Rather than the gorgon Medusa from Greek mythology, this lizard named Medusa is more along the lines of a basilisk. Basiliks in European folklore were lizards who could kill just by looking at you in the eye (hence why this lizard named Medusa always had that blindfold on her more human-like form. And the more human form, with the exception of the snake-like tail from the waist down, is a callback to the Lamia from Greek mythology).
So, gonna drop some Manga spoilers here. That "thing" you are always questioning is called Q chan, and is actually Count D's grandfather in a mythical disguise. According to the wiki, Count D's family are decedents of an ancient Chinese civilization who had close connections with animals. As such, they were taken into Chinese high society and seen as valuable assets by the royal family. However, when a woman of his people rejected a prince's hand in marriage, the prince became angry and ordered a mass genocide of the civilization. Only one man of the people survived and vowed vengeance against the human race for this injustice. To keep their people going into the future, they created imperfect clones of themselves to continue their bloodline. D's grandfather is the one who originally had the title of Count given to him and has since been just a brand of the shop. He lied to his grandson about going away on business and took an animal disguise to guard our Count (the one in the story) from his dangerous and very manipulative son, our D's father, who hates human beings with a passion.
