They got sloppy. Believing that all the Salamancas & Eladios cartel were dead. Who else would have come to see Hector? What reason would they have? An if Hector was to put the bomb on himself. How the hell would he do it? So basically it was their assumption to the pattern that let their guard down. An they felt they didn't need to personally check Hectors body or wheelchair. An Walt used that to his advantage after finding out Gus & Hector aren't old Cartel friends. That they are enemies. That the only reason Gus keeping him alive was only to get close up to him face to face & taunt him. Making him suffer until he dies. So it makes perfect sense as to why Gus's security didn't check the wheelchair. Nor did they try to see who else would follow up or even visit Hector. He thought he won & got too comfortable. They probably underestimated Hector and didn't really see him as a threat. He can't organize a hit and can't set up any sort of trap himself, so once the room was checked they likely thought they were in the clear.

Tyrus, taking illegal actions, was aware of the risk - for example harm, arrest, death (his case). From what we have seen in both of the series, Tyrus was an evil person who killed other people, as you mentioned it in your comment. Morality is highly personal value, but for me he does not deserve to feel sorry for him - he's just a regular criminal.
