    Accutane - 5 Tips by Dermatologists
    Accutane or Roaccutane is a tablet is also known as isotretinoin and is a form of vitamin A. It has been the mainstay for the management of severe acne or scarring acne in Australia, Europe, US and Asia for the past 30 years.
    Accutane works by targeting the oil glands, and reducing oil production. In lower does it can help remodel dermal collagen and reduce scar formation- including acne scars, Accutane can also reduce blackheads, whiteheads, zits, cysts and pimples.
    Should I go on Accutane for my acne?
    Roaccutane should only be used if your acne is severe, cystic or scarring. An off label indication of Roaccutane is if your acne does not respond to normal acne medications. An assessment and discussion with a Dermatologist is needed to ascertain if you are suitable for this medication, as there are numerous side-effects associated with this drug.
    Accutane has a 99% success rate for the treatment of acne. An average person will need a course lasting between 7-10 months. Improvement is seen within a few weeks of starting this medication. In severe cases of acne, an early and transient ‘flare up’ maybe seen, however your Specialist will undertake steps to minimise this side-effect.
    What side effects can I expect on Accutane?
    The most common side effect is dryness of the lips and skin. Universally seen common and very easy to manage. Moisturise frequently and you will sail thru the course of treatment. Use a lip balm at least 3-5 times per day, and moisturise your skin at least twice a day.
    Some patients may get dry eyes on Roaccutane/ Accutane and using eye drops may help, especially if you wear contact lenses.
    Your Dermatologist will also order special blood tests throughout your treatment course, making sure your Liver and Cholesterol levels are in check. Female patients are warned in regards to the serious consequences in regards to birth defects whilst on Accutane. Other side effects that are less common including-
    Flare up of acne (usually early in the course of treatment)
    Headaches
    Abnormal period cycles
    Muscle aches and pains
    Ingrown toe and finger nails
    Abdominal symptoms
    Mood changes and depression
    How long is a course of Roaccutane treatment?
    The course of Roaccutane/ Oratane is variable and will depend on many factors including the type of acne, your body weight, the location of your acne and the side effects you may experience on this tablet. As a guide-
    Average length of treatment 6-9 months
    Severe acne involving bacne acne, trunkal acne 10-12 months
    Low dose Roaccutane / Oratane program 14-20 months
    Your Dermatologist will decide as to what is the optimal course of Roaccutane/ Accutane treatment based upon your acne type and your lifestyle.
    Will a course of Accutane treat my acne scars?
    That is a difficult question to answer, as results are variable. Accutane is not designed as a treatment for acne scars, but a treatment to treat and cure acne. Treating active acne forms the foundation for acne scar treatment.
    In regards to Accutane and acne scars the following facts apply-
    Some patients can receive mild improvement of acne scars on this drug. Low dose Accutane has been shown to remodel scars and increase collagen production.
    Roaccutane/ Accutane patients are not candidates for laser scar revision during the course of Roaccutane, and a wash out period of 3-6 months will be needed.
    We now have a method of treating acne patients on Roaccutane with a non- laser method of scar revision called RF or RadioFrequency. This treatment has proven to be safe and effective in the majority of patients, however careful monitoring is needed. This forms the basis of the Acne Free RF Program, which was designed by Dr Davin Lim.
    What skin care should I use whilst on Accutane/ Roaccutane Treatment?
    Looking after your skin whist on Roaccutane/ Oratane/ Accutane is vitally important and can markedly reduce the side effects of this medication. Keep your acne skin care regime simple, and moisturise frequently!
    Clean your skin twice a day with a low irritant cleanser- I recommend Cetaphil, QV Wash or Neutrogena Gentle wash.
    Moisturise your skin twice a day- I recommend Cetaphil moisturiser.
    Moisturise your lips 4-6 times a day, most importantly just before bedtime as this will reduce severity of ‘dry lips on roaccutane’.
    Using a high factor SPF sunscreen twice a day can reduce the incidence of sunburn on this medication. I recommend La Roche Posay.

      Most of acne problem is due to hormone/genetic...some ppl are just lucky.. they dont even wash their face use facial cleanser but still have flawless skin.

      @@elitegammer1730 imo..the most effective way to get rid of acne is by consulting dermatologist..no facial cream can solve your hormone problem unless you take antibiotic or accutane which is u cant get it from any pharmacy..

      Dr Davin Lim very late response, hopefully you will see this, but mu doctor put me on 80mg off the bat. You said you typically put your patients on 10mg to start. I am already getting the side effects pretty hard 2 weeks in, do you think this is too harsh of a start and dosage? I dont want the severe side effects some people have like split lips

      I started off with 50mgs a day, and I dont think it’s gonna be a higher dose later on (I’m 173cm btw). But im now on week three and over all I dont have any bad side effects at all! Little bit dry lips but definitely not all that bad.. does that mean my dose is too low or..?

    I was on accutane 20 years ago. I had cystic acne on my face and back. I work as a hairdresser and my self esteem was so low, from customers asking me if I was contagious, and refusing to let me do their hair. I would cry every night. This drug saved my life. It was life changing. My dermatologist, was honest and told me I will always be prone to acne ,even after using this drug, but, it will never come back to to same severity i had. He was right. 20 years later I do have breakouts, but nothing out of control. I never had any negative side effects other than dry lips, and, I made the mistake of waxing my eyebrows. ( don't do this) I was fine in the sun . I would take this drug again in a heartbeat.

      I was a non believer of this product. Once I was getting acne conditions, I decided to give this acne guide *AcneCureOnline. Com* a try. It worked for me. Within 1 week, I can notice the results. I now have a better and clearer face.

These people are the dregs you should filter out. They absolutely do not count. Drink the coffee and toss out the grounds.     For the first time in years I’ve gotten out of bed, looked in the mirror, and had a face free of zits-okay, I have one small little pimple. It feels SO good and freeing. I also normally have super oily skin (oily two hours after washing it or sooner), which meant I was constantly uncomfortable. Now I can go to the movies with my friends and not feel gross halfway through. For the first time in so long I can finally enjoy myself. The side effects have been manageable and absolutely worth it.

      You hear only about the extremely bad stories on the internet... You don´t hear about the millions if not hundreds of millions of patients it has cured acne for.. If it was that dangerous it wouldn´t get released to the public at all.. It is just like antibiotics or other medication, because there is risk with all medication but the risk is very very minimal.. We are talking 0.1-0.5% risk to get something that cant be fixed again.

    Definitely live, breathe and chug water 24/7. I was so dehydrated I was blacking out and so fatigued. I thought I was dying! BUT I found out it was dehydration AND I was doing it to myself. DRINK TONS OF WATER!!

    SO glad I have found your channel!!! My 13 yo son is struggling with severe acne and has for probably 1-2 years. I had to switch his normal pcp,of 13 years, to get a referral to a derm. She was shocked that it had taken so long. Said he already has scarring and gave us the choice of an antibiotic and topical,or Accutane.I have heard horror stories of the Accutane so we chose the other but a month in and no significant change. so next check up I may switch him. I will most certainly be showing him your videos, and this one in paticular. So he will see how important the SPF is!!! Thanks for sharing your knowledge with us all. You are enjoyable to watch!

    Excellent video! Watching this has, for the first time, put my mind at ease about starting isotretinoin and you've mentioned all the things that my derm "forgot" to tell me about the drug. And you put it into context about lifestyle, as I'm worried about muscle pains as I work out 3-4 times a week and all my derm mentioned was "don't drink protein shakes" -_- great video!!

      Orion hi! I know you didn’t ask me but I’m about to finish my first month in a couple of days & I did breakout the first 2-3 weeks but it’s calmed down now! It may be because I’m only on 30 mg a day or I don’t know maybe I just got lucky but I didn’t break out as badly as I thought. Also, I haven’t experienced any face dryness, just dry lips, & I get a bit more tired than usual. But I’ve already noticed a HUGEEE difference, especially with scarring as well!!

    Thanks to Accutane, I've gotten the results that I want! The dermatologist told me that I should take 20mg for 13 months. Dryness? No, I didn't suffer dryness, because water is life HAHAHA I drink almost 3L of water everyday.

      @@fahadlamisa8719 hi! The accutane helps me clear my skin from acne. The downside of this is you self medicate, you can suffer from liver damage. So always consult your doctor.

    I took it almost 20 years ago. I stopped early as the joint pain and muscle pain got in the way of sport - and the hair falling out. Luckily it did it's job and not a pimple since. Not that mine was bad - it was just "recalcitrant" and would not go away. The thing they worked out AFTER I had gone through it was the propensity to cause depression. When I found that out years later it made that whole time period in my life make so much more sense. It was so weird feeling upset - but not being upset. It didn't make sense and I knew I wasn't upset - but I was.... I wish they'd have known of that link sooner - like before I took it. The blood tests for liver function every 2 weeks were fun too. Gave me confidence to keep on the piss... I was at uni afterall!

    My son is on 40mg a day (day 16), and the derm might go to 80mg next month. I’m not sure he should go that route so soon, because his acne is moderate. My advice is to research and don’t be afraid to ask your doctor questions! I’m a patient advocate and tell all my clients to be personally involved in their healthcare.

    I suffer from back acne and it started from behind the neck at around 16 years old. I’ve been bodybuilding since I was 15 years olds, I won’t lie I’m a sucker for ice cream , when i eat bad I can notice my acne is getting worst. Now I’m 23. First month on accutane 40mg , I cleaned my diets, dry lips, dry face, but that’s all. I can notice my skin is getting better . Now I’m currently on month 2 at 80mg. So far so good. I feel that my skin is getting thinner. It feel weird but I like the sensation :) . I take my accutane with food like avocado, oatmeal with Virgin coconut oil. I’ve heard that it react better with fats... my face is pretty much acne free but dryer. I can’t wait for my back to clear up! This treatment is long but worth it at the end! I don’t drink alcohol. Apple ciders vinegar with cranberry juice in the morning to detoxify the liver is a must. Thank you for your tips Dr Lim.

    It's been a year of completing roaccutane for me. I had the basic side affects. bt it was the best decision for my skin. also, my dosage changed over my 8 month treatment and works wonders. I still do get the occasional zits but nothing to complain about. speak to your dermatologist frequently, it's worth it

    Starting accutane on a low mg dose is smart because it prevents your hairline from thinning as much.

    Thank you, Doctor Lim! I am on my first week on accutane, actually it's been only couple of years since this pill is approved for use in my country. Now I am 40 yo, and I am suffering from acute acne since my youth. My dermatologist was a bit reluctant to give me the pill as here doctors do not have much experience using this medication, all thinking it is too dangerous... And finaly it was my ginecologist who geve me the prescription, as she knows me for ages and understands the dept of the damage acne is causing to my life. I started on 20 mg, and after my first full blood test at the end of the month normally will switch to 40mg. I am still not sure if that will solve my acne problem, as by now I have tried almost everything - expensive topical solutions of all kinds, cosmetic and laser procedures, antibiotic treatments... But it was motivating to watch your video. Thank you!

    Just started accutane on 30mg/day 2 days ago for cystic acne and hidradenitis suppurativa, after literally a decade of trying every antibiotic under the sun I am hopeful I might get somewhere this time! Thanks for the tips!

      @@bocastyle It does help with the HS but hasn't made it go away entirely (for ref I have stage 2 HS with my main problem area being my armpits) I had a couple of times where I ran out of accutane pills cause of covid and needing to do pregnancy test to get my next prescription and I did notice that I would start getting more lumps fairly quickly even though the medication lasts in your system for a month, so im not sure how lasting the effects will be on my HS once I have finished the full course. It does seem to have really helped with the lumps I was getting on my face (which according to the derm was cystic acne not hs) and has helped with scarring that I had too so I am hoping that lasts after im done. I can update again once im done with it all if its helpful? I know finding info about HS is hard! (Also sorry if this was way too much info but like I said, HS info is hard to come by)

    Hi Dr. Davin, im currently on low dosage accutane of 10mg tablets 3 times per week. I have seen your video which says that it is fine to go for a laser resurfacing while on accutane without delayed laser treatment. May i know which research have shown this result? Thanks!

    I hope this helps someone....I thought I would have to try Accutane but I realized that it was several things, including.... my BB Cream, no kidding, and a GF of mine was using a BB Cream too that was giving her terrible horrible acne until I told her to throw it away and it cleared up. However, it can be metabolic, certain oils my body can't metabolize, such as Olive oil or any fried foods will give me a small pimple usually, but Olive oil will give me really deep, and really terrible cystic acne, also cheap chocolate, because it has wax, and cashews, as well as certain skin products, gave me acne. Testing out foods and oils to see which gave me acne, changing my skincare products and my diet plus being extra hygienic with my pillow covers, and learning seriously to never ever evverrrr touch my face, and take regular pro-biotics cleared it all up forever for me.

    Thanks Doc. Your video is informative and accurate. I have been on Isotretinoin 6 weeks. So far, my skin looks clear and all tiny zits are gone. Last week I had a sip of beer which was not so good. it actually made me tipsy very faster than my normal pace. I rather not drink at all from now on.

      Yes, some people do get drunk easier on roaccutane. Its not in the product information, but it can affect your liver if you drink way too much.

    I have just started my 3rd month on Roaccutane and have been uped to 60mg by my dermatologist. The acne has gotten less reddish and inflamed and some pimples have almost disappeared but still a long way to go…

    I had very mild acne (a few cysts that I was usually treating them with benzoyl peroxide) and extremely oily skin. I had this problem for over 12 years (I am 26 now) and it was getting worse from year to year. I finally decided to try Accutane. I don't have polycystic ovaries or hormonal problems, just big sebaceous glands on my face (like my mother and grandmother). I took Accutane for almost 3 months and I am super happy with the results. I was from the beginning on a higher dose (40mg and I have about 60 kilograms). The cysts were gone from the first month and I didn't get any breakouts in the last 1 month and a half. Should I stop here or should I continue for the full 6 months ?

      evelina dumitrescu keep going and finish the complete process. I have been in the same situation as you. dermatologist recommended to go with the full treatment so ask him/her. I have also suffered with severe acne including painful cysts in different areas and Accutane has been good to me.

      maybe you started with a high dose for your body to handle the acne does go away mine was gone after 3 months if you are experiencing flare ups tell your doctor he may give u other meds to take along with accutane to decrease the severy of the flare up

    I have also question for you. I went on Axotret (that how is called Accutane in my country) 1,5 month ago. My side effects are a little bit dry skin, lips and eyes when I wake up, or when I need to work on my computers at nights. Since one week I noticed that my joints started to cracking a little bit more but it is not painful.
    I weight something about 70kg, my daily dose is 40mg since beginning. Isn't it to much? I really want to come back to working out at the gym after this treatment but I am concerned about the impact of this drug on joints.

    My case is strictly genetic it sucks! People tell me not to have dairy or sugar, and I tell them I've been on a NO processed food, NO dairy, NO added sugar, and NO carb diet for the last 3 years and im still breaking out like nuts. I've thought about doing accutane and honestly some of the side effects people have been having, short term and long term have been scaring me off from taking it. I already suffer from Lupus disease so im just worried my arthritis will get worse than it already is. Or gain a whole new array of problems.

      Hi, optometric technician here!
      Accurate causes dryness which can occur with the eyes as well. If you wear contacts you may notice irritation and uncomfortable fit due to the dryness and you may want to visit your optometrist. Otherwise, OTC artificial tears such as refresh gel, refresh mega 3, systane complete are good options to use as needed. If you have contacts you can use saline solution or an artificial tear that’s preservative free, such as refresh for contacts. Eating leafy greens and fish/omega 3 foods/supplements can help quell the dryness.
      The blurred vision is a side effect of dryness

    3 months after I stopped taking Accutane (took 5 months of 40 mg a day), I did a keratin treatment cause my hair was porous and super dry, right after it started falling crazily, I think I've lost half of it in about 2 months. It didn't recover completely now, 4 months later, but it got better. The issue is now my acne is coming back and it's super depressing. Wouldn't mind taking a second course of Accutane but I'm really afraid I'm gonna get bald. Bald and full of pimples doesn't sound too good... what should I do?

    i started a month ago taking 40mg a day. i experienced many side effects during the first 2 weeks from really bad dizziness, weird unclear vision and of course the dryness. My derm told me the dizziness was probably caused by my eyes getting dry so i applied refresh tears and i think they worked because i don't get any bad dizziness as before. also i am currently in my initial breakout... it's sooo bad. only reason why i haven't killed myself (hyperbole) is because i am hanging on to hope of the outcome :( but honestly iv'e never seen myself break out so bad. I just started 60mg 2 days ago. TIME PLEASE GO BY FAST!!

      +Allen Dnk hey. yes I just started month 4 and the results are AMAZIIING. I honestly had no hope for my skin. I thought this was just going to be another useless medication.. wow was I wrong. my skin is completely acne free. the only thing I have is blemishes or hyperpigmentation which will go away in a few months but I haven't broke out in about 1 month!! I do have to say tho, it's been very tough. I feel like a vampire because every time I step outside my skin burns like crazy. so use a lot of sunscreen. you will see very good results. trust me. just be patient. good luck! :D

    I know it’s very important to wear sunscreen on Accutane but I’ve been to the beach a couple of times and have forgot sunscreen and haven’t burnt. Maybe it’s just my skin type? But they stress it a lot it the sun doesn’t seem to have affect on me

    Hey doc, my genetics are pretty wrinkle proof because we don't have dry skin in our family and the skin is thick and takes Sun very well (quarter Egyptian ) but now I have to take Accutane , will that cause less resistance to wrinkling later in life ? Because I heard it makes the skin dry and Sun sensitive :( is that persistent after treatment? Also I have a brown freckle in the white of the eye, can Accutane cause more of those?

      Depends on the recovery of your skin. The guideline is 6 months, but your Dermatologist may elect a shorter time line based upon your recovery and the density of Fraxel.

    I took my 30th pill (40mg) today. Also got my blood work report!
    Within a month, my Triglycerides went from 60 to 100mg/dL.
    LDL- from 90 to 110 mg/dL.
    HA1C- from 5.2 to 5.4%
    Total cholesterol is 165 which is normal. And everything else in the blood report is ideal too!
    I have a healthy lifestyle. Eat pretty clean, a lot of good fats, moderate protein, low carbs and no sugar at all. exercise 2-3 times a week.
    Accutane certainly did raise those levels I mentioned above.
    4 more months to go and it'll be alright!

    Hi Dr Lim, massive fan. After years of dealing with acne, I am finally considering going on accutane. However my father took accutane when he was younger and has experienced some permanent symptoms that has meant his skin doesn’t self-moisturise at all and the skin on his face has been left looking flushed constantly. I am slightly worried that this reaction could be genetic and that it might be a bad idea for me to follow suit...what do you think?

      Kamal Lalchandani I also noticed when im using cetaphil mosturizer my pimples takes time to heal and look fresh, and if i dont use mosturizer my pimples starting to peel and dry. Sometimes im thinking if the mosturizer make my pimples worst.😕

      @John Daryl Simene Das true, cetaphil says it's for all skin types. & tbh, it's bs. It should be recommended 4 ppl with normal to dry skin. Def not for oily, unless sum1 wanna look roasted.🤷🏽‍♀️

      @@choysam2400 Look up the side effects of Cetaphil, there's actually plenty of side effects in using & matter of fact it does makes your acne worse. I recommend u use cerave. Trust me, u won't regret it 1 bit.

    Hey Dr Davin, I have an oily skin but my overall face acne is in pretty good shape. However, there is a sebaceous cysts that always return on my face cheek, even after I cut one out in surgery. My dermatologist said that I can't prevent future development of cysts because of my skin type. Do you think that Accutane is the answer for this??

    Accurate really helped my skin, but 1 year post treatment, my skin reverted back. My derm has me considering a second round, but it's such a harsh medication. Would laser be a better option for me?

    It's been 15 days of taking isotretinoin 10g tablets... My acne purged severely for the first 12 days., but my skin is still improving

      @@xeey-3073After about 15 days.there were only 2-3 active zits and others were healing..
      But after about 18 days 1-2 zits started appearing almost everyday. This continued for almost 30 days. My doctor changed my medicine ( a tablet which accelerates healing and a topical antibiotic) .. It's abt 20 days taking the new medicine very less improvement

    I just finished my accutane about a month ago. Is it okay to use The Ordinary's AHA 30% + BHA 2% Peeling Solution and Niacinamide 10% + Zinc 1%?

      i wouldn’t do this until after you’re done. i also use those and my dermatologist said don’t bc i can make my scars worse

    I was on Accutane for 5.5 months at a pretty high dose for my weight. Had angular chillitus and looked like the joker. End of treatment my skin was glowing. Unfortunately, it came back and it’s been a cycle of differin, retin-a, skin looks good another breakout antibiotics spiranolactone etc. I’m starting another round of Accutane soon.

    I just started my second month. My doctor started me on 40mg right off the bat. I had SOME dryness on my scalp which I honestly didn’t even notice until I was just playing with my hair. My lips are pretty dry but with aquafor chapstick and water it’s manageable. My body is not dry at all. Purged a little the first 3 weeks but it wasn’t THAT bad. It honestly has been a breeze so far. I also changed my diet a bit which seemed to help.

      FilOfficial not nose bleeds but I have noticed in the last week or two my nose(on the inside) has been super dry and scabbing

      Kristen Stites oh ok, i started accutane 1 monts ago and already got 4 nose bleed.. I just start put Vaseline in my noise and I’m feel better ! Hopefully we make it each other :)

      yea I'm pretty sure fish oil capsules are more than okay bc they help w the inflammation too i think. How is your progress?

    Dr.Davin, I'm freaking out here because am losing hair quite a bit and noticing that my hair is thinning. Is this normal? Plus my doctor told me to take hair genesis, is it any good?

    Hello, ive had everything from mild to moderate to severe acne off and on over the past few years due to starting and stopping antibiotics and topicals etc. i have recently decided to go on accutane to get rid of my problem once and for all even though my acne is only moderate at the moment (I breakout often but it’s not cystic). My doctor has prescribed me zenatin (i think its called) 40mg right off the bat. Is this too much to take right off the bat or should I ask to be put on 20 instead to start off...? Thanks

    Accutane was recommended to me, but I declined. Sadly, I already have extremely dry flaky skin and I think Accutane would be unbearable. Thank you for offering tips though.

      No problems. I personally don't prescribe Accutane, as I am a procedural dermatologist. My days of treating acne are long gone, but my colleagues help carry the fight. No one likes prescribing this drug.. its the one of last choice. Any good dermatologist would want your acne to go away with NO medication.. but if all else fails...

    i’m on 10mg and taking every second day so starting off really slow and it’s frustrating sometimes and i’m in the big breakout period of accutane but it’s worth it

    Can i use HealthAid Livercare 60 Vegetarian Tablets with Accutane?
    I use Accutane on low dose(20 MG).first month 2 accutane tablet per week and for another 5 month one tablet per week.

    I wish you were my dermatologist, I went to a dermatologist about my acne on my forhead and jawline, she found out that it was most likely hormonal, but still she prescribed me a blood test so she can put me on medication (not sure if its accutane or antibiotics). but to me that was a little odd and she did not explain. I'm still having doubts weather should I go on her treatment , or should I go to a gynecologist to get checked first and solve my hormonal problems, because that might solve my acne problem as well? , I don't know, any advice here would be really helpful :)

    I was put on a very low dose 10mg/daily cuz my dermatologist intends to treat my acne without me suffering any side effects at all and she said I will be on it for quite some time.. I have mild/moderate acne that didn’t respond to other treatments, so I truly hope this will help me.. Thank you for these wonderful tips Dr ❤️ I think your was the most helpful one!
