Slight palatal expansion would’ve finished treatment in less than 6 months. Semi rapid expansion, 1mm expansion a week= stable results, more tongue space on upper palate, and horizontal growth of the maxilla with little tipping of teeth. Her lips would have leveled out but since you decided to take out premolars, the main concern for the patients protrusion (poor tongue posture) will get worse overtime. Tongue on palate, lips sealed, teeth slightly touching, standing/sitting up straight, and nasal breathing constitutes proper tongue posture. Conventional orthodontics are not trained in this issue. After the braces+extractions treatment, the patients skull grew down and back with a new issue of having retruded jaws. Her nose grew down instead of forward, her lower lip grew further back from where it should be, and her maxilla is recessed by a few millimeters. The face before treatment was fuller, slight younger looking, and attractive (with great potential). After treatment, she still retains some of her features, but the mid face is flatter, her lips went down and back, and her lower face looks slightly caved-in. Besides aesthetic concerns, the health of the patient has a 50/50 chance of getting better or worse for years to come. A table of measurements for teeth alone are not what constitutes good treatment vs bad treatment. A great orthodontist will look at the the relationship of the jaws AND the face to determine what needs to be brought FORWARD (not BACK) and avoids extraction at all cause unless medically required if in interference with patients health. I’m a victim of premolar extraction with my life negatively impacted because I was young and naive about orthodontics. They took a beautiful boy with so much potential and pulled his jaws back to make him look plain. Not only that, but I’m suffering the health and emotional problems brought on by extraction. I’m lucky as I have wisdom teeth and I’m looking forward to having my extraction spaces reopened to give me my face back but it will never be the same knowing what life could’ve been if I didn’t get extractions. It gets better day by day for people like us, but for the people still reading this please be aware of the treatment you are getting into and be cautions of any treatment plan that you feel can negatively impact you health for the long term. Goodbye
Hi at what age did you get your braces? I extracted 4 theeth cuz I'm 20 I can't get a palatal expansion and I'm starting to regret it cuz my face is already flat but I have an open bite and every doctor said I need an extraction it's been 5 months and I'm so afraid it will change my face I like my face but the doctor said that nothing will change but I'm so worried can you tell me what did you have ? And at what age you did it(sorry for my English it's not my first language)
Thank you for sharing this case, Dr. Chang. How did you resolve the mesially tilted molars and posterior open bite? Through additional refinements? Anything you would do differently to avoid the bowing effect and dumping of molar crowns?
Please show us more ABO cases. Thank you for all these educational videos.
My teeth is exactly like this and i really need braces 😭
Is it normal to extract the second premolars and not the first?
depends which are the smaller ones (the smaller ones are usually extracted)

Sir.. how do you close the bilateral posterior open bite without brackets ??
just spontaneous eruption will close the open bite gradually
