ice everyday. I am by no means an athlete like Steph or Zion but I would love advice on healing. I was given a walking boot the first week of September and wore it for about three weeks. Just started physical therapy last week with three sessions down so far. PT had me doing calf stretching, ankle movements with a resistance band, leg presses with just my body weight, and step up exercises. I can bear weight on my foot and walk around normally but if I or physical therapist touch my ankle on the outside, inside, or upper right side of my shin I still feel some pain. My orthopedic doctor said I have severe soft tissue damage but nothing calls for a MRI unless it doesn't get better by next month.
    Should I be concerned that I still feel some discomfort on my ankle? Do I need to still wrap it every day? Should I get some kind of ankle brace? Do I need to be icing a couple times a day? I've heard sprains can take months to heal but just asking for your advice on anything you could provide that I could do at home to make it heal properly. Thank you advance! 👍

    if you play basketball, you will inevitably roll your ankle a few times (non-contact and contact). The key has always been how much pressure you put on it, i think learning to immediately fall as soon as you start to feel the roll is really important. Here Zion doesn't fully jump off his ankle, but he does a little. That cant be good.

    Hey doc. Just starting the video but In Anterior Inferior Tibiofibular ligament injuries do the ligaments get adequate blood supply? My professor explained why Knee injuries can tend to be incredibly bad due to lack of blood supply but I was wondering if this had the same effect on Ligaments across the Body.     Doc I need your help! Can you do a video about behind the knee muscle injuries. I've been hurting and it's been uncomfortable to walk for 4- 5 months. Every time I think I'm okay I go play ball and I'm in pain again. Can you do a video informing people of the importance of taking care of hurt muscles. And how ta small injury can lead to alot of discomfort if you don't take care of it

    Wow. I rolled my ankle 1 good time when I was 18. I’m 42 now and my ankle rolls all the time now. Had to quit playing pick up basketball. It sucks knowing that at anytime my ankle will roll. Now it all makes sense. Thanks for this. Ps I have since been able to play BBall after trying out the Nike PG5 and a brace. The design of that certain shoe and the tech does help. I have t rolled my ankle playing in a few months of wearing that shoe. But rolled my ankle on the slightest invert in the concrete walking through Dollywood 2 days ago wearing regular Nike air max.

    I suffered a bad high ankle sprain in college. Had probably 3 more ankle sprains in my college career. Then in the arena league I sprained it badly in practice. Finished the season but after that year I was done.

    I wore ankle braces, goaltending futsal for a reason. The braces bring my foot back to true when I had to jump. I played at 230lb(rough league, had to intimidate). Had I not worn ankle braces, I would have been injured all the time. On the other hand doc... Cant fix them if they dont get hurt in the first place

    I had many ankle problems up to sophomore year in high school. I even broke my ankle at one point. In my recovery, I spent a full year focusing on increasing my ankle mobility (specifically, Achilles tendon mobility). I have not had an ankle sprain since then. That was 30 years ago.

    I remember In PE I tried dunking and my foot was like this and I jumped off it I sprained my ankle and it took a long time to fully recover and probably took an inch or two off my max vert

    i have fractured my 5th metatarsal twice . both have been from the twisting of my ankle. both being non contact . same motion has zion has done in this video. i hope he’s alright

    I've been saying since he was in college that he's the closest thing to Bo Jackson I've ever seen. What I mean by that is he's too strong for his own body which means him having a career ending injury before his actual prime was inevitable. Plus, Zion his playing on frickin hardwood which means it was just going to expedite these injuries as opposed to Jackson playing on grass for the most part.

    I have a minor form of skeletal dysplasia in my family. (No, I don't know what kind, and no kind I have ever looked up fits. (I contacted a group one time that wanted to know about "undiagnosed a new forms of skeletal dysplasia and was told that the fully affected members of my family were not short enough. (I'm partially affected, or at least have a relatively mild phenotype.)) One of the effects is ankle instability because of bone deformations in my ankles. IT doesn't affect me much now, but when I was a little kid, I probably turned/minorly injured my ankles dozens of times each. As I grew up, it happened less and less, but I still have ankle instablity . . . I think I naturally altered the way I walked over time (Not an athlete) subconsciously. I wonder if Williamson has something similar.
    (And before you say, "he's tall", the affected men in family are/were average height and above. (I"m 5'10", my great uncle, who was more severely affected was 6'6" and even had the nickname "Big James." The affected female members of my family are/were 5'0", 4'10", 4'10" and 4'8".)
    Interrestingly, affected member of my family (including myself) also have distortions in proprioception.

    I rolled my ankle a thousand times, mostly on the way down on grass after playing some soccer. After that I figured out that I have great ankle flexibility, bc people told me it looked awful😅. I hope my guy is taking his time with this injury, I thought he would ball out from the jump

    I had a terrible sprain about a year and a half ago and it’s still not the same. Couldn’t imagine trying to get it back to the point of putting any kind of athletic force.

    Do you feel on top of ankle instability (maybe more things), it’s that most basketball shoes are narrow toe boxes, causing toes to not be fully optimized for motor control moving upwards to entire lower body.

    It's non contact, but to apply that bump was nothing, when he had no time to react at all and correct his leg between getting bumped and the landing
    The need for those exercises are real, don't doubt that, and Zion weight, power, play stile (that maybe he could adapt too, even while taking those especialized exercises, as Donovan Mitchell problably tried to do since his rookie season playoff ankle roll, but maybe developed some ankle instability later) put extra stress on his body, making an even bigger matter, but when those mechanisms are there with as little timeframe to react as Zion had, i have real doubts on how much would It help ?

    This happened to me all the time, My ankle would roll, I'd hop on in for a few steps, then it'd be fine. I have Ehlers-Danlos Syndrome and maybe Zion has it. Very stretchy tissue throughout the body. Makes you quicker and explosive, but more prone to injury.

    I had horrible non-contact ankle sprain where my foot ballooned up like crazy. I couldn't walk for months. It has been well over a year since then and my ankle is still fucked.

    Repeat ankle sprain made me quit basketball team when I was in College. I train so hard only to be injured in the first game of college tournament for two consecutive years.

    I just went through a horrible ankle sprain, almost my entire foot was purple and swollen. Been a few months and still have issues with it 😬 but I’m back to hooping on it with an ankle brace.

    To help correct that, he has to look down to have a better landing experience. Some Athlete's only focus on taking off and never look down when landing or taxiing the plane to a proper takeoff location. ( I Love my Airplane Analogy)

    Years ago in high school I sprained my ankle in basketball. Since then, I've rolled my ankle and sprained it numerous times, playing football, playing volleyball, many times just walking. It's so easy to sprain it again after the first time.

    Zion Williamson, unofficially the highest jumper during an ankle twist, it's incredible that he didn't just fall down after he rolled his ankle, but jumped like 18 inches after that.

    My feet cave inward so the opposite both knees poped out and knees caved in I fixed knee caving in and strong again but years later I need to get my knees popped back if possible.

    Could it be Zion's signature shoe? I find the new Jordan Zion 2 design has a chunkier forefoot midsole with Flimsy upper even with that forefoot strap.. Perhaps Zion could benefit from Nike's Foamposite technology like the Foamposite one / Foamposite pro which has a more rigid upper but will conform to his foot shape the more he wears the shoe. Just my theory.

    Tough call Doc. Not saying you are necessarily wrong, but just looking at the angle he is approaching the defender, it almost looks like he is trying to avoid contact and trying to sweep his left leg past the defenders knee. With that long step, it is almost impossible for him to get the angle to evert his ankle sufficiently to get him a solid base to plant properly. I think with his long step to avoid the defenders knee, he is adducting at the level of his hip which is what is causing a relative varus moment to his lower limb. This is the kind of move that you would typically see a guy slip and slide out and fall on the left side of his body, which I think from an ankle standpoint and for sure as it pertains to risk to his lateral ligaments as well as his peroneus brevis and base of the 5th MT (thank goodness it isn't his other side as you alluded to), is the safer thing to do.
    Again... not arguing with you that Zion may not have proprioception or not, but that is an awkward manoeuvre (sorry I am Canadian) as it looks like he is trying to slip past that defenders left outside leg. Just my take. Keep up the good work.

    1 or 2 years of Disuse atrophy always weakens the muscle memory as well..... to go from inactive to full sprint. That was a massive mistake.

    I knew that weird outrigger on those new Zion 2's would cause a bad ankle roll. The shoe is flat and wide in the forefoot but if you step on the side of the foot and it rolls its an automatic bad roll because it's so rigid. Hope it's not too bad.

    Doc, I was looking to see if you've remarked on Zion's non-contact hamstring injury a month ago, which has benched him ever since. I think they're not even planning to reevaluate until five or six weeks in. Seems to me that this is an even worse sign of things to come than the non-contact ankle sprain, but IDK.

    Never had a doctor take the time to explain this. I had surgery after a rolled my ankle and after that I thought I’d be good but I’ve pretty much sprained my ankle at least once a year since. I tried braces, wraps, and different footwear but it doesn’t help. I’ve gained weight and it’s even worse, and it’s getting to the point where I have to walk very slow and look at where I’m walking.

      Dunno if you have tried these but get into minimalist barefoot shoes, look into it at least. I know it sounds all hippy but the design is that ,all they are is a thin rubber sole and a wide toe box so your feet can act as tho they are barefoot. It will take weeks to get used to them and you need to re-learn to walk. But I've been wearing them for 6 months, I've not sprained my ankle since.

    Single leg RDL on a buso ball is what I do at least once a week. It’ll instantly reveal any ankle instability

    What. he got injured again? SMH. with regards to the ankle sprain yes. it had some bad effects when i had it in my middle school days my foot that got it was never the same and i can still see some deformity at the top of my foot that was a result of swelling.

    I had a injury like this about 9 years ago and now my ankle pops every time I rotate it because I didn't get it treated. I can still move the same but the instability is still there. After years is it possible to get something like that fixed or are my ankles done for?

      The popping might not go away, but it doesn’t technically mean much on its own if it’s not painful.
      But the stability could largely come back. The laxity of your ligaments is likely worse than pre-strain, but you can still strengthen the muscles. Let me know if you want guidance on that.
