It is Not really a confusion but ever with the other tenses ( not with present simple) is very transparent but ever with do/ does (present simple) totally makes a change to the sentence. Previous to your explanation I had already asked another person about this topic. But Thanks for your explanation, now I kind of understand the connotation of ever in simple present questions. "Do you ever" is about the present. "Have you ever" is about one or more times in your whole life up until today.
Can we use the word "ever" on a positive sentence using present simple tense?
The only situation I can think of is if you use "If + subject + ever..."
For example: If I ever see him again...

I've never seen or heard that before. It's possible that they do that to make it sound more romantic/dramatic.
