As for my theory, I think that disc 5 comes before disc 11. When we hear disc 5, we notice that the character is walking inside caves towards the ancient cities and trying to open a portal, but he encounters the warden and tries to escape from him through the portal .... and here begins the story of disc 11 inside the portal

The sound at 0:06 too. The wiki describes the disc as containing "haunting sounds," and I think these sounds are what it's referring to. The sound at 0:09 especially. It would almost be like an earworm, repeating over and over, sometimes leading to the rest of the track, but those sounds. They really do haunt you

@@camera2178 No, the Warden was specifically based on Disc 11, so retroactively the monster is the warden. Also, "Herobri-" is never said, disc 11 was added before the Herobrine was first created.
The 1:01 sound is way high pitched for warden. Warden can be inspired from this, but the monster here is enderman

If you listen closely at the start, you can hear a kind of deep clicking like the sound the warden and sculk make. This is a very interesting part of Minecraft and maybe it contributes to the lore of the game. I hope they add more broken discs like this, it makes the game feel a tad more suspenseful and has me hoping for a minecraft storyline.

Snail Farms wasn’t a wolf that killed him. Obviously an Enderman as you can hear him earlier in the disc. He’s in a cave, fiddling with a map and a compass trying to orientate himself, he coughs after he lights a fire with flint and steel. He hears/sees an Enderman and runs out of the cave. The end barking was either:
A: His dog
B: The Enderman
C: The sound of the disc shorting out.
Either way the Enderman killed him and the transmission cuts short before anything else is heard.

Greedo well a lot of this aren’t sounds in the game, like the *coughing*. It sounds like barking which why we can assume it could be his dog. But its not confirmed. It could be the other two that i listed. There’s no clear answer.
Greedo I’m completely aware that of the differing sounds, but logically it is an Enderman that killed him, as we both here the Enderman, and this CD was released in the same update as the Enderman.

He digs himself up, first stone and then dirt at the surface, he hears his dog barking right before he dies to the enderman he was running from.

Steve came across Enderman (that at the time it was the new mob) he didn't know he couldn't look into its eyes so Enderman gets angry and Steve ran to the cave while Alex was recording disc 13 and Steve lighted a lighter and he coughed due to smoke, he mined until he came across Enderman and Husk he turned on the page until he finds the information about Enderman so he warned Alex that she should run away from Enderman and run to the desert while he fighted with Husk then Alex tries to run away from Enderman but she came across him and so he kills her and in the middle of the battle against Husk, Steve dropped the disc so the disc 11 was broken.

The story of Minecraft's disk 11...
(as told by my crazy imagination right now)
I knew I shouldn't've come here. I knew it was too dangerous. And yet, here I am, in the dark, all alone. Well, mostly. Of course, I knew there would be zombies, skeletons, spiders, and possibly Endermen, but never in a million years did I expect this...
This all started a few days ago. I was in the overworld, above ground, looking for some mushrooms, when I saw something... strange. A beam of sorts, shooting straight up into the sky as far as the eye can see. Normally, I would not be one to seek adventure, but this intrigued me, so I decided to investigate. As I drew closer, I saw a green, grassy clearing. Right at the edge of the clearing, I stopped. Someone was there.
"But that can't happen, because this is a solo world," I thought to myself.
The person was standing near a large, iron structure with a teal box on top. They didn't seem to be doing anything strange, just mining wood and building a home. But this was strange. No one else was supposed to be there because it wasn't supposed to be possible. And yet, there they were. Their hair was brown and short. Their skin was tan, and their shirt was blue.
I swallowed my fear and took a cautious step into the clearing. Then another.... and another, until I found the rhythm of walking. The closer I got to the person, the less afraid I was. Once I reached their structure, I drew in a deep breath.
"Hello!" I said with a smile.
The person didn't say anything and continued to gain wood. I figured they hadn't heard me, so I took a step closer and repeated myself.
"Hello!"
Again, nothing. At this point, I was sure they heard me. Frustrated and no longer afraid, I walked in front of them.
"Excuse me? Who do you think yo-"
I stumbled back and screamed. Backing as far away as possible, I cowered and tried to tell myself it wasn't real. It couldn't be real because it wasn't possible. They had no pupils. Their eyes were pure, blank, white.
I summoned the courage to take another look, and sure enough, there was no color. The person continued to chop at the trees as if I wasn't there.
At this point, I was more than a little afraid. I was terrified and ready to run, but I needed to know how they got here.
I stood on shaky legs and approached them. Once I was only a few feet away, I spoke.
"H-hello?"
Still, they didn't react. I stepped in front of the tree they were mining. They stopped swinging, and their ax fell to the side.
"Can you see me?"
I waved my hand in front of their face, but they didn't react. At this point, I wasn't sure what to do. For a few minutes, we just sat there, staring at each other. Right before I was about to leave, they spoke.
"You. Shouldn't. Be. Here. It. Isn't. Safe." Their voice was low and raspy, and they paused after every word.
"Why?" I asked.
"The. Fairy. Only. Flies. Underground."
"What does that even mean?"
"Do. Not. Find. The. Fairy."
"WHAT DOES THAT MEAN?!" I screamed. But they didn't answer. They just picked up the ax, walked to a new tree, and began to swing.
Dazed and confused, I wandered away.
"What fairy? And how would it fly underground?"
I wanted to find out, so I headed home and began to prepare.
I grabbed a shulker box and began to pack. I stuffed bread, steak, armor, extra tools, water buckets, and torches in the box. After I finished, I grabbed a few books, a few maps, and my disk which I used to record things that may be important, and stuffed them into my backpack. I was packed, now I just needed to find the "Fairy". Whatever that meant.
The person said underground, so I began to dig. I dug until my hands bled and my feet cramped. Finally, I reached a cave. I recognized it as my usual resource cave. I knew I had explored every bit of this cave, so I continued to dig. "The fairy only flies underground" wasn't much to go on, but I was determined. I wish I had just given up.
After days of non-stop digging and mining, I reached a new cave. This one was large and open, with mobs around every corner and a large patch of dirt near the entrance. But that was normal, so I was not afraid. I began mapping it so I could find my way out easier later. I searched around for a bit and found some diamonds, but no fairy.
Then I heard a noise. It wasn't a typical cave noise like a spider or a zombie. It wasn't even a slime or a zombie villager. It was deep like a zombie, but rattling like a skeleton, but also, squishy, like a slime. I turned to try to find where it was coming from, but it was like it was all around me. No matter where I turned, I heard it from every side. It was getting louder and louder until... it stopped. Quickly, I pulled out my disk and pressed record. Then the sound started again. This time it was deafening. I couldn't hear myself think, and the sound was now right behind me.
I didn't look back. I ran as fast as I could. And that's how I got to where I am now. The sound, quieter now, is following me, but I'm faster than whatever it is. Could this be the fairy that person was talking about? I don't know, but I keep going. I fumble for my map to try to find my way out of the cave. My breathing becomes ragged and loud and my footsteps even louder. I can't see anything, and the sound is far away, so I take a break to light a torch. The smoke fills my lungs and I cough, but I am thankful for the light. With the help of the torch, I check my map again, and it seems I am only a few hundred feet from the exit. I'm still catching my breath when the sound became louder again. Quickly, I stuff the map into my backpack and continue running. I find the large patch of dirt that was there when I entered and breathe a sigh of relief. I'm almost safe. But the sound is again right behind me and is louder than ever. Then it abruptly stops, and all I hear is the flickering of my torch. Now, I am more terrified than ever. I keep running until I trip and fall flat on my face. My disk shatters into 15 different pieces. I try to pick them all up, but there isn't enough time. I take what I have and continue running until I am out of the caves and above ground again.
I look back and see a figure holding a disk and staring at me with two pure, blank, white eyes. Then they blink and turn away. The noise recedes until I can't hear it anymore. After all of that, I am shaken, and more than a little confused. I don't think my jukebox will play my disk anymore, so I set it down on a small hill. After walking for about 2 minutes I turn back. I see a creeper carrying my disk away. I think about going back to get it, but I figure the creeper will have a better use for it than I will.
Now, safe, but alone, I return home to sleep.

Theory:
Steve was mining in a cave. He didn't bring any torches, so he brought flint and steel instead. He lit a fire to make light, and consequently he coughed from the smoke. He flipped through his journal. Then he heard a mysterious sound. He looked at the source of the sound, as that is a normal human reflex. His fire went out, so he lit it again. He saw a glimpse of the glowing eyes, and ran away in panic. He ran outside where the creature teleported in front of him and killed him, causing the disc to stop abruptly.
The creature that was chasing him and kill him was an Enderman. The sounds heard throughout the disc sounds much like an Enderman.
The creature isn't Herobrine because Herobrine doesn't make the kind of noise, also Herobrine doesn't physically exist. It is only a rumor in the Minecraft community.
The clicking noises were flint and steel, because in reality, lighters would make a clicking noise when lit up a fire.
Many people believe that the noise at the end is Steve's dog. But there isn't enough evidence to actually be a dog.

0:41 is where he hears the Enderman and starts to run, with increasing vigor and speed. Then all of the sudden it switches from walking on stone, to placing dirt blocks. It's like he is trying to block an entrance to protect himself. Except it just so happens to be that dirt is one of the few blocks that can be picked up by Endermen. The dirt failed to stop it , leading to the death of whoever it is in this music disc. The disc broke as it fell to the ground while he was killed.

Theory: (warning long)
This player named Steve11 was recording him playing an hardcore world running from mobs (zombies, skeletons etc).then he accidentally got him self into an mine. after he lost the mobs he had sit down after that he was lighting a candle because it was very dark. And then suddenly some dark gas appeared and so he started to cough and then got up too see what was causing it. After he got up he saw white glowing eyes and started to quickly pick up his things. Then all of a sudden he heard railing sounds then saw it was a mine cart. He quickly started running the only way there was. (The part where it cut off) he sees light from the outside he needs to go fast so he does and trips and holds on too an broken track because he will fall into the river which is very strong. he was ran over by the cart. Right before he dies in a water flow outside he sees the Steve skin but with glowing white eyes. Shortly after he was found the footage was blacked out only seeing 2 white glowing eyes. the players online were quickly creeped out except for user “Herobrine”

Disc 13 is first. Man shot at twice by skeleton. 2nd shot connects. Then 13 cuts off. Jump to disc 11. Flint n steel, book, running across stone/dirt. Jump back to 13 after the cut off period. Blown up by creeper. Lands in water, climbs out and staggers to safe place. Done
I have a better idea. See how the dirt and cobble noise is consistent following each other? I think this person was frantically spamming cobble/some stone then ran out to dirt

Here is my theory
Steve and his family live in a small inland (or seaside) village. One night, Herobrine enters the village, murdering everyone except Steve and his family. Steve's wife takes the children and runs in a different direction, but Steve doesn't know that (Start Disc 13 here). (End Disc 13, Disc 11 here) He runs and runs, thinking Herobrine is on his tail. Then, he ducks into an abandoned mine. He sits down on the cave floor and lights a fire. He coughs a little and walks around with his compass, trying to get an orientation. He sees a tall figure in the dark duck behind a rock. Wondering what it is, he goes "Oh?". Then his light goes out. He takes a seat, puts his pack on the ground and opens it, pulling out his map and checking to see where he is. He faintly sees glowing, purple eyes. He lights another fire and looks in the direction of the eyes. He sees it. He realizes that it showed a sign of agitation, drops his compass, and starts running out of the cave. He doesn't have an orientation because he dropped his compass, so he runs frantically in whatever way he could go. Just when he finally makes it out of the cave, and is running on the soil in the direction of the village, the enderman gets him. But Steve doesn't die, the enderman knocked him out. He falls down a small hole and bangs his head on a rock, giving him amnesia. The weird sounds at the end are of the tape shorting out (Disc 13 from here). Now you play as Steve, trying to find answers to where his family went, for that's all that he remembered after his amnesia. The tape in which he had was taken by Herobrine after Steve went unconscious, and stashed it in a secret place. The only thing left of it is a banged up disc, now known as Disc 11. The other parts of the tape is Disc 13, which was the ambience of that spooky night.
