    My husband hasn't had gout in a long time. We were gluten and dairy free for years and then went grain free. We started keto and IF last year but went off during the holidays. I have celiacs and hashimoto's so I still stayed grain and dairy free. My husband ate wheat and some dairy. We were eating a lot of sugar over the holidays. He now has the worst gout flare of his life. He did the usual stuff tart cherry, ACV, no sugar, no grains, no
    dairy and no red meat. It gets better but flared back. I think because he fasted longer. So he is going to the doctors today. He is on week 3 of gout. I think your plan is reasonable. He has really bad gut. I think probiotics is good too. Ibuprofen is messing his gut up right now. He is 57 yrs old. I am scared that it could affect his kidneys or heart. He quit drinking alcohol a year ago.

    I dropped 53lbs implementing fasting 16/8. I eat Whole Foods with only water about a year now. All my numbers are great now but I’m still experiencing Gout attacks at times. I’m completely exhausted and confused. 🤦🏻‍♂️ so much to learn but I am willing to rid this condition.

    I cant tell you how much this means to me…..your content is just so excellent, and encouraging! My big challenge is that I am weaning off Prednisone, it is the only med that I have been able to take for chronic gout but it screws up everything I am fighting so hard to improve! I am about to start Allopurinol (yrs ago I couldn’t tolerate it) to see if I can tolerate it now at a very low dose. My Rheumy anxious to lower UA which is at 8 (down from 8.5 after strict Keto in 2021) suggested I take lo dose Pred concurrently while retesting Allopurinol.
    My goal now is to get back into Ketosis and get Ketone meter, get UA meter (HMO , only pay for a glucose meter) and after your article on weight bearing will see about PT as I am limited to chair exercise.
    I am ahead of the game re sugar and alcohol, but need fine tuning.
    Dr Pete: 1. did you have to experiment much with doses of Allopurinol, do you adjust daily, weekly, monthly? 2. I fell off the wagon this past Feb for a big birthday and within a week fell out of remission, big time spread of gout . Can one night light up those pathways? 3. How long have you been on Allopurinol?

      1) be careful about allopurinol. Honestly, the data and feedback about this drug is mixed. Remember that allopurinol doesn't operate on gout but rather is uric acid lowering and no one actually understands the role of UA in gout. Some people HAVE gout flares when they take allopurinol and there is feedback that once you start taking allopurinol you can't stop taking it because of the start up of flares. The theme of this video was eliminate the strong drivers and to get your lifestyle under control. This takes months. Consider quecetin and tart cherry. 2) Yes, one night is enough to cause issues because of the acute rise in intracellular UA followed by the acute rise in extracellar UA. 3) I've been on allopurinol for about 1 year. In retrospect I wish I had instead focused on quercetin and tart cherry.

    Im 36, completely fit and healthy (as far as i know).. but am hypereuracemic and fast for longevity and other health benefits. But now get gout regularly due to this. I havbe 2 questions.. 1. Is it likely i have an underlying disease like hypertension etc? 2. Can I / should I continue to fast, knowing it increases my uric acid level?

      It would be correct to say that you have an underlying predisposition to the inflammatory condition that drives the gout flare. The hypertension is an outcome like the gout flare. Fasting drives acute changes in uric acid and the gout flare loves large swings in UA. The fluctuation matters. It would be best to make sure that you eat a whole real food diet (no processed food or meat) and eliminate hyperglycemia, alcohol, sugar, and high UA meats like organ and processed meat (high glutamate, AMP, IMP, etc.). Eat a range of whole protein but not the same protein everyday. No grains, potatoes, rice,...anything that generates high glucose. Get the diet first, measure UA to know your trends, maybe bring back fasting after you are diet stable. Don't be in a hurry and be super consistent.

    Hey doc what about gout and prolonged fasting last time i fasted for 8 days wanted to fast for 14 but started really bad lower back pain went to ER and saw that my uric acid levels were 12.7 how can i manage this situation

      Unfortunately fasting causes large swings in UA. One reason is the large increase in ketones which compete with UA at the level of the kidney for excretion.

    I got bad gout from extreme fasting lost 35 lbs in 3 months … will I be normal ever again ?

      A great question but I suspect there will be no answer. This video is heavily targeted to people who have metabolic disorder, and not the affects of autophogy in relation to gout. I have an excellent keto diet regime established over 9 months and have lost 29kgs. So, like you, I want to inquire about high weight loss, and the linkage of autophogy to gout and how to stop it?

      @@sanpat2220 oh yes, to say the least. I’ve had three separate flares. The first two required colchicine (6 tablets over three days) to overcome the pain in my left ankle! (No toe joint involved) and the third flare is happening right now.

      Look into wearing a copper bangle around your wrist. Read up about it. That alone could solve your problem. But if you want to add other things as well: Also consume cream of tarter, 1 teaspoon a day for boosting potassium (don't go more than 2 teaspoons a day). You can also drink half teaspoon of bicarb before you go to bed with water on an empty stomach (not with food). If you were in South Africa you could also take Lennon's product (natural age old medicine) called Bruindulsies.

    I am doing Keto now for 9 month and also OMAD. I don't eat a lot of salad, but else I don't smoke, or drink alkohol and feel really good, because I lost more than 130lbs. But I recently got for the first time gout in my big tow and my blood work showed uric acid being 6,9. I don't want to start Alpurinol, but do you have any suggestions what to do? Or is this, because I lost so much weight?

    I’m on keto diet since last 5 months lost around 43 kgs but gout flare ups are coming atleast twice in a week. But I can manage with my work and routine work. What I need to do if I’m completely out of gout attacks? Please suggest

      When you begin a keto diet UA rises through keto adaption. Strong fluctuations or changes in UA favor flares. So step number one is to get stabilized in the lifestyle. This can take up to a year or more. Meanwhile take a critical look at your lifestyle. Make surer protein is a broad selection and that you do not suffer from sleep apnea.

    Do you have clinical data that shows this is effective? I tried keto and my bad cholesterol is high and good cholesterol is high now. 😅I feel worse. Idk I just want to have my cholesterol to be normal and feel more normal again.

      Not knowing anything about your lowcarb experience, your compliance, or how long you've been keto, it is typical for cholesterol to rise along with LDL in this lifestyle. It is understood now by the scientists and medical doctors that follow the science that cholesterol is not a marker of cardiovascular disease. The role of LDL is still up in the air but there is some recent data that suggests that LDL may not be an issue if you are compliant with the low carb lifestyle. Though, these results are preliminary. Remember, I am not a medical doctor and I am not offering medical advice. Not feeling well may have more to do with how you are transitioning into the lifestyle rather than the lifestyle itself. If its that you are uncomfortable with the lipid panel typical of low carb then maybe change your eating plan.

      CoQ10 improves mitochondrial function. It is the only mitochondrial supplement I take that has actually been shown to reach the mitochondria. I take it because I am extremely active and it improved my recovery time from exercise--I definitely feel better and recover faster. As with all things, it seems to work for me but that doesn't mean it will work for you.

    Give up potatoes? I’m good with everything else, but potatoes? Potatoes are a rich form of potassium, and potassium combats uric acid in the kidneys. So I ask, what is the science and proof to back-up such a contrary statement?

      Potatoes contain starch which is converted to glucose. Potatoes cause a hyperglycemic event as far as your liver is concerned which in turn can activate the production of fructose from glucose in the liver which produces uric acid. Foods that produce hyperglycemia should be avoided if you suffer from gout.
