  • In this grand rounds lecture Sarah Hallberg, DO, outlines recent research that suggests patients with metabolic diseases like type 1 and type 2 diabetes can achieve exceptional glycemic control using a low-carb ketogenic diet. She summarizes the current dietary recommendation for patients with metabolic disease and why this approach may not be effective, and discusses ways to help support patients in the long-term while maintaining this type of diet. Dr. Hallberg is the Medical Director at Virta Health and Medical Director and Founder of the medically supervised weight loss program at Indiana University Health Arnett.

    I've been on Keto for 2 weeks. My A1C was 7.6 (176 glucose) at the start taking Glimiperide and Metformin. Now after 2 weeks I just took a post meal glucose and it was 91 and I havent taken any diabetes meds in days. My ketone level was 1.6. Keto seems expensive. I said this to the cashier as she was ringing up my new keto foods. She responded "my best friend's husband just lost his foot from diabetes. Is keto really that expensive?" She made an incredible point.

      now try the same food that you used to eat that has complex carbs and now your blood sugar will spike even more keto makes the function worse not better and keto is one of the worst diets ever

      @@CncBlades You must be a vegan with an idiotic and ignorant comment like that. Actually my average glucose is 117, been on a hiatus for a month now due to a kitchen remodel. It's also stopped my afib and cured my digestive issues.
      Keto is one of the best diets. There are tons of people commenting on sites like these to prove it. Keto is THE ONLY diet that can help diabetes.

      It's not! At least to me it's not. Find the cheaper cuts of fatty meats. I used to eat a lot more protein (strength training) so I cut way back with keto. Add coconut oil, avocado oil, or butter if not enough fat. Buy frozen veggies.

      @@TInyK12 We're on the same page here. I don't do a whole lot of coconut oil but I do a ton of Avacado, olive and butter. Infact I do a shot or two of olive oil (yep, straight up like bourbon) every day.
      I probably eat too much protein on keto but I've been there too as far as strength training goes. Before the Marines, 30 years ago I was training to be a bodybuilder and probably consumed 3g per pound of body weight a day. My BMR was 8,500calories and my bodyfat was unmeasurable.
      Can't get enough broccoli, brussel sprouts and cauliflower. Now it's a pain to get out of bed some days.

    Why so many RIPs? People who die are gone from this world and are not resting anywhere. Sarah was a marvellous dietician and, like many others, helped me so much in my journey to rid myself of diabetes meds.

    Tried the non strict keto just low carb only and intermittent fasting, it works! Lost 19 lbs. in 6 months from 124lbs. to 105lbs. Skin rashes gone and digestive system feels better now!

    Good day Doc! Im into Intermittent Fasting "TMAD" Twice Meal A Day 16:8 Method for several months. Im TYPE 2 Dietetic for 22 years and on medication prescribed by my doctor. GLIBEN before Breakfast and METFORMIN after, GLIBEN again before Dinner and METFORMIN after.
    I'd like to ask if there is an adjustment for my medication schedule since its probably Lunchtime already when I eat food?

    Which is better? Keto or whole food plant based? Specially for diabetes. So much conflicting info online.

      Animal meat is a much more efficient source of nutrients and a solid protein to energy ratio. To get adequate protein would require a TON of veg.
      It would be extremely hard to get 1g of protein per ideal lb of body weight and stay low carb. And low carb is far better for diabetes treatment

      Do a Google search of long-term effects of ketogenic diet and you will see how dangerous her diet is. It might work in the short term but long-term it creates a lot of adverse health effects. This diet should only be undertaken under the supervision of a physician. Beware fad diets.

      She has Stage 4 lung cancer. She has been fighting it for 4 years. If you want the whole story watch the video she did this week with Peter Attia. She is really courageous and quite a fighter.

      @@guylainelanoie3283 she should go to the gerson clinic in California, they say they have been curing cancer patients for many years, my dr had breast cancer for 45 years she is still alive. There was a young man with stage 4 colon c, he went to the gerson clinic, he said they cured him the cancer is gone

    How do you prevent T2D patients from going into ketoacidosis? Is the UI ketogenic treatment program basically a gradual weening of carbohydrates with increasing intake of fats & protein through continuing monitoring?

      I was told by my Keto Doctor that You can only get into a Ketoacidosis state if You are Type 1 on insulin .

      Ketoacidosis is only possible in type 1 diabetics who do not inject enough insulin. Extreme low insulin causes runaway production of ketones

      I’m T2D and I developed hyperosmoler ketoacidosis. I had a few sick days and thought because I couldn’t keep food down, that I should not take insulin. Also because I was not eating. Ended up in ICU. So not just T1D can develop ketoacidosis.

      With extreme insulin resistance (type 2 diabetes) when your liver releases glycogen, it will not go into your cells, so it stays in your blood. When you eat pure fat, there is no glucose in it to raise your blood sugar BUT you liver might release glycogen, raising your blood sugar. Protein will raise it if you eat more than your body can use for muscle protein synthesis.

    Dr Hallberg was a true blessing! Imagine dedicating you life and knowledge to helping people! How unfair it is her dying from cancer? So many people in the world that have no desire or passion to help humans and they live long lives? She touched so many people, and its terribly sad to watch her so on fire and sharp as a tack, and a few years later gone? Life is marvelous, but surely unfair! Imagine if she lived, she would have had a whole lifetime of more strong work! A great loss! I think of singer Eva Cassidy that died in 2005 ! How s life so unfair- forgive me,we have a huge part of population that will never educate themselves, and/or dedicate thier lives to do what she did! She is so succinct in her delivery ! She is a true communicator! Darn it!

      I've seen it too much! The number one nurse at my dad's oncology office died from cancer. They say she was wheeled in, smiling, chatting, died a few hours later. How is that even possible! But I know it does. That's how my sister lost her father in law.

    RIP Sarah Hallberg. I started low carb/Keto after watching her Ted talk video almost 2 years ago. I have lost almost 100 lbs and feel great. I didn't know her personally but she had a great impact on my life and others that I know. A great loss to the world. RIP Sarah.

      @@Rob-vt1mz A lot of the pre-packaged foods are expensive (breads, snacks, condiments), but just make your own. There are a few very popular keto enthusiasts on YT that provide plenty of recipes. Best of luck to you! I'm about to re-start this week.

    I feel pretty strongly that the only way true progress is going to happen is to do to big food what was done to big tobacco. These companies know their products are lethal when used as directed. It is time to start suing.

    One reason why exercise seems to help with diabetes type 2 is that it uses energy. Insulin is also an energy storing hormone.
    So, it seems that fasting would help diabetes type 2 and insulin in using energy so the body becomes more insulin sensitive.
    One problem with fasting is that it can be done wrong. One's body should be fat adapted first to avoid some issues with fasting. Then, one can fast to have lower insulin levels, while keeping fasting more sustainable and safer. Then, the body can use its stored energy and become more insulin sensitive.

    There are also substantial benefits for mentally ill patients. I have issues with PTSD and delusions on carbs. On keto, my symptoms are reduced dramatically, probably largely thanks to the increased GABA. Makes bodybuilding a bit more difficult though. Even so, life saver.

    Im doing OMAD, the Dr mentioned an 18 hr eating period and mentioned a prolonged fast risked lean body mass. Asking for thoughts on PMAD.

    Just call it a MUFA diet. Not low carb. Brassicas, squash and melons are carbs but should absolutely be staples in the keto diet. Especially the brassicas and leafy greens, they should really be eaten DAILY at least. Just my opinion based on personal experience. ❤

    I went on here Dr. Hallberg’s Virta program for 6 weeks. Great program. Lots of support. I am off all blood sugar and blood pressure meds. But in six weeks on Keto (less than 30 g total carbs) I could not lose any weight! I left up 1/2 pound from the start! Not muscle. Didn’t exercise. I met two of my goals and now need to dig deep to try to figure out the weight thing.

      May watch some of Ted Naiman videos. P/E Diet.
      Stay on low carb. Increase protein and reduce fat intake. That should help start moving the scale. Hope that helps!

      My wife has the same issue. I think the hormonal difference slows weight loss. Stay the course. It will happen. You might want to add some walking and resistance exercises.

      Amazing that you got off your meds. Decrease your fat intake gradually each week until you see progress in your appearance. I don't recommend a scale.

      It's great that you're seeing progress on the medication front! Check with your doctor and see if maybe a fast or short term calorie restriction paired with exercise at your level.

    I been keto for a year. Was never obese but had a few extra pounds from poor food choices and eating too often. Lost 25 lbs and have kept it off with keto + IF. The only issue I run into is during a long Ride or run I feel like I do not have the power I use to have. How should a endurance athlete best fuel him/her self when doing keto life style?

      @@CncBlades Please elaborate how its the worst diet? I as well as many others have been able to keep weight down and feel good from doing this. As mentioned the only issue I encountered was low power on long endurance events. I have since corrected this by not being in a fasted state when I ride and consume foods like Dates, Figs, dried fruit and jerky while riding.

      @@g99se9 Part of my issue was riding in a fasted state. Adding food to fuel the ride makes a good difference and that includes some carbs from dates, figs and other dried fruit and jerky.

      @@mhaas281 alright well basically ketogenic diet is worse for your health because #1 ketosis isn’t supposed to be long term it is a state of heavy stress and while it’s actually amazing to be in ketosis for a little while it’s not meant for the long run and constantly stressing the body which increases cortisol and reduces sleep quality #2 the cause of diabetes is intramyocellular lipids(fat inside muscle cells)which if your eating high amounts of fat it is more likely to turn into fat instead of carbohydrates turning into fat#3 if you eat red meat on a ketogenic diet it’s also really bad because red meat is a type 2a carcinogen.I can provide more info as to why a ketogenic diet is one of the worst diets by the way it was also ranked 37th on best diets overall but yea

    Is this recent? Hummm...🧐 Dr. Hyman introduced her but I know his current patient rrecommendation is mostly a Plant based diet. I find this video a bit odd and antiquated in its “ presentation.... 🤷‍♀️

    What a gracious scientist ! Excellent presentation, despite too many interruptions by people pointing out “omissions” or asking questions they should have asked only during Q&A sessions.     Still didn’t get to the ROOT cause of insulin resistance. It’s bc too much fat is stored in places it shy be, like skeletal muscle, etc, which disrupts the way insulin can work at the level of the cell. Going keto just removes the sugar, great, but only going low fat actually gets to the root of the problem of insulin resistance.

    I started the Virta Health program in Nov 2021 and in 2 months lost 14 lbs and, more importantly, my glucose numbers are way down and I'm now off of Januvia. I highly recommend this program and the benefits of the keto lifestyle for Type 2 diabetics like me who have struggled with this issue for years.

      @@jmtennison2893 Yes, there is a cost (I think around $300/month) but my insurance company paid for it (Blue Sheild). If yours doesn't, keep to 30g of carbs or less/day. You can find lots of reliable videos by Dr. Sarah Helberg, Dr. Steven Kinney and Dr. Eric Westman to keep you on track.

    Hello I am insulin resistance I’m taking insulin 30 units and I’m taking him short acting which is 10 units twice a day and I started the keto genic about a month ago and it’s helping me feel good and it’s helping me lose fat fat loss just as well in my moods and I think you for your presentation I’m slowly working it out so thank you for your presentation it’s help me more in like my life or better than what the doctors would even think of

    KETO IS Great if you can do it! However I had Hep B as A Child, it messed with my Liver! To much fat! I also have false upper teeth so chewing meat can be difficult! I love my veggies but have to cook them! I keep trying to find my go to menu!

    What about if I'm vegetarian how can I help my wife and I lose weight I can't eat meat?

      Just eat salads with some nuts, seeds, and limited beans and legumes. Adding eggs would help.
