  • Up to 40% of patients who undergo spinal surgery for lower back pain will develop postlaminectomy syndrome, or failed back surgery syndrome (FBSS). Dr. Josihi outlines the pathology, causes, diagnosis, and treatment options that primary care should know about.

    Wow. Way to mostly blame the patients. How about if the doctors do a better job with evaluation and diagnosis BEFORE surgery and if they work towards better procedures with lower rates of failure? Or, work on safer techniques or honing their surgical skills?

      ken seymour Totally correct as a retired nurse, over the years I have seen where the Insurance Companies and the Government tell the Physicians what they can do. New is on pain medication, but they even tell them on other drugs, i.e. antibiotics, how often they can prescribe other drugs, or if they write a script for one drug, but it isn’t the preferred You are penalized, and have to pay more for what was ordered or ask the Dr for one that is. Also the Government dictations how often you have to visit your pain Dr. For people on a fixed income with pain it’s not only difficult sometimes to make their appointments but can also a hardship financially due to the new increased visits. I Love this country, but exactly how many Laws have been enacted for our benefit but are really hardships.

      ken seymour There is also the case where a person can have a chronic tissue based problem (cervical stenosis and osteophytes, inflammation and scar tissues) that can’t be treated successfully surgically and these are a trigger for central sensitization flare up. This is a bottom up situation and In this case opioids work to calm and prevent the trigger source pain thus helping to keep the central system calmed down, keeping a lid on all the mind body top down symptoms. Sadly with the overreaction and irrational response to the “opioid crisis” the bottom up sufferers have lost the one tool that works consistently to reduce their top down suffering. Cannabinoids require a lot of experimentation to find the right one that works without the high or sedation. They may not be the best choice in bottom up Central Sensitization. The bottom line is these patients are complex and require an initial investment of time that most primary care practitioners want nothing to do with. So we are misunderstood and left to flounder and suffer.

    My failed back surgery was due to the surgeon not fixing the problem. He was supposed to do a foraminotomy with the fusion. My post op MRI showed my foraminal stenosis is worse. Did he pack more bone in there while I was sleeping??? I talked with my PCP, I can’t seem to get a second opinion as my surgeon will not release my records and without them no one will see me. My fear is that I’ll wake up paralyzed or incontinent, diapers and wheelchair in my future.

    • @michaelcarlin7171
      @michaelcarlin7171 Год назад

      Your surgeon CANNOT refuse to release your records. They are YOURS. Talk to the Medical Board in your State and/or the health system that employs your surgeon (unless he/she is private practice). File complaints if need be. See if you can find a local attorney whose specialty is malpractice and see if you can get them to write a letter demanding that your records released (nothing has to be said about a lawsuit, but the surgeon will recognize the names). If having the OR report is the most important thing to piece together, that can come from the hospital where it was performed, it doesn't have to come from the surgeon - but that may not be the only thing you need. Good luck.

      Hi Joann I had fusion L3-5 due to spondylithesis but actually had the surgery because of the instability. I had bilateral leg/buttock pain for at least 5 years. Had all the injections, pt, ablations etc. then it came down to stability and the choice was no longer mine. I went into the surgery being told this is for stability of the spine if we get pain relief it’s a bonus. It so happened as soon as I woke my pain was gone. My back pain was through the roof but the leg pain was thankfully gone. A week later I was doing great. 31/2 mos later it’s doing fantastic I still have issues but they are all due to atrophy from my muscles from walking around the way I was. It’s a long road but I’m hopeful. And I was petrified out of my mind. I’ve had every procedure you could think of and was never even nervous. This I was petrified almost cancelled 2 days before. Lol. Had L3-4 laminectomy L4-5 partial laminectomy in 1994 to relieve severe leg pain. Immediately Pain got worse post-op. Was back in surgery a week later to remove a subdural hematoma (surgeon nicked the dura with his equipment during the first surgery causing a bleed) on the spine at L4-5. The L4-5 partial laminectomy became a complete. Took a year to get back on track. In 2007 the back had become unstable. Had lumbar fusion with instrumentation L2-L5 at UCLA. Now in 2020 i have blown discs below and above the previous fusion and lose hardware floating in my back. In November 2020 will have fusion from T11-S1 with new hardware. It’s a bad road to start down. PS. Surgery was to be done April 2, 2020, but thanks to covid19 it was postponed as all non emergent surgery was. It could be an over stressed neurology. It depends on symptoms. Sit in a quiet place with a pad of paper and do a top down bottom up “inventory” of symptoms. Check your breathing and if your mind is full of racing thoughts or stuck on something you are “ruminating” on. Do you have pain every day. Do you have a variety of pains and discomforts? Do they move around and increase and decrease throughout your week. How is your gut?
      If you do have a “sensitized neural system” it takes time to come to a confident conclusion that it is the main problem. Most doctors are now aware that this does happen to some people and that’s good but they aren’t all ‘up’ on the specifics of it.

    Difference between syndrome and symptom….back pain post surgery is a symptom reflecting different possible causes, indeed wrong diagnosis or wrong decision of surgery (as surgery is a good business and creates good incomes), another cause which is totally missed or denied: allergies or extreme sensitivity to metal implanted. German surgeons have published more realistic statistics: 20 % of joint post surgery (with implanted material) are allergic reactions…but because Melisa test is not famous (not really appreciated by surgeons for any reason…)tests are not done before surgeries…. Allergies are causing tremendous pain….all over the body…and are only considered real if redness or heat are appearing around the wound…. Allergic reaction is inflammation in the joints, thereafter circulating in the entire body, becoming a metabolic cause…

    • @imveryhungry112

      What hes saying is that it might be good to look at other patient factors. Rather than doing surgery 1, then surgery 2, then surgery 3, then surgery 4. And the patient still be in pain after all these surgeries. Is that really bad medical advice? Statistically 40 percent of people are saying that they still have pain AFTER the surgery.

    Calling it failed back surgery is really deceiving and confusing. If you had fusion and it didn't fuse, then it's a failed fusion. But if you just still have back pain doesn't mean that the surgery failed. The fusion may look great.

Just think twice before you choose to have surgery. I never used a walker before surgery and almost a year and am still on a walker. Also have gone through rehab 3 times!

    From the point of view of a failed back surgery, i assure you, Dr., that i did EVERYTHING you could possibly do to prevent and prolong surgery. I dont appreciate stating it may be mental. I assure you my pain is real and would put you on your knees. It IS up to the surgeon to make sure everything possible has been done and the correct diagnosis made. I’m 38, almost 39, and know what my future holds. I would never go to a Dr. Like you. You’d be the one telling me its all in my head. In fact, i just had X-rays that confirm that my back is continuing to degenerate. I don’t know how you’ve gotten 302 likes on this video. Hope you have a blessed day.
