    *Summary*
    This podcast episode features Dr. Judith Bruchfeld, an infectious disease specialist at the Karolinska Institute in Sweden, discussing Long COVID with host Vincent Racaniello.
    *What is Long COVID?*
    * *[**00:04:41**]* Long COVID is a syndrome with over 200 described symptoms that persist or develop after an acute COVID-19 infection.
    * *[**00:05:04**]* Symptoms typically last more than three months and can be constant or recurring.
    * *[**00:04:44**]* There is no definitive diagnostic test for Long COVID.
    *Common Symptoms:*
    * *[**00:06:13**]* Fatigue
    * *[**00:06:17**]* Palpitations
    * *[**00:06:17**]* Pain
    * *[**00:06:17**]* Brain Fog
    * *[**00:06:17**]* Dyspnea
    *Who Gets Long COVID?*
    * *[**00:21:07**]* Studies suggest 10% of people who have had COVID-19 may develop Long COVID.
    * *[**00:17:17**]* Demographics differ depending on initial infection severity:
    * *[**00:17:24**]* Hospitalized Patients: Mostly male (75%), median age of 57, often with pre-existing risk factors.
    * *[**00:17:47**]* Non-Hospitalized Patients: Mostly female (81%), median age of 45, generally previously healthy.
    *Possible Explanations for Long COVID:*
    * *[**00:08:47**]* *Viral Persistence:* The hypothesis is that the virus might not be fully cleared from the body, particularly in female patients.
    * *[**00:13:40**]* *Immune System Dysfunction:* Inflammation may play a role in Long COVID.
    * *[**00:16:09**]* Patients show signs of microvascular dysfunction in the heart.
    * *[**00:13:54**]* High and persistent anti-spike antibodies.
    * *[**00:15:59**]* Dysfunctional endothelial cells in blood vessels.
    * *[**00:27:00**]* Altered levels of CD8 T cells.
    * *[**00:18:16**]* *Hormonal Changes:* Hormonal shifts might contribute, especially in females experiencing menstruation cycle aberrations or early menopause.
    * *[**00:36:18**]* *Genetic Predisposition:* Genome-wide Association studies are ongoing to investigate potential genetic risk factors.
    *Treatment and Management:*
    * *[**00:10:04**]* Paxlovid, an antiviral, is being tested in a randomized trial to address viral persistence. Results are pending.
    * *[**00:21:50**]* Therapies are focused on symptom relief:
    * *[**00:21:55**]* Medications to slow heart rate and improve energy levels.
    * *[**00:22:08**]* Pulmonary and cardiac therapies.
    * *[**00:22:11**]* Physiotherapy for breathing problems and postural issues.
    * *[**00:39:44**]* Occupational therapy to support daily activities.
    * *[**00:40:28**]* Psychological support.
    *Key Takeaways:*
    * *[**00:37:40**]* The impact of Long COVID is significant and ongoing, affecting millions worldwide.
    * *[**00:23:58**]* Vaccination reduces the risk of developing Long COVID.
    * *[**00:38:27**]* Patients experiencing lingering symptoms should consult their doctor and seek appropriate support.
    * *[**00:41:35**]* Research is rapidly expanding to better understand the mechanisms of Long COVID and to identify effective therapies.
    *This is an evolving field, and further research is necessary to fully understand and treat Long COVID.*

    4y 4m for a family member. Because they were infected at the height of the epidemic but not hospitalized they were not tested, and many studies require positive diagnosis, meaning a test in the respiratory phase. No energy, housebound, gets headaches from displays (phone, computer, etc). They have to careful "count their spoons" (see Spoon Theory). They live in Kansas, among the worst places to live in the horrible US "healthcare" system. SSDI denied, too. We help, and it helps some. The _only_ thing that measurably helped was cutting out high-histamine foods...their favorite foods as a vegetarian. How long will we play this game that effectively consigns these people to death. It's called economic eugenics.

    I had a 15 day course of antivirals and nothing changed. I have very high inflammation numbers, for me, I think some type of medication to bring down the inflammation might help some. I was diagnosed with chronic lyme, reactivated Epstein Barr, ME/ CFS and lastly long covid. Everyone is different so I'm glad antivirals have worked for some.

    35:48 I think the connection with lung damage and some long covid may be hypoxia: this would lead to a consistent erythropoietin response by the body to get more oxygen, along with shutting down hepcidin production in the liver to keep absorbing iron. This would explain the early menopause and blood vessels like a 90 yo. Women react more efficiently to hypoxia than men, so that may explain the difference. And of course, GI damage with B12 malabsorption from covid would directly lead to anemia and hypoxia, with the same responses as above
