Don't worry, it's just a bug. When the threat level rises, enemies may get a lifesteal buff. Some bosses incorrectly lifesteal off of inanimate objects, thin air, or their own parts when switching phases. In this instance, Joseph lifesteals off of the walls he obliterates when he drops. The devs know about it and a hotfix is in the works--it may have even rolled out by now but I haven't checked.
The funny part about that bug, if you get out the circle and dodge the attack then Joseph regenerates full health but if to take hit then he won’t regenerate.
