Reducing Swelling from Facial Injections or Surgery

    Hi Dr. I am starting week 5 of my deep plane face and neck lift, with fat transfer. I am a healthy women. I do not have high blood pressure, or diabetes, or anything else major that would seriously affect my health, or ability to heal. By day 5 after surgery I was extremely swollen, and bruised. I went into the NP to make sure I was ok, because I was also very stretched from the surgery itself. As well, I had a puffy, swollen, and very, very bruised area on my neck where the drain tube was. I was told my swelling was a lot, but everyone was different. A partner doctor came in and tried to suck fluid from my neck, and concluded I did not have a hematoma. So now at starting week 5 I am still swollen, but not as bad. The side of my neck that was so bad from the drain tube is still some swollen, and still bruised, but not as bad. The left side of my neck is developing these nodule things. And while I don't have my loose skin on my neck anymore, now the skin is so tight it is like creases everywhere, and it looks more like a fat neck. I can't feel my ears, and at the edge of my left earlobe and my face, I think there is a stitch still there and it is painful. I can't feel the outer edges of my face. They are still hard from fat, and being swollen. My temple areas are so tender, and something is off the way the skin got pulled up. I am not able to massage lotion from my forehead to my temple because extra skin gets bunched up and it pinches. I have to stop part way, let the skin loose, and then continue to moisturize my temples. Also, on the side to under my right eye there is now like a crease with a swoop down in it. This is painful and irritating. I think this is partly due to my skin being stretched so tight, and I can feel some fat transfer there that is still hard as a rock. On both of my cheeks in different areas, I have "holes", where it seems there is nothing under the skin. When I smile it looks like huge dimples in the wrong places. I was told the fat would soften and this would all smooth out. My entire face and neck are still so sore I have to be very gentle when washing or lotioning, or sleeping, or anything. Even just sitting, my upper cheeks, towards the outside that I can't feel, where the fat is still very hard, feel irritated all day long. I am scared these issues may be permanent, being as it is starting week 5 and I still have this swelling, bruising, hard fat, and pain. I read that sometimes it can be permanent. I am scared. I had my surgery on May 20, 2024. Today is June 24th. I see my actual surgeon on July 2nd. Do you have any suggestions for me, or thoughts about this? Please tell me if you know of something I should try, or just anything. I am desperate to feel better. Thank you for all of your videos! Angie

      Angie without seeing you it is difficult to diagnose the problem. Temporary numbness of the skin and ears is an expected outcome from a facelift where the sensory nerves are disrupted - the sensation will return as the nerves regenerate. Swelling following fat grafting is possible depending on the volume of fat that was grafted, and individual response to the fat. Swelling can also occur if there is a seroma formation from the salivary glands -sometimes this fluid has to be drained. Nodules can be from reactions to suture material, lumps of fat that formed granulomas or hematomas that have organized.I am sure that your surgeon can address each issue when you see him or her.

      It is possible that those are little scars or granulomas. Hope that is helpful.

    Dear Dr. Kopelman, I had a face lift on Jan 23. Everything was going well, each day looking a little better. Then on February 13 I woke up and there was a new area of swelling on my right cheek. It was oblong, and about 3 “ long, above the jaw, under the cheek bone. I still have it now, almost 2 months from its first appearance. I saw my surgeon about a week after it happened and he said to keep doing my facial massage. I have, including getting a jade roller and a gua sha stone and using the lymph drainage massage technique. Nothing makes any difference. Do you have any thoughts about this? It is ruining my otherwise lovely facelift results.

    After surgery swelling maybe normal but after 15 days swelling 😢 is it normal sir? Well I did Dimple plasty . And after 15 days it’s still paining and swelling too

    I was told by a cosmetic nurse that a small amount of prednisolone taken in advance of filler-procedure will drastically reduce the amount of swelling… Is this correct?? I don’t have fillers but have concidered it to adress vertical lip lines.
    Prednisolon is a medication I certainly don’t want to experiment with.. What are your thoughts?

      The swelling can not be significantly reduced by taking steroids in advance of an injection- most swelling occurs from the hyaluronic filler which attracts water or from a small amount of bleeding that may occur especially if she uses a needle rather than a cannula to inject Steroids decrease inflammation and good injection techniques don’t precipitate a lot of inflammation so the treatment is unnecessary Hope this helps!

    Hi Dr, love your videos as always. I am now on day 3 after my upper blepharoplasty and my right eye looks great. My left eye reacted VERY badly to the lidocaine (which the doctor gave me more of on that side because I was in pain) and swelled up LIKE CRAZY right after surgery. He gave me some steroid pills on day 1 but I am now on day 3 after the surgery and my left eyelid is still totally swollen (on top and below), so much so that I can barely open the lid. It looks like my eye is closed because it is so swollen. I am getting increasingly depressed by this... though I know swelling is supposed to peak on day 3 and then decrease I am SO SCARED it will just stay like that and my eyelid won't open 😵 have you ever had to deal with intense swelling on one eye that made it impossible to open the eye for a while? How long do you think it can last?

      Swelling can sometimes be asymmetric after surgery. If the swelling appears red and painful rule out an infection by visiting the doctors again.

      @@drkopelman It's not painful thank god but just still very swollen after 5 days with the eyelid drooping... I can barely open it. Doctor said he's only seen it happen twice before... Hard to be patient and not freak out, I even went to the Emergency clinic yesterday because I was so scared. I am on antibiotics now (even though they ruled out infection), they think it is just edema... so annoying! Thanks for your reply.

    I got facial fat transfer a few days ago and still very swollen it doesn't seem to be going lower

      It takes a few weeks before the swelling resolves but the fat may continue to evolve over several months

    How do You reduce swelling from sculptural injections

      Sculptra is mixed with sterile water and swelling is expected for the first few days following the procedure.As the water is reabsorbed the swelling will subside

    Hello I’m 6 weeks post op after having a deep plane and the lower eye swelling is still there? Is this normal? Also temples have a puckering through a nylon suture underneath will this go? I’ve just finished antibiotics with my GP. I massage the arrears but nothing is working why? My surgeon who’s in Turkey and I’m U.K. says it will go, but 6 weeks now and it hasn’t

      As per my knowledge 75% of the swelling will only go in 1st 2 month for the swelling to go completely (it may become un noticeable as days goes on ) it will take around 6 months to 1 year as per the surgery done and place of swelling

      Really depends on the patient. Some people are great candidates for surgery and others fillers are a wonderful option

    There is an easy solution to the problem of swelling from dermal fillers-Don’t get no fillers!!!
