Did anyone notice pharmacist Charlotte Green read "methotrexate 2 mg in 2 mL" to Dr Duncan Campbell when the label and paper chart clearly states "methotrexate 12 mg in 0.5 mL"?
Not really, but I did notice that when Dr. Robinson took the medicine out of the refrigerator, it clearly said: "Vincristine" but later on doing the checklist they passed it off as Methotrexate.
