"Agry Little Spad" is, I think, because of the difference in sounds in English and Japanese. The katakana of the name, romanised, says "agurii ritoru supaddo", which is the best approximation of Ugly Little Spud possible with the sounds used in the Japanese language. Re Japanese translation error. The Japanese litterally says "agrii", so it's wrong even in Japanese katakana. Also Spad is just Spud in the Japanese writing system. So clearly the English names were translated by someone who had never seen the movies and didnt know what a spud was.
The Japanese text just says the English words "Ugly Little Spud". The only error was when they went from Japanese back to English. Interestingly, the Japanese text next to "Ghost Runner" is the English words "Jogging Ghost".
It's a transliteration error rather than translation, due to Japanese writing having a more limited amount of syllables. Same thing that caused the Aeris/Aerith nonsense with FFVII
