I haven't seen the movie, but to explain a little: The two men have been trapped in a sort of "time loop" inside the stairway, key point being they still age. The other man decided long ago that he wasn't going to give up so he kept exercising and doing his best to stay in shape
Spoiler alert: This is an alternate dimension/time loop they are trapped in. They essentially are the life force of their identical counterpart on earth. If they die, the counterpart dies. Neither party knows this. The time loop provides a refresh of food, materials, etc. It does not remove the waste. For some reason this is done in pairs. When one dies a door appears to another location/loop with another person. When the person goes through the door they quickly forget there ever was a door. What the old man screams when he dies is him remembering the door and shouting not to go through, because it's essentially a trap. Whatever is fueling this wants him to go through. The only possible way to, maybe, break the loop is to not go through the door. The door appears right before this dude shoots himself. I vaguely remember that all being the case anyway. Basically these ppl be fucked.
