Let me explain: Most health insurance plans require you to pay some amount of money (deductible) out of pocket before they start paying for your healthcare. Once you reach your deductible, health care is much more affordable. However, for greed related reasons, the deductible resets on January first, so everybody tries to get as many doctor visits and elective surgeries done before Jan 1st when their insurance deductible resets and they suddenly have to pay out of pocket for everything again. The CEO does not want people to get health care in December because the insurance company will have to pay for most of it. This is when I remind you that US health insurance companies make about 20 billion in profit every year

Wow.., I got a little distracted by the discussion of "that brain tumor" And it made me wonder which stupid insurance company owned the Medicare Advantage plan that my mother switched to before hers came back. I haven't the energy to try to find paperwork that's 23 years old But when I came home that day and my mother said something happy like I'm going to have a different doctor! Or my insurance is going to be cheaper! I had a really bad feeling about it. It wasn't wrong. But I was In my 40s and didn't have the first idea about Medicare. Even after three doctor visits of my mother where she was told she had high blood pressure, depression, and come back in 2 weeks and we'll talk about it some more (That was the time I sent my brother with her) I figured it was just her getting ignored because she's female. But finally she went to the grocery store and passed out in the parking lot. They had to pick her up in an ambulance so they took her to the ER and scanned her. And lo and behold, There was something inside her head. It still took 3 weeks to get an appointment with a neurosurgeon and in the meantime we lost power from a tropical storm and then she got pneumonia. Strangely at the time I was employed by a company that did their insurance with United healthcare. Maybe they were better in 2004? (I seriously don't know what company was covering her when the tumor came back and they couldn't be bothered to dig up her past medical records or you know, ask).

My son was born the day after Christmas. He came a few weeks early so he could stick it to the insurance company. It's the greatest Christmas present he'll ever give me.

My doctor told me he purposely posted my surgery on Dec 30th …just to piss BCBS off. Idk if that’s legal or ethical but he is one of the best I’ve ever experienced.❤ He truly cares for his patients.
Legal, ethical, and just generally awesome! It's hard to find doctors like this anymore. I remember when an Rx was not "supposed to be covered" and my doctors would tell me they'd be right back and get on the phone personally with my insurance company, I could sometimes hear them raising their voices all the way from the exam room. They'd come back in smiling and defiant and let me know "WE DID IT!", emphasis on "we", they loved to use that word. It's been so long since I've had an experience like that or like yours. I've asked my newer docs over the last decade if they could make that call and they look at me like I am from outer space. You got yourself a gem! I don't know why people get into the business of caring for people if they well, don't care about people.

I worked in billing department, which also handled prior authorization. We get WAY MORE denied authorizations in the last quarter of the year than any other time in the year. Funny enough that these denials usually are approved quickly in January of next year, when all the patients' deductibles and out-of-pocket get cleared and reset.

I actually like knocking my deductible out in January. I budget specifically for it, and it leaves the rest of the year open for imaging, injections, and surgeries without having to delay or stagger too much because of costs. But I'm super lucky to have a low deductible plan. Oh, and once you hit your out of pocket max? Absolutely 100% slam every medical thing you've ever thought about maybe needing into that year. Make your insurance company cry. It's the best gift you can give yourself.

Forever grateful to the ENT office who squeezed my daughter in for her ear tube surgery on Dec 31st because we had met the deductible that year. We found out she needed them literal days before that and they scrambled lol
In 2017 my neurosurgeon did the same with my neck surgery I needed because I was stupid and had an orthopedic surgeon operate on it first and he botched it. I talked to the neurosurgeon in late October and I had surgery over the Christmas holiday. That surgeon saved me.
We are SO BUSY the last three months of the year because all the people who have met their deductibles and OOPs come in for hearing aids. It’s like Christmas for us too because we get to do the “your out of pocket cost is nothing!” dance with them. I hate having to watch my patients struggle because they can’t afford help. Insurance coverage for hearing aids is the WORST

If there’s one thing we docs love, it’s screwing over the insurance companies that love to screw over us and the people we are trying to help.

I don't know if your baby was premature, but seeing Dr G's video on neonatology made me feel happy that all these passionate people are helping protect those tiny babies. My mom worked in the ER, and said his portrayal of neonatal nurses is hilarious and exactly right - she said they're the scariest people in the hospital, all to protect those delicate babies. I know everyone who works with kids feel the same way. They really care about what they're doing, even though they might lose some ability to speak to adults along the way.
I am so impressed by Bill's ( I hope it's ok to call you that) capacity to talk about real issues without boring his audience. As a physician myself, all these are incredibly frustrating to me and I feel sorry for our patients. It is mind boggling that we have such a system that has become such a burden on the healthcare. As somebody who just had an emergency with a burst appendix - don't wait until Jan 1st! 9 out of 10 docs recommend you have it right away! Also this was my bill: Ambulance to the hospital: $0 ER exam: $0 CT scan: $0 Six day stay in a single bed room, with my own bathroom complete with shower. Also had TV and free Wifi: $0 Emergency operations that lasted 2.5 hours: $0 Countless IV bottles of various anti-biotics: $0 Various other meds: $0 Food three times a day - and an open buffet where I could eat how much - whenever I wanted: $0 As many "gastro surgery dept" special - fresh strawberry and cream (with an egg yolk) smoothies - I wanted: $0 Grand total: $0 Thanks to the Norwegian health care system!
@@_an14 Since this is the gastro surgical dept. it is important to get people to eat (or rather - it is harder to get people to eat). Thus they have concocted this mix - fresh strawberries, a bit of cream (high percentage of fat), and one egg yolk (full of nutrients), some ice-cubes and they run it for a few seconds in a blender. It was very tasty (10x tastier than your regular hospital food) - and full of nutrients. It was served with a drinking straw in a tall glass, with a bowl of Ritz crackers on the side.
@@cbl6520 Americans spend more on healthcare than anyone else. But, people defend it because they are okay with spending $5,000 a year on healthcare if it means they don't have to spend $2,000 in taxes.
Hospitals can be just as bad as the insurance though. I once had a surgery in early Dec because I met my deductible. In February I got a bill from the hospital saying I owed them more money. They told me the price I paid was the previous year's price and the surgery price had increased in January.
Thank you so much united healthcare! I didn't know until now that I would get to drink exorbitantly in December if I became a doctor! I now fully know I wish to go into medicine. I am so glad that once I become a doctor I will simply be able to drink for an entire month and definitely never have to deal with immense bureaucracy that exploits people's desire to do good things for the communities they are in!

It's so true I can't even properly laugh. I remember going to a hospital ER once, being in one of the lowest points in my life. The first thing that happened was like $200 was collected from me by someone with a rolling kiosk card scanner that was operated by a very apologetic and depressed looking hospital worker who had the unenviable job of informing ER visitors that they now had a whole new set of problems waiting in the wings for after they were discharged. They were the insurance collectors. Not sure what their real titles are, but that is a job that would be terrible for your mental health. I can imagine. I didn't take that personally, but I thought it was a joke at first when they were asking me all these insurance questions when I was barely conscious. Like sure bro, here's my insurance card can you read it for me. Oh, I need to sign papers? Oh, and you need a couple hundred dollars from me? Okay, is there anything else I can give you? The deed to my house as collateral? My car title?. Just put it on my tab and I'll cry about it a few weeks later when the invoice comes through the mail. I got a call from some UHC rep a month later and they were asking a lot of questions that boiled down to: "we noticed you had a bad time. Sorry to hear you almost died. Is there anyone we can possibly hold financially liable aside from a broke peasant such as yourself? Aw, no? Okay, that's too bad. So we aren't going to cover this because it falls under rule XYZ section ABC. Have a great day!"

It's "truths" like these that make me soooooo happy to be Canadian!! I never understand when other Canadians complain about our health care. Compared to the US, we have paradise - unless you're rich, of course, and expense is no consideration! We do have deductibles - mostly with respect to prescriptions, glasses and dental. They're apparently luxuries and are covered by private insurance.

Yeah I can’t believe how in almost every country GLASSES/eyecare are not covered by national healthcare/insurance?! No coverage for dentures, hearing aids, foot care or many medical devices here in Canada, or, at least, Ontario, either. My dad was in his late 80's, when he needed all of the things you mentioned and I added. He and my mum were scarily close to being broke, by time his dentures were paid for. My early 90's mother needs hearing aids, new glasses and dental work. Can't afford it. I need dental work and new glasses. Not going to happen. Our health care system is good, compared to America's but not great.
Unfortunately the ones that complain are likely going to US for healthcare. Having several chronic health issues I’ve been in quite a few health groups and met lots of people from other countries who end up coming to the us and paying out of pocket Here’s an example Hyperparathyroidism Rather common condition especially for woman. It’s a tumor of the parathyroid gland. Treatment is a 15 to 20 minutes surgery to remove the tumor. In the US we have quite a few doctors skilled in diagnosis and treatment In Canada a patient who is suspected of hyperparathyroidism must go on a waiting list to see an endocrinologist. This can take up to two years to get into see one. The endocrinologists in Canada are far less trained in this condition and often take a wait and see approach. So the patient waits 2 years to be told let’s watch your condition There are fewer surgeons in Canada who do the 15 minute surgery. Surgery there can take 2 hours or more. The incision in the US is an inch. The incision in Canada can be across the neck digging around to find the tumor. So what doe hyperparathyroidism cause? Kidney stones, sever constipation, nausea vomiting , headaches , memory loss, confusion and 100% of the time osteoporosis When I had hyperparathyroidism I was able to ask my doctor to run specific labs, he obliged, refer me to a specific endocrinologist and then to a surgeon who had extensive knowledge in this condition. It took me about 3 months from diagnosis to get surgery I also have a chronic Csf leak and it’s pretty much the same scenario for Canadians who have this condition. Months or years to see a specialist who doesn’t have enough experience to properly diagnose. So they are watched, put on meds and bounced around on waiting lists to see doctors. No system is perfect and the US does need an overhaul of the medical system but I’m not convinced government healthcare is the way to go. I personally think private insurance companies are ok except we let them get too big and a few monopolized and pushed the little guys out resulting in a few big companies with too much control Government healthcare works for healthy people but can be a nightmare for chronic illnesses or illnesses that need experience and expertise. It’s definitely not perfect. If my two illnesses saw pretty much the same scenario play out from Canadian members in health groups I’m pretty sure it’s a problem with many chronic illnesses 😢
We had a case here in Montana where a family was trying to get help for their loved ones severe mental illness without much success. He finally did get into the state hospital (which is horribly staffed and underfunded like the rest of America). However, they discharged him because the insurance wouldn’t cover his stay and was brought home symptomatic still. His mom struggled to get him help and care he needed and it ended up tragically in her being attacked and killed while he was delusional and psychotic. I don’t understand how we are a first world nation, but our healthcare system sucks.
Hi from Australia. What's a deductable? What's an 'out of pocket maximum'? We have universal health care paid for by ALL taxpayers. So my Medicare deductable via tax on my income is 1.5% a year. If I didn't also have private health insurance I'd pay an extra 1% as I'm an over $100k earner. My insurance covers more than the basics - while not fabulous, it's way better than what the USA offers. Good luck people. America is looking more and more like a failed state every week.
I'm in Canada now and just finished a bacterial infection (first time in my life missing school/work) and my cost for three weeks in ICU: Zero. some pills. 20 bucks. I almost died. In the USA I WoULD BE DEAD. Grew up there. This has aged like milk.

This is way too relatable. I had severe pancreatice in January of 2022. Maxed out my personal deductible and out of pocket and didn't pay a penny for my shit load of meds this year. I'm not joking when I say I'm tempted to drink myself into hospitalization again so I can afford the medical bills for my wife and I next year.
It's good to see them donating to charity to write it off in taxes. This means they'll have more money that they'll definitely pass on to their members in lower premiums. No way they'll pocket those write-offs as profit.

Well, in both instances, I started radiation therapy in December, finished it in January...the difference in my bill between December and January was astronomical...if only the doctors could have pushed my radiation treatment to November...but yeah, I guess my point is that it's stupidmy financial liability differs depending on the month that I need treatment.

My doctor told me he purposely posted my surgery on Dec 30th …just to piss BCBS off. Idk if that’s legal or ethical but he is one of the best I’ve ever experienced.❤ He truly cares for his patients.
Legal, ethical, and just generally awesome! It's hard to find doctors like this anymore. I remember when an Rx was not "supposed to be covered" and my doctors would tell me they'd be right back and get on the phone personally with my insurance company, I could sometimes hear them raising their voices all the way from the exam room. They'd come back in smiling and defiant and let me know "WE DID IT!", emphasis on "we", they loved to use that word. It's been so long since I've had an experience like that or like yours. I've asked my newer docs over the last decade if they could make that call and they look at me like I am from outer space. You got yourself a gem! I don't know why people get into the business of caring for people if they well, don't care about people.

I worked in billing department, which also handled prior authorization. We get WAY MORE denied authorizations in the last quarter of the year than any other time in the year. Funny enough that these denials usually are approved quickly in January of next year, when all the patients' deductibles and out-of-pocket get cleared and reset.

I actually like knocking my deductible out in January. I budget specifically for it, and it leaves the rest of the year open for imaging, injections, and surgeries without having to delay or stagger too much because of costs. But I'm super lucky to have a low deductible plan.
Oh, and once you hit your out of pocket max? Absolutely 100% slam every medical thing you've ever thought about maybe needing into that year. Make your insurance company cry. It's the best gift you can give yourself.

That's the best way to do it with insurance. For the premiums/deductibles they charge, it's the best way to get your money's worth!
Forever grateful to the ENT office who squeezed my daughter in for her ear tube surgery on Dec 31st because we had met the deductible that year. We found out she needed them literal days before that and they scrambled lol
In 2017 my neurosurgeon did the same with my neck surgery I needed because I was stupid and had an orthopedic surgeon operate on it first and he botched it. I talked to the neurosurgeon in late October and I had surgery over the Christmas holiday. That surgeon saved me.
My daughter did the same thing for her daughter…genius!

We are SO BUSY the last three months of the year because all the people who have met their deductibles and OOPs come in for hearing aids. It’s like Christmas for us too because we get to do the “your out of pocket cost is nothing!” dance with them. I hate having to watch my patients struggle because they can’t afford help. Insurance coverage for hearing aids is the WORST

If there’s one thing we docs love, it’s screwing over the insurance companies that love to screw over us and the people we are trying to help.

To remove these events from the heinously oppressive soci-cultural context of what it's like for people trying to get healthcare in this country is to fail to understabd what's going on.

I don't know if your baby was premature, but seeing Dr G's video on neonatology made me feel happy that all these passionate people are helping protect those tiny babies. My mom worked in the ER, and said his portrayal of neonatal nurses is hilarious and exactly right - she said they're the scariest people in the hospital, all to protect those delicate babies.
I know everyone who works with kids feel the same way. They really care about what they're doing, even though they might lose some ability to speak to adults along the way.

I am so impressed by Bill's ( I hope it's ok to call you that) capacity to talk about real issues without boring his audience. As a physician myself, all these are incredibly frustrating to me and I feel sorry for our patients. It is mind boggling that we have such a system that has become such a burden on the healthcare. As somebody who just had an emergency with a burst appendix - don't wait until Jan 1st! 9 out of 10 docs recommend you have it right away!

Food three times a day - and an open buffet where I could eat how much - whenever I wanted: $0
As many "gastro surgery dept" special - fresh strawberry and cream (with an egg yolk) smoothies - I wanted: $0

@@_an14 Since this is the gastro surgical dept. it is important to get people to eat (or rather - it is harder to get people to eat). Thus they have concocted this mix - fresh strawberries, a bit of cream (high percentage of fat), and one egg yolk (full of nutrients), some ice-cubes and they run it for a few seconds in a blender.
It was very tasty (10x tastier than your regular hospital food) - and full of nutrients.
It was served with a drinking straw in a tall glass, with a bowl of Ritz crackers on the side.
So how much if your income went to taxes?
@@cbl6520 Americans spend more on healthcare than anyone else. But, people defend it because they are okay with spending $5,000 a year on healthcare if it means they don't have to spend $2,000 in taxes.
Hospitals can be just as bad as the insurance though. I once had a surgery in early Dec because I met my deductible. In February I got a bill from the hospital saying I owed them more money. They told me the price I paid was the previous year's price and the surgery price had increased in January.

Thank you so much united healthcare! I didn't know until now that I would get to drink exorbitantly in December if I became a doctor! I now fully know I wish to go into medicine. It's so true I can't even properly laugh. I remember going to a hospital ER once, being in one of the lowest points in my life. The first thing that happened was like $200 was collected from me by someone with a rolling kiosk card scanner that was operated by a very apologetic and depressed looking hospital worker who had the unenviable job of informing ER visitors that they now had a whole new set of problems waiting in the wings for after they were discharged. They were the insurance collectors. Not sure what their real titles are, but that is a job that would be terrible for your mental health. I can imagine. I didn't take that personally, but I thought it was a joke at first when they were asking me all these insurance questions when I was barely conscious.
Like sure bro, here's my insurance card can you read it for me. Oh, I need to sign papers? Oh, and you need a couple hundred dollars from me? Okay, is there anything else I can give you? The deed to my house as collateral? My car title?. Just put it on my tab and I'll cry about it a few weeks later when the invoice comes through the mail.
I got a call from some UHC rep a month later and they were asking a lot of questions that boiled down to: "we noticed you had a bad time. Sorry to hear you almost died. Is there anyone we can possibly hold financially liable aside from a broke peasant such as yourself? Aw, no? Okay, that's too bad. So we aren't going to cover this because it falls under rule XYZ section ABC. Have a great day!"

It's "truths" like these that make me soooooo happy to be Canadian!! I never understand when other Canadians complain about our health care. Compared to the US, we have paradise - unless you're rich, of course, and expense is no consideration! We do have deductibles - mostly with respect to prescriptions, glasses and dental. They're apparently luxuries and are covered by private insurance.
I have many relatives in rural Alberta who are like that. I keep saying to them "Are you crazy eh? Be glad you don't live here in the US."

Yeah I can’t believe how in almost every country GLASSES/eyecare are not covered by national healthcare/insurance?! Like 50% of people would be unable to function properly without glasses or contacts: society would collapse if we all just stopped wearing them at the same time cos nothing could get done properly, what the hell! 😂 people wouldn’t be able to drive/read/find things etc.
No coverage for dentures, hearing aids, foot care or many medical devices here in Canada, or, at least, Ontario, either. My dad was in his late 80's, when he needed all of the things you mentioned and I added. He and my mum were scarily close to being broke, by time his dentures were paid for. My early 90's mother needs hearing aids, new glasses and dental work. Can't afford it. I need dental work and new glasses. Not going to happen. Our health care system is good, compared to America's but not great.
Unfortunately the ones that complain are likely going to US for healthcare. Having several chronic health issues I’ve been in quite a few health groups and met lots of people from other countries who end up coming to the us and paying out of pocket
Here’s an example
Hyperparathyroidism
Rather common condition especially for woman. It’s a tumor of the parathyroid gland. Treatment is a 15 to 20 minutes surgery to remove the tumor. In the US we have quite a few doctors skilled in diagnosis and treatment
In Canada a patient who is suspected of hyperparathyroidism must go on a waiting list to see an endocrinologist. This can take up to two years to get into see one. The endocrinologists in Canada are far less trained in this condition and often take a wait and see approach. So the patient waits 2 years to be told let’s watch your condition
There are fewer surgeons in Canada who do the 15 minute surgery. Surgery there can take 2 hours or more. The incision in the US is an inch. The incision in Canada can be across the neck digging around to find the tumor.
So what doe hyperparathyroidism cause? Kidney stones, sever constipation, nausea vomiting , headaches , memory loss, confusion and 100% of the time osteoporosis
When I had hyperparathyroidism I was able to ask my doctor to run specific labs, he obliged, refer me to a specific endocrinologist and then to a surgeon who had extensive knowledge in this condition. It took me about 3 months from diagnosis to get surgery
I also have a chronic Csf leak and it’s pretty much the same scenario for Canadians who have this condition. Months or years to see a specialist who doesn’t have enough experience to properly diagnose. So they are watched, put on meds and bounced around on waiting lists to see doctors.
No system is perfect and the US does need an overhaul of the medical system but I’m not convinced government healthcare is the way to go.
I personally think private insurance companies are ok except we let them get too big and a few monopolized and pushed the little guys out resulting in a few big companies with too much control
Government healthcare works for healthy people but can be a nightmare for chronic illnesses or illnesses that need experience and expertise. It’s definitely not perfect.
If my two illnesses saw pretty much the same scenario play out from Canadian members in health groups I’m pretty sure it’s a problem with many chronic illnesses 😢
Plus, if you hold onto that baby, you might win the January 1st New Year's baby pack, complete with a year of diapers and a lifetime of medical bills!
We had a case here in Montana where a family was trying to get help for their loved ones severe mental illness without much success. He finally did get into the state hospital (which is horribly staffed and underfunded like the rest of America). However, they discharged him because the insurance wouldn’t cover his stay and was brought home symptomatic still. His mom struggled to get him help and care he needed and it ended up tragically in her being attacked and killed while he was delusional and psychotic. I don’t understand how we are a first world nation, but our healthcare system sucks.

Hi from Australia. What's a deductable? What's an 'out of pocket maximum'? We have universal health care paid for by ALL taxpayers. So my Medicare deductable via tax on my income is 1.5% a year. If I didn't also have private health insurance I'd pay an extra 1% as I'm an over $100k earner. My insurance covers more than the basics - while not fabulous, it's way better than what the USA offers. Good luck people. America is looking more and more like a failed state every week.

This is way too relatable. I had severe pancreatice in January of 2022. Maxed out my personal deductible and out of pocket and didn't pay a penny for my shit load of meds this year.

Like you, Dr. Glaucomflecken, I've had cancer twice (as a young adult). Well, in both instances, I started radiation therapy in December, finished it in January...the difference in my bill between December and January was astronomical...if only the doctors could have pushed my radiation treatment to November...but yeah, I guess my point is that it's stupidmy financial liability differs depending on the month that I need treatment.
