Sakuya the most iconic? I think that probably belongs to Flandre and Reimu. Cirno nowadays is also surpassing in popularity, despite the fumos starting with inu sakuya. Not too certain whether Reisen would be more widespread as well, but Momiji is probably more popular (without anyone realizing it). Then there also is Yuuka, who is definitely more popular. Popular in the sense of being widespread. In the case of Sakuya, only her Flowering Nights theme keeps making appearances everywhere, similar to Flandre's. also Cheeeen and Suika. And Okuu for some reason. Sakuya probably competes with Kaguya and Eirin in this regard.

So, I can say from the eyes of a newbie, Flan(dre) is hands down the Touhou character with the most "mainstream" exposure, maybe because her crystal wings and fanon derangement are so iconic. Who else gets a lot of exposure in thumbnails? Cirno, for sure... But I also see a lot of MoF characters like Suwako, Sanae and Nitori in thumbs. I'd say that Sakuya seems to be a fan favourite of many but only from within the fandom, maybe not the mainstream, as much... if "mainstream" is even the appropriate word to use. Just my two cents based on observation. As to Reimu, yeah, I saw the whole "Let's get her into Smash Bros" thing, but IDK... she's cool and all, but a bit OP and overexposed like Sephiroth is (I'm more of a Kefka fan myself)... Plus I don't find myself attracted to her lazy devil-may-care personality as a character. Marisa is much more interesting as a character because of her workaholic try-hard attitude. I see thumbs of her pop up from time to time, but you're right, not nearly as much as other characters. I guess you could say Marisa is to Reimu what Luigi is to Mario... Kinda. I still need to learn quite a bit more lore and separate the canon from the fanon.

I agree with everything you said, but just to clarify, I was talking about the Nintendo franchise "Super Smash Bros"... Seems lots of Reimu fans want her as part of the Smash Bros fighting roster. I don't think that will ever happen because of licensing issues. The way I understand it, I think ZUN wants Touhou to be available to everyone (within the parameters that he set). As I learn more about the whole franchise, I might adjust my tastes accordingly. As an aside, when I see grown men playing with Fumos, my mind automatically goes to that scene with Dark Helmet playing with his action figures in Space Balls. Whatever floats one's boat, I guess, but not my cuppa tea.
Also, I think it could mean someone's 3rd in Command. Like, The Devil's Hand could mean the Devil's most trusted servant, and then the Devil's Second Hand could be the Devil's second-most trusted. But I'm not 100% sure, since I thought Sakuya was indeed Remillia's second-in-command.
