- Bimaxillary Protrusion Treated with Insignia® System Customized Brackets and Archwires
Abstract
Correction of moderate-to-severe bimaxillary protrusion is challenging, particularly without orthognathic surgery and/or temporary anchorage devices(TAD). This case presents a 4-first-premolar extraction treatment of a 12-year-old boy with a chief complaint of lip protrusion. The diagnostic evaluation revealed a convex profile, steep mandibular plane angle, bimaxillary flared incisors and protrusive lips, deep bite, and deep curve of Spee.
The orthodontic treatment was accomplished with a customized appliance (Insignia® system), including the self-ligating brackets and progressive archwires, constructed by reverse engineering from a 3D digital set-up. Extraction spaces were successfully closed in all four quadrants in 13 appointments over 19 months without using TAD supplemented anchorage. The facial profile and occlusion have been significantly improved.
The 3 keys to success of bimaxillary protrusion treatment with digitally engineered mechanics are precise estimation of anterior torque compensations, appropriate use of auxiliaries, and careful adherence to the recommended archwire sequence. With correct treatment planning, the outcomes are enhanced by minimal treatment adjustments, and decreased treatment time.

使用數位化矯正裝置成功治療雙顎前突有三個關鍵：1. 精準預測前牙矯正所需的torque補償，2. 在適當時機使用輔助裝置，3. 依循系統建議的矯正換線順序。在正確的治療計劃設定下，使用數位化矯正裝置能減少矯正過程中需要調整的次數，縮短整體治療時間，更有效率的達到理想治療效果。

I did this yesterday! Overall they're not painful, just a bit uncomfortable. I hope I will get the desired result because I've been really insecure about my side profile.

The condition of course. Unfavorable as in western media used to more commonly make fun of Asians by stereotyping this kind of teeth.

Really hoping to get this done someday, most of my facial insecurities deal with the lower third of my face. The profile looks horrible and my philtrum is already longer than it needs to be with my small lips. I feel like this would be beyond helpful to my self-esteem

Braces causes sleep apnea because of the pulling of the teeth backwards you won't have space for resting the tongue this will cause you breathing problems

My ortho waited a whole year and 2 months to have the extractions done. And even after 8 months now (I got the extractions back in feb 22) the gaps are still there. Like you could literally still fit an entire tooth in each of the gaps. And I’m so frustrated because I was supposed to get my braces off this October, but it looks likes it going to be a bit longer. Probably much longer at this rate… 🙃🙃🙃

Why did your Ortho wait so long? Were your teeth really crooked? To my knowledge, yes, you need to align and level your teeth first before you can close any gaps, but still that's pretty long...
Did they thought it could be fixed without extraction, then changed their mind after a year or was extraction always in the plan?
For me, I have bimaxillary protrusion, but my teeth are not crooked, only slight overcrowding in my lower incisors. My ortho extracted my top premolars within the first week and installed the top brace few days after that. Two months later the lower teeth and jaw followed. She needed the space for the teeth to wiggle around and aligned themselves, before finally closing the gap. The levelling and aligning she predicted 6-8 months.

Reading all the comments gives me mix feelings about taking four teeth out :( this guy has the same problem as me looking at it I think it makes sense when u have a small arch and his lips do stick out a lot he looks normal after it would be scary to take teeth out and u have like that sunken mouth look it would be horrible

Yes, I have thick pouty lips before extraction and braces, after like a year I noticed that my face became wider and my lips are like moved backward and smaller, my chin is more forwarded now and im happy with that, but i wish my lips remained the same :(

@@yomomgay7170 Yes you can get braces after having the tooth extracted. I had to get 2 of my canine teeth taken out on my right side, top and bottom teeth, before I got my braces. Hopefully that helps you

I got four of my teeth removed..
I used to have an open bite (5mm) and most orthodontists had suggested extractions since I couldn't afford surgery. So I opted for a camouflage technique instead, osseous genioplasty to fix my protruding chin, 4 premolar extractions (wisdom teeth had already been extracted), lingual braces with tads for 2 years. I now have wire attachments behind my front teeth and wear a retainer for 12 hours a day and a tongue crib to better help me position my tongue. It has been worth it.
@@jazmin2111 it's a bit difficult to say! It has become a bit slimmer but that was from the genioplasty and masseter Botox injections but im not sure about the premolars themsleves. it loiks better now tho!

My smile is already fine but i am very insecure about my side profile and my overbite so i was planning to get braces but my new dentist (since the last dentist just scammed me) informed me that I'd need to sacrifice my perfectly fine pre-molars first, i was very hesitant at first but thanks to this video and bunch of research I am now able to trust my new dentist, thanks!!

For me, I'm actually happy with my smile, and my teeth alignment are considered fine, it's just the protrusion really bothered me and the fact that I can't close my lips properly. It's rather scary having to go extract healthy teeth and having the empty spaces while the treatment is going. Do the empty spaces bother you much? How long till they close?

Hi everyone! My daughter's teeth moved into place so quickly after extractions. And she will take off her braces just before Christmas 🎄. I can't believe how quickly time passed by. I’ve had my braces for 6 months now and the gaps in my lower teeth are semi closed but the upper ones are still there but on their way to close too! The gaps don’t bother me tbh and they’re not noticeable unless you open your mouth wide open.

Not if you have a protruding teeth, there are no spaces left for the teeth to move and to eventually straight up. So extraction is a must

Not necessarily. There are techniques where the orthodontist can expand the arch non-surgically and allow the mandible to come forward which is what should have been done in this case rather than extractions. This child will likely grow up to have further breathing issues now that the extractions have occurred and there will be less space for the tongue once the spaces are closed (his airway is already noticeably small even for his age)
My first dentist only removed 3 teeth 2 premolar on upper teeth each side and 1 premolar tooth only in the bottom idk why, the middle line of my teeth upper and bottom doesn't even align lol anyways i was 18 now 24 got braces again bcuz I didn't wear my retainer and i still have minor protuding w/ my bottom lip, which is my insecurity now I'm very vocal to my dentist about what I want to achieve. We did some stripping of some teeth cuz I guess I only need a bit of gap to move my bottom teeth inside. I just wish all dentist would also be vocal about procedures and explain it not just do it. Also if you got some teeth extracted make sure to wear ur retainer After braces because it will shift back and you'll see the bone loss where the teeth was extracted, food gets in there which led to inflammation or infection so wear ur retainer.

I just got my 4 premolars removed. And the dentist said that Change in body is a very slow process and bone resorption and deposition rate varies from person to person. It may take 3-6 months for the gap closure but depends on the position of the jaw,bone,teeth, and your regular follow ups

I think you can always reduce size but it’s harder to make it bigger as you age. So yes but the retraction May be less drastic since your bones are fused. My sister did it at that age and it worked

Just extracted 2 teeth yesterday, I feel like it's okay, the feel is like the same after my teeth extracted, not uncomfortable but just feel a bit weird because there's a big gap. But braces will fix it I believe.

Please to those who already haved this process tell us your story, you can help someone to decide you were wish you knew this before start this treatment but help other to know about that.
For me it addressed the TMJ headaches i was having prior to treatment, i had ovejet and overbite with crowding on lower arch, had 4 extracted. Gaps are closed now but my arch is narrow so i feel my tongue with teeth while talking. My profile looks great though. My dentist will use dental expanders to address my narrow arch if i continue to have same arch problem after removal. He says molars will get back to prior position once forces are removed so would have no tongue issue. Let's see. Some people face none of these problem, like one of my friend. If your jaw and palate is fully developed as adult ortho may consider extraction, or he'll expand palate if it's underdeveloped or if one is minor.

Unlike orthodontics, orthotropics is all about forward and upward growth, pushing your upper and lower jaws forward and upward.
Ailar Kma imagine the exhaustion in our jaw and chin when we can’t properly close our mouth. I think your face slimming down isn’t a bad thing.

@@squirrel8397 I just went with my orthos recommendation, my teeth are still protruded a bit now, but not as much as before. And my friend seems to think that it's cute since it looks like a bunny
@@norules7805 I see. Do you know how much your teeth protrude now? I also have protrusive teeth and I want to avoid extractions but my orthodontist said my teeth will protrude more if I don’t get extractions. :/

this is my 3rd year using braces, I've already extracted 4 teeth and still have overbite
and i have a new problem, my tongue gets bitten often because my jaw is narrowed T^T
One of my tooth was damaged and didnt grow which affected my backer teeth. The dentist told me that I have a small jawline so 4 teeth extractions is okay, so i agreed and now im going to get braces in a few days. Im a bit worried tho cuz i wonder how I'd look afterwards

then ask your dentist to extract your tongue too

I am very very confused that whether I should go with experience ortho or I should go with young orthodontist , these young once have franchise and they have all the new equipments and Demon braces as well and are knowledgeable also.... but on the other side one is very experienced , please suggest me, I am very depressed with my decision making , which one I should go for experienced or equipments ? I rely need your suggestion and it will be my final decision .... Waiting

@@diasroshi4097 going good, I had a tooth that was pointing at 45 degrees incline and surprisingly took only about 3 weeks to align it straight.

The case is same with me. I have to do extraction. My dentist said the same too within 24 months.
I used to have the same teeth condition like him. Even the same shape of teeth..and after braces..I also get the same result as him..this reminds me of myself..good memories..😂😂
