It's both. When you create your save file the game will pick either the inner hall or catacombs as the place where crow mauler can break through the wall, but also, crow mauler will despawn when you enter mahabre.

i dont really know about funger speedrunning so i might be wrong but wouldn't save scumming for an empty scroll at the start to get phase step save quite a bit of time?

LRT pauses the timer during loading screens. That's why I use 2 timers now. One is real time and one is time with load times removed.

Sub 6 would require new strat but sub 6:10 is probably possible with better catacombs and no mistakes.

In-game time vs real time, notice how the 2nd one pauses on loading screens. Prob to even out scores when using different hardware.
Yeah, it's there to even out the difference. Even with decent computer the loading screen can be kinda long sometimes, especially when using obs.
Maybe fear and hunger should get a hololive edition after many months.
