  • Educational video describing the condition known as mallet finger.
    A mallet deformity, or drop finger is caused form a blow to the finger at the DIP joint that forces the joint into forced flexion. The injury typically occurs from playing ball sports such as baseball, football, and volleyball. The patient is unable to straighten the DIP due to an injury at the tendon insertion. The finger is bent in hyperflexion and looks like a mallet due to disruption of the terminal part of the extensor tendon from its insertion into the distal phalanx and the patient is unable to straighten the DIP. The patient will have a flexed finger and will be unable to do extension of the DIP. The injury can occur as an avulsion fracture, bony or purely a tendon injury.
    An x-ray may show avulsion of the distal phalanx or subluxation of the joint.
    Treatment
    •Extension splint of the DIP for about 6 weeks or more is the usual treatment
    •Movement should be allowed in the PIP joint.
    •Night splinting may be needed for longer periods.
    •Indications for surgery
    oVolar subluxation of the distal phalanx
    oAvulsion fracture with a large joint fragment more than 50%.
    oAn open injury
    Mallet finger with subluxation of the DIP joint is an indication for surgery. May require open or closed reduction and pinning of the fracture or the joint. A single pin is usually sufficient for the treatment of a pure tendon injury. Alternated method for pinning of a mallet with fracture
    Complications:
    1-The residual deformity that usually does not affect the function.
    2-Swan neck deformity: care must be taken during treatment to avoid this deformity and the PIP should be moving freely.

    In mid may 2017 I injured the tendon of my hand little finger. The distal phalanx wouldn't straighten.
    I got a Stack splints and kept it for 6 weeks without removing it.
    After that time, on 6th July, I removed it. The little finger was straight for about a few days, but then it would bent at about 15 - 20 degrees.
    I was disheartened. The doctor said it would stay like this and there's nothing we can do.
    I couldn't agree with this. Instead of throwing out the Stack splints, after about a week from removing it, I put it again for another 5 weeks and I underlaid a little piece of fabric / cloth / cardboard paper in the Stack orthesis right under the top part of my little finger (distal phalanx) just to keep it a little higher than normal.
    And then in mid August I couldn't believe it. It worked. My little finger was straight again and has remained like this till now - two months after removing the Stack orthesis.
    I must say that I prayed to God and some saints every day to help me recover. My prayers were heard. It was a miracle!
    Therefore, do not abandon hope and keep the faith.

      I'm glad to hear your finger straightened out, I did this same injury to my little finger when I fell off my bicycle. I wore a splint for six weeks but after that I still couldn't straighten the finger, my doctor wasn't interested and just said "oh well, I don't think we can really do anything about that". So now I have a clumsy, deformed finger which looks wrong and feels wrong inside. I try not to think about it but it bothers me at work and sometimes when I go to put my hand in my pocket my finger catches on my jeans. I know it could be a much worse injury and I am thankful for what I have but man I wish my finger could go back to how it used to be. Thanks for your encouraging words, hopefully a hand specialist can help me.

      Unfortunately, i've got the same injury, while practicing sports. After two months of treatment, my finger has become capable to move and so son. However, I still feel pain in my finger. And there is yet one year that I've got this injury. I've lost my hope to become totally healthy again and, perhaps, I will be enforced to leave sports that I really love, like boxing and bjj. Sometimes, life sucks.

      Thats great news!! The tendon injuries are always tricky and often require that little extra time in the splint.

      Woah chill yours is obviously not that bad. I had to have 4 metal rods and screws for my mallet finger from a surgery they did. I had to have two surgery’s for this. And I didn’t tell anyone for seven weeks

      This just gave me so much hope bc my doctor told me there was nothing I could do after two months post injury and I'm still splinting it bc I don't believe that to be true either. I read that if you take off your splint or don't treat it right away you just have to wear the splint for longer

    had a bony mallet finger last august, hitting my pinky while loading my suitcase in the car going on holiday... didn't take it seriously, thought if it was broken, I wouldn't be able to bear the pain. cut long story short 2 weeks after doctors said, you 're too late in... bone has started formulating but broken part of the joint is off. you need surgery... surgery done 3 weeks after injury... 1 k-wire in and casting for 5 wks. doctor takes it off. physical therapy started to get the DIP joint moving, 3-4 days a week for 4 weeks... 24 sessions after, the bump on the joint does not subside.. we take an x-ray... joint has healed but bony fractured little piece did not fully sit in the joint... partly out... 50% of times i flex my finger, i hear a crack as if my cracking a knuckle. lag is not too much but grip and strength of the pinky are impaired... very expensive lesson to take health issues seriously and act immediately...

    I had a low impact version. Sliding my fingertips across a car seat and it got caught in a wrinkle in the seat. I heard a loud pop but it wasn't painful at all. I continued the last thirty minutes of my work shift hoping it was temporary. I got home and self diagnosed my mallet finger.

    Very nice video demonstration. Couple questions though: how does a pure tendon injury differ in terms of healing time? And how can people identify if a pure tendon injury is a complete rupture or just a torn/stretched tendon?

      Not a doctor but just what I learned while doing my research. If you can't move the appendage then it's ruptured. Small tendons like the one in the finger are difficult to partially rupture.

    Hi there thanks very much for your video. I had this injury for 5 weeks but didn't go to get the treatment done until last week and they said that i have
    have mallet finger injury in my index finger caused by intra-articular fracture of the distal phalanx. They recommended me the stack splint but is more longer and is covering my other joint that is not needed and is infact bad. Frankly it is not comfortable and the skin has discoloured etc.They also warned me that I am 4 weekks late so they don't know if it will be straight enough now. I asked them for a better splint which allows more breathing and can let me type as well. The answer was that they beliee in stax splint and they have no time to search on the internet for find me an alternative. I know there are more comfortable splints available but I can't take the risk more as I am already late in the process and I cannot be sure which one would really works. If you can please advise, I would really appreciate that.

      I just received the Oval-8 finger splint. I had to research it myself because the doctor only recommended a traditional splint with tape which fell off every time I washed my hands.

    I do have 8 mallet finger since childhood.can it be treated with splint or sugical treament is needed

    This happens to me 2 years ago,because i am playing basketball and i got the ball hit to my ring finger while trying to catch it. This makes me weak just by watching it because i remember what it feels

    Idk if this is the same but my is worse, it happens to me in my middle finger and It happens in between the proximal phalanx and the middle phalanx, also between the middle phalanx and the distal phalanx

    I'm three weeks in a ten week treatment plan. I'm noticing that I can lift my fingertip underneath the tape. I have one of the cheap splints they just cut off pieces from a strip and bend then tape. Yesterday I noticed when I stretched I felt the tip lifting up. Which means it's good and connected. I'm also noticing a lot of itching going on in there. Same spot right along the top of my finger. Another sign that it's healing properly but me being able to feel it lifting is a very encouraging sign. I was so worried that nothing was happening lol. Especially after I let my finger slip down low in the splint one day and it was bending a little bit. Thank goodness it wasn't bending too much.
    Before the lifting and heavy itching there was slight itching going on all week. Now it's to the point I'm scratching for more than a few seconds at a time.

      The Oval-8 finger splint allows you to scratch it. There is no tape involved.

      @@jaybloomfield5082 lol too late now but if it happens again I'll know. I just took what the doc gave me. I had to wear it for eight weeks. I ended up getting my full range of motion back. And I attribute that to me exercising it whilst it was in the splint. Throughout the day I would just lift it as high as I could against the tape.

      Thank you for your reply. I am under the impression that the finger has to remain 100% rigid in order for the tending to heal itself. By exercising it and moving it, weren't you continuously tearing the tendon again?

    In about 2014 I injured my finger and it turned out to be a mallet finger. I wore a splint for quite a while and checked in about every two weeks with my surgeon, and I did exercises whenever I could remember to (I was twelve so I wasn't that serious about it). My surgeon decided that I could do without the surgery and my finger would be alright. But recently my finger has begun to sting so bad I cried today. And the joint just near my nail is very hard and it feels as though you can feel my bone. I'm beginning to think it didn't heal properly but I'm so confused if that's just normal.

      Hey sorry to hear I have the same injury. Generally with bony injuries surgery is decided depending on how far the fracture extends into the joint line.

      Definitely not normal. I had a full recovery with none of those symptoms going on. Mine was about 3 years ago now.

      @@BodyFitPhysiotherapy mallet finger is not a bone injury it's a tendon injury that has the potential to break some bone off depending on the impact.

      Thanks @@ThePrufessa however from my understanding a Mallet Finger can be the result of a fracture or rupture of the tendon.
      In some cases there is an avulsion fracture of the tendon where the tendon is pulled from the bone. Either way the extensor mechanism of the DIP has been impacted.

    My fourth finger from my thumb on my right hand is dent like that, and I think it was since only just recently too... Maybe a few weeks? I don't know.. But it doesn't hurt though, and I'm sure it it was never like that before. It's not as bent as it's mostly shown in the vid though.

    Hi i have a question please reply: I have broke my mallet finger before 7 weeks the broken place is in the middle its diagonally / so the doctor made a surgery and now its seems very good but my finger its kinda numb is that normally my doctor said that after the wound is heald i need to start with exercise/ therapy and i wanted to ask you about my wound should I cover it with bandage or should I leave it uncovert thx

    mallet toe ,, may be not tear in tendon ,,,, may be weakness in finger extensors ,, And the physiotherapy can treat this with elctrisity , like faradic or diadynamic current ,, and the splint ,, w should avoid the surgery ,,,
    and some current make the healing faster , like Mes current

    I'm not sure if I have mallet finger but I had a huge volleyball game yesterday and I set the ball completely wrong. I felt terrible pain but brushed it off and kept going. but when I got home I looked at my finger better and it's curved and doesn't want to straighten unless I force it. the tip joint hurts terribly if I move it even slightly. even though the finger is curved I can still move the finger as a whole. But bending hurts so much. HELP I HAVE ANOTHER GAME MONDAY AND I CAN'T AFFORD TO NOT PLAY. PLEASE ANYONE. SHOULD I SEE A DOCTOR!?!
