I played Madame my senior year in high school and was the unintentional cause of the biggest laugh during the final matinee.
Because it was a cheap high school production, my costume as the box was a decorated cardboard box with my head, arms and legs sticking out of it. The "doors" (they were cut out of the flat side with one side being folded to make a hinge) were held closed with Velcro so they wouldn't go flying open until I needed to get things out of them. My torso was hidden by a long shirt disguised to look like a dress. It was overall a pretty neat costume.
It's the scene where Belle meets Mrs. Potts and the Madame and I'm chewing the scenery and being offered seconds. I get to the "let's see what I've got in my drawers!" line and try to open the doors...and they're stuck! I struggle and struggle and struggle and crack jokes with Mrs. Potts and Belle about watching the gray stuff and the pudding en flambe. By some miracle, the doors got unstuck and I thought "we're good." I then clear my throat and said "NOW let's see what I've got in them drawers!" And I yank.
Cue me yanking so hard that I not only send the dresses inside go flying out all over the stage floor but actually managing to rip the "hinges"!
The audience shrieks with laughter and I got a round of applause. Our choir director (who was conducting) was doubled over with tears in her eyes. Belle had to turn upstage so she could hide the fact she was laughing. Meanwhile, Mrs. Potts and I are looking with a mixture of horror and trying really hard not to laugh.
Once everyone had calmed down enough, I said the only thing I could think of..."How embarrassing" before turning my attention to the dresses that are all over the floor. I couldn't bend over to pick them up. So I had to say "I wore...that...when I last performed at the Royal Opera" while pointing to the dress in particular. When the scene finished, Belle had to gather the dresses and carry them off. Fortunately, the stage manager was waiting for me offstage with a roll of duct tape so we could fix the doors.
That evening, during the cast party, our director passed out her awards and I won "Most Creative Use of a Wardrobe Malfunction". I have a lot of fond memories working on that show.
TLDR: I tore my costume as the wardrobe. Hilarity ensured.
