What is Leptospirosis? How does it cause Organ Failure? & How to treat?

  • This animation is about a Real Leptospirosis case I encountered during my training at Medical ICU. I talk about Microbiology, The initial symptoms, Complications & treatments for the disease. The causative bacteria are Leptospira, a zoonotic species that spread via the urine of infected rats, dogs & cattle. During the Immunological phase, the body produces antibodies against the pathogen. But these antibodies also attack the Liver, Kidneys & Other organs including blood vessels. The gold standard test is the Microscopic Agglutination test. The diagnosis is mainly clinical, & needs prompt treatment with Antibiotics (Cephalosporins, Penicillin). We arranged urgent Plasma exchange (plasmapheresis) for this patient as he developed severer pulmonary hemorrhages.

    From what I know abou leptospirosis, I feel it is clinically very similar to yellow fever. One physician (bacteria already had been discovered in his time, but no viruses) actually felt yellow fever _was_ leptospirosis, I think.

      Yes. Yellow fever has similar symptoms. But it's a viral illness.

    3 Days After Exposure, Mild Leptospirosis
    _1. Fever_
    _2. Muscle Aches_
    _3. _*_Conjunctival Suffusion_* - red patches in the eye; characteristic feature of leptospirosis

    7th Day of Illness, Sever Leptospirosis
    A. *Liver dysfunction or failure:*
    _1. Yellowish Eyes (Jaundice:)_ Jaundice occurs when there is excess bilirubin in the blood. It is a sign of *liver* involvement in leptospirosis.
    _2. Dark Urine (Coca-Cola-colored):_ Dark urine can also be a sign of *liver* involvement in leptospirosis.
    Other Symptoms:
    _3. Confusion_ - in severe cases, leptospirosis can lead to confusion or altered mental status.
    _4. Rapid and Shallow Breathing_ - pulmonary involvement
