No. It was a 12” single. It’s available on Discogs, albeit at a ridiculous price.

No, just on vinyl and cassette. The self-titled 1984 U.S. LP had two pressings on Arista(8214 and 8281). It made the 8281 pressing after MTV started playing the video, and from what I have discovered, not every 8281 pressing has it.
