    The best discussion on this subject. This man should be teaching all ENT’s since I feel many doctors do not know the subtleties required for proper diagnoses. His knowledge is second to none. I was chewing gum everyday for two months, constant stress and lots of caffeine black coffee and developed muffled hearing on one side. Still not certain if it’s patulous or obstructed. I don’t get autophony but I also had no flu, cold or allergies leading up to this. So not sure if I’m mixed. Now thinking it may be also TMJ due to the daily chewing gum.

    i’ve had these issues for at least 4 years and has been finally improving for 6 months! so many tonsil stones and lymphatic clotting had to be cleared before i felt any relief

    I fit all the boxes (lost 90 pounds, allergic rhinitis, reflux). Was diagnosed with PET and tried everything for two years but nothing has worked. Now debating surgery. This has been helpful.

    My biggest problem is feeling like I am constantly in a tunnel, and hearing aids only amplify the problem. I often get sinusitis and I used to use pills with pseudoephedrine to clear it.

    For me there is no sense of fullness, just shooting pain each time the pressure equalizes. So, I have the obstructive kind. It hurts so much, and sometimes shoots every 30 seconds. Can last for days. I'm kind of desperate for a fix.

    Dr Poe, I hope you see this message. I have listened to your podcast and I’m wondering if there is a link to use of spironolactone and eustachian tube dysfunction/patulous eustschian tube. I am trying to figure out why I am experiencing my symptoms.

    Hi, I have PET since few months in both the ears. One ear suddenly started popping out of nowhere in the middle of the night. It was very scary. Recently, got operated few days before for left ear as the ear drum was more mobile. Dr. Dennis was looking my case and suggested Shim Surgery.

      How are they doing today? Have their eardrums been stiffened? Mine are so mobile that the movement doesn't let me sleep at night, in addition to the cracking when I swallow because the tube is open. Was he able to help them?

    I’ve been dealing with a vestibular issue for 2 years. I have hearing loss in one ear. The vestibular therapist basically said all of my symptoms come from my left ear. My main symptoms are lightheadedness, balance issues, heavy dependence on my eyes. I saw the ENT who ordered the VNG test which was cancelled and there is apparently nobody in my area who does this now. If I have a cold virus I feel pressure in the left ear. Sometimes my own voice is loud in that ear. Loud sounds can make me dizzy. Now my pcp thinks I may have a Eustachian tube disorder.

      I did have an issue similar what you had. I had hearing loss in my right ear about two years ago. Now I still have tinnitus on my right ear, feel unbalanced when I walk and clogged on my left ear. Also I heard a clicking sound in my left ear when I chew. My next plan is to find a specialist who can check my allergy and Eustachian tube.

    I was supposed to see Dr Poe back in May, 2018 but saw an associate ( I forget his name) at Bringham and Womens. Had all my records sent there from the VA and on that day their PET machine wasn’t working…
    Anyway, I have Autophony and after trying tubes I now have a bad infection!

    Does anyone recommend a specialist in Cleveland area? Very frustrated with lack of knowledge or treatment of this. It's been 10 years, and I'm certain it's patelous eustachian tube

    Recently was diagnosed with PET. Do patients normally regain hearing after their PET symptoms go away? I just had a grommet implanted to relieve pressure but experienced hearing loss - is this a normal side effect of tubes or is this just exacerbating PET symptoms? I'm just looking for some answers because the tinnitus/hearing loss/autophony are crippling me.

    I believe my severe Tinnitus is happening because of my Eustachian Tube remains in the OPEN position. When go into high elevation areas my ears NEVER equalize anymore like they normally would. My Tinnitus was caused by doing extreme breath work exercises, following a guided meditation. The retention breath hold phase caused too much pressure buildup in my head. Causing my Head to now have a 24/7 high pitched ring, and my autonomic nervous system to go into high alert, fight or flight mode. Extremely hard to sleep at night. Can Eustachian Tube be closed??? Or fixed to where it will function normally again??? I truly believe my onset of Tinnitus is from Eustation tube disorder. Any feedback will be most appreciated. Thank u!

      @@fynyxnation hey there, not yet unfortunately. I am hoping to hear from somebody out there who may have an answer to this problem. The only way I have been able to cope with it so far is by adjusting to it. I keep myself distracted, and I have learned to just try to ignore it. Put my mind on something else. Night time is really the only time I have issues anymore with sleeping. I have found a product called instaSleep that has done wonders for me. I am now able to get seven to eight hours Sleep uninterrupted. I found a really good RUclips channel and website by dr. Ben Thompson Treble health. He gives updates periodically on all the latest technology and studies of research that are out there. Hope this helps!

    I’ve been diagnosed with PET, but I get far more loud crackling than the classic autophony symptoms.

      It takes time to get in but if you go to your primary care doctor and ask for a referral or even going to urgent care they can also refer you to an ENT. I've definitely been there

    I have tinnitus 24/7 ear clicking popping like if I tie my shoe lace my left ear pops but my right ear will never pop could this be ETD

    Had shims for pet and I would never have done them should I have known that dr poe can manage his pet psychologically and not surgically. I really dislike that he doesnt talk about the facts that he manages chronic pet just fine without surgery and does not actually think surgery is an answer,
    At least that's what he tells a lot of patients after surgery fails.
    Why not say that before and save us money and time and stress?
    shims are not giving good results. For some reason almost everyone else I know who had shims had the same results I had : constant infections ,drum ruptures,mrsa,mastoiditis ,hearing loss and damaged ears after removal. Then we ended up with severe damage AND PET anyway! l have to wear hearing aids forever , have constant infections, mastoid problems, inner and middle ear damage. My drum collapsed. And we still have pet to deal with. My drums are so scarred up from the infections.
    The entire time us patients are suffering with PET 24/7, and think we need a surgery it turned out that what we need is to find a dr who will help us manage it like dr poe manages his - not surgery.
    He controls his pet, he does not have surgery on his. He says he can do everything with pet , and not from surgery.
    So why do them on us????
    We end up with pet anyway, and damage on top and hearing loss.
    So are the surgeries realistically just money?
    If we are getting damage, and still end up with pet, then it would be safer to charge to teach is how to deal with the pet like poe deals with it.
    If he can do surgery with pet and we are suffering from it , then he knows how to handle it and can teach us that.
    That would be worth it. Maybe not make them rich, but is a dr doing work because of passion to really help or because it's giving a good pay check ?

      I have had clicking/crackling in my ears for the last year, which started immediately after I got a cold. I used to think the clicks/crackles occurred when I swallowed, but eventually I realized that no, that wasn't so, but rather, they occurred every single time when I took air into my nose in order to breathe.
      So for that part, you may be having the same misperception as I did. This is easy to test: try to breathe through your mouth for a minute, and see if the clicks still happen as you are doing so. If they don't, then swallowing is not what is causing it, but rather, breathing through your nose is doing so.
      As for how to fix it: unfortunately, I have no clue, and I need to know that too. I have tried every home remedy described anywhere on the Internet, and I have seen my GP doctor about it, and an ENT specialist too, and they were both totally useless and offered me no useful treatment.

    I don’t get how these issues continue when we can’t just clean the sinuses close to the Eustachian tubes or even go in the Eustachian tube and clean it with the help of a camera that actually go through the Eustachian tubes to the middle ear. Ik we have technology to make this work y’all gotta push for these ideas bc in reality from what I read, it’s like doctors don’t do shit and let it linger for like years or months. Some docs don’t give you an automatic appointment to another ENT doc to help you, like doctors from what I experienced just are elite as you think or as we think the are, they are not top tier like that.
    I think I have mucous in my damn ears and I have post nasal drip daily and it’s not going away and my appointment is in UVA in JUNE. It takes a while for me to find a ent local bc not alot of people accept my insurance. This whole medical field is jsut weird as fuck to me. I think we’re not living in the right times or we are too dumb with this medical help. We should already have contacts with doctors 24/7 as a nation, we should be able to communicate with them from our home 24:7 and honestly I think it’s takes a long ass time to be a doctor I might be wrong but a lot of these surgeries and knowledge can be learned in a short time and I think future doctors should be able to help the world quicker than how we are being helped. We are so damn slow with shit. I think we let the patients jsut wander back in their lives with issues and even if they grow stronger from it sort-, they basically still suffer with their issues and doctors jsut say what’s wrong and don’t actually fix SHIT. a big fuck you to any docts jsut letting their issues linger bc you have an ego issue or you are too scared to admit that they are too much too handle and you don’t send them somewhere else the same day and give them an appointment bc ETD with ear fullness and tinnitus and vertigo and sound sensitivity and clicking and popping sounds AND wet sensations and itchy ness and some what annoying ear aches daily really take a toll on you. These issue changed my POV to the medical field bc they don’t do shit when you are down bad. The emergency rooms don’t know what to do bc they have no supplies or special equipment for ETD or ear issues like mine except for maybe some cover up medications. We need help with this shit. I also get boomy voice stuff too after an exercise.

      I also got blocked ears from sinusitis last year in the spring 2022. So yea I still got that and it’s worse.

      @@4kdubs they gave me a neil med rinse with medication to mix with it but I’m scared to do it it gave dizziness I did it wrong but ill try it again I made it go through my sinuses to my middle ear and it caused dizziness I messed up:/ but I’m still dealing with these issues daily it gets worse when I try to get into music it was soemthing I had HUGE goals with sorta!
