I am Gleason 6 with one of 6 core positive at 50% the rest have little however, I am on replacement testosterone because I do not make testosterone (56 last test off TRT) my life without TRT is abysmal. Depression, anxiety, irritability, timidity etc that makes my life hell. I will consider radiation but will not do androgen diprivation. Quality of life has to be a consideration so I am at odd's with my urologist. i'd rather a more aggressive AS with more biopsy and DRE. My PSA is a steady 5

My opinion...Fire your Urologist...Gleason 6 will probably never require treatment...TRT shows no signs of causing prostate cancer but low testosterone does, along with a myriad of other health issues... Do as your new vetted Urologist says...Good luck.

Urologists may not be up to date on the latest advances in treatment; for any OTHER type of cancer, full-time board-certified oncologists help select treatments. Urologists are generally surgeons, a different skill set.
