  • Recently I made the switch to Dovato as my HIV medication. In this video I cover WHY I switched from my previous medication, Biktarvy, what my experience was taking Biktarvy, what my experience is SO FAR taking Dovato, and any pertinent information regarding side effects etc related to Dovato.
    **CORRECTION** Tenofovir Alafenamide is NOT an INSTI as I mentioned in the video. Bictegravir is the drug in Biktarvy that is the INSTI

    1:40 NOPE, sorry! Tenofoviralafenamid is a nucleotide reverse transcriptase inhibitor, not an INSTI. I think you meant to say "Bictegravir" ;)

      Congrats 🎉 with the new meds Raif! My dear friend is also switching from Biktarvy to DTG with Descovy, because felt so depressed on BIC. We hope also soon to get on Dovato, for to avoid TAF. Did you feel it’s easier, better without TAF for you now? read Taf was also linked to weight gain.

    Wow OMG it is incredible to view a video on here that directly relates to some if not almost all of my experiences over the last 6 years... I honestly can't thank you enough for posting..... I was on Biktarvy for about 3 years (my second regime of meds) and even though I was in my early 40's at the time I'd always maintained a slim frame though diet and gym..... boom BIKTARY hit and almost instantly i went from 73kg to 90kg.... I put it down to the fact I had started a new Uni degree and was stressed but id never in my entire life been that heavy - so of course came a bit of depression and a few other things..... I started on DOVATO a couple of years ago and almost straight away I'm back down to 74kg which is pretty hard to do when you're nearly 50... energy is back.... back at the gym... other things are back to normal... I still have a little anxiety but no where near as bad...

    Yeah, since I am on a very complicated Medicare/Medicaid insurance plan I may have to switch to Dovato for purely insurance/cost savings purpose. I can't wait to see your post visit video. I also have had and still have major disabling life long digestive problems, so I sure hope that Dovato may bring me some relief. Glad to have stumbled on your content AGAIN, this time I made sure to hit the subscribe button.     Hi there! So I’ve been following you since 2019 and you’re just the friend that everyone needs so thank you for that. I do have two questions for you-the last labs you had with Biktarvy, how were your kidneys? 2) would you be willing to talk about how sex has changed (if it has) throughout the years? Especially because you mentioned the gas part, I’m interested to see how you were able to manage your intimacy with this because I think it affects more of us than we can imagine.

    Where have you (this content, and you of course) been all my life??? Thank you for sharing openly and honestly in such a public way, it's awe inspiring. A drug is a bunch of chemicals. Any drug you put in your body over the counter or otherwise has some level of toxicity (ie side effects), so by definition if you reduce the number of drugs 3 to 2 you will automatically lower the toxicity. Another consideration is that combo drugs interact with each other and my have a multiplier effect on a particular side effect. Combo drugs came into being because they found multiple drugs worked better for viral suppression than single drugs.
    My gut has always been my weak spot too . . in the early years wasting syndrome, parasites, etc . . . let's just say it was difficult to get around. It took many years to stabilize. Thankfully, at this point gas in both directions is the most common issue, and not that bad. Hope the transition to Dovato improves things for you. Cheers!

      Yes you are correct, they are all toxic and are causing some sort of damage along the way as they pass through the liver and blood, also the kidneys. In addition also parasites and other heavy metals are almost in everyone and that adds to that toxic load as well. This is why a detox of heavy metals is very important to rid your body, cells, bones, tissues and organs of heavy metal, poisons, pesticides, herbicides, insecticides and medications if you want your body to function at optimal level. Many people lack this knowledge and overloading their bodies on multiple medications with no detoxing which is killing the liver and kidneys. These toxins are breathed in, are in tap water and in our food supply chains. We are in constant bombardment.     Thanks to Raif's videos and getting me talk to my own doctor over the last few years I would have zero problems dating someone who is HIV+. I am on PreP, if he is undetectable, then chances are pretty minor.

      Unfortunately when I had to switch my health insurance to Kaiser I was no longer able to see Gladstein 😩 I really hated that. The other option you might want to ask about is Juluca, also Dolutegravir but combined with Rilpivirine instead of Lamivudine. And thanks for all the love and support. So very much appreciated! It’s encouraging to hear

    Hi Raif, by any chance did you ever consider the injectable med (I think it’s called cabenuva) instead of doing dovato? since it is not an oral route drug, it may also reduce the possibility of exacerbation of gi symptoms you may be having with the oral meds

    I also swapped from Biktarvy to Dovato because of the weight gain. But like Raif, there were other factors in play.

    Hi Raif,
    RE the legal doc. You can upload it as a doc to Chat GPT and then ask it to create summaries, and ask any question you like about the document and chat GTP will answer it accurately. I found this works best with chat GTP 4.0, 3.5 is still subject to what’s called hallucinations it means it can make up answers but 4.0 rarely does this. The downside is that there’s a cost for chat GPT4.0.

    I think ViiV also makes the drug Juluca. Why did you choose Dovato over Juluca? Pros/Cons?
    The toxicity is around the effect on your kidneys. Some drugs - HIV or otherwise - have negative impacts on the kidneys. The big downside to the old wonder Rx, Atripla, was how it might effect kidney function and creatinine levels.

    I have been on Dovato for about a year now. I have tiredness on a daily basis. My job helps with that as an Animal Control Officer, it keeps me on my feet and moving, however, during my days off its very hard to get motivated. I made the switch from Biktarvy as the Tenofivir was destroying my only kidney! I loved Biktarvy and I had no side effects that I can think of that was a direct result of the meds besides the kidney problems! The only new symptoms I have is shaking and tiredness, which could be a result of other meds I take! Thank you so much for sharing your experiences with both of these medications fam🤙

    I would like you to comment on weight gain, especially around the abdomen, I am taking Delstrigo and I have gained weight 16 lbs of visceral fat around my abdomen. I also am experiencing some severe low blood sugars. I used to work out three times a week was very active prior to going on Delstrigo. I can’t remember what I was on before but I have been battling with the weight gain, gall stones, and non-alcoholic fatty liver disease. So I am vegetarian I don’t eat red meat or any meat. My daily calorie consumption is 1700 to 1800 cal a day.
    I try to walk every day, but the low blood sugars I experience have interfered with my serious workouts. I weigh the same 180 but body composition tests have shown that I’ve lost muscle mass and gained fat. My question is do you think a switch to a different medication would help alleviate this?
    I am on supplements and eat a good diet with no fast foods or fried foods.

    @RaifDerrazi I ended up switching to Delstrigo. The pill is huge compared to Biktarvy. While on the latter I gained over 70 pounds in about three months. I've luckily never had Covid but I also experienced extreme exhaustion on Bik. Now on Delstrigo I'm feeling a bit better. Ended up starting Ozempic as the weight would not go down and that has been helping a lot.

    I’m currently taking Triumeq. The first time I took it I actually felt like I was dying, the side effects were so bad for the first 24 hours. Thankfully it settled down, but i do get a little gassy 😬😳 which can be rather stressful in the wrong company 😂. But having a high protein diet could contribute to that maybe?.

    Hi Raif, please do a video on HIV drug resistance. The problem is terrifying but real and it seems to be a huge taboo in our community. According to the WHO, over 10% of HIV patients in the world are affected by drug resistance: what causes this and what can we do about it.

      I wanna make it short: it's the cleverness of nature (or possibly evilness in this case) that causes drug resistance. HIV has this incredible ability to mutate (mutation on steroid, basically) that, all of our mathematical epidemiological models have predicted that, all drugs would eventually fail. BUT, I have to stress BUT, all hope is NOT lost. Last-line-of-defence drugs, such as ones that prevent viral attachment onto cells, dramatically reduce the risk of drug-resistance. Furthermore, I have immense hope for gene therapies coming in the pipelines. KEEP THE FAITH!

      @@hoangleminh6990 Absolutely! I have great hope in gene therapy too but we need to be pragmatic and start acting today to preserve the efficacy of our treatments and protect our health. I heard a + guy say that his ART protects him against future re-infections but this seems to be a huge mistake: when you turn +, you have to be even more cautious and avoid risk contacts! I love my current treatment (Juluca) and I hope it doesn't lose its efficacy any time soon.

    Been on Biktarvy and yes has caused weight gain. Was meant to change to Dovato but result viral load had a very slight viral blip which on 2nd blood test went back to to 14 copies. Also not sure if taking supplements might interact with Biktarvy

    Hi Raif, I was taking Biktarvy as my first line treatment. for first three months I didn't have any side effects. But after 4 Months with Biktarvy I started getting rigging ears (Tinnitus) so I asked my Doctor if I could switch to Dovato to see if goes away and nope I still have it. do you have Tinnitus as well?

    Do you feel it was hard to gain muscle on Biktarvy? I feel Biktarvy puts the body in a diabetic like state. I was taking black seed oil which modulated my glucose response and I was purposely eating fiber as my second meal , and avoid carbs in the beginning of the day. I wasn’t gain weight but my body just looked “softer”. I always had a “muscular” appearance while also being lean. Now that I’ve started binging for about 2-3 weeks. I want to gain weight, I want to know if it will be harder for me to build muscle while eating properly. Were your weight problems persisting while you were on Biktarvy and trying to lose weight?

      As you can see in my bodybuilding photo I had very good muscle development and I was extremely lean. I was very much able to achieve my fitness goals. But everyone is different so I can’t say how it might affect you. I just have the mentality of assuming that I have control over my body and adjust accordingly

    Dolutegravir seems to be the star ingredient these days. I'm on Juluca (dolutegravir + rilpivirine), 1 pill daily and I love it, zero side effects.

      @@RaifDerrazi Def consider Juluca. Only thing about it is it has take with meal. Very similar to Dovato by same company, but probably less powerful than Dovato hence probably less toxic, which is used for "maintenance" drug only.

      @@dino91001 Juluca is a recent drug and my nurse called it 'the Rolls Royce of HIV medications'. It's a brilliant treatment, I couldn't be happier and I thank God for this, I am truly blessed.

    I just had a exposure on aug 15 and from next day i have every symptoms of hiv till now and experiencing very much fatigue and lethargy had rash and swollen lymp node today and had fever on aug16 to 19 ,my doc wanna me to test me this month last ,i lost hope and i spoiled my life , eventhough i had protected sex i dont know how i got hiv ,i just lost hope and dont know how am i gonna live,it hurts.

    And to my knowledge one less drug for your liver and kidney to filter through. Not necessarily toxic just filter out through the body because I believe from what I was able to understand it can cause liver inflammation or high liver function.

    I gained a small amount of weight on Descovy and I was delighted, because all my life I’ve been underweight and trying to eat more. But now it’s fallen off! So as much as people say Descovy (like Biktarvy) causes weight gain, I think for most people it’s only small.

    I was on byktarvy switched to Dovato because weight gain, ever since i turned poz i gained so much weight. I cant way Dovato changed anything even after 8 months or so on it. I changed my regimen to cavenuva as of late for past 6 months and seeing how it goes. Very frustrating, i hope i get better.

      Yea I’m sure it can be frustrating. Of note, a recent study came out suggesting that even though some medication leads to weight gain…going OFF of the medication did not reverse the weight gain

      Have you ever tried Genvoya for your ARV regimen?? One pill a day,minimal side effects. Gained 50lbs after being diagnosed w HIV 6 yrs ago from stress/emotional eating,recently started a high protein,low carb,zero sugar diet & have lost 30lbs in less than 90 days….realizing Genvoya is NOT the cause of my weight gain is just another win win for my ARV regimen.

    Interesting, I didn’t know that having weird dreams and depression were a side effect of Biktarvy. I don’t experience depression, but weird dreams… more often than not. Another one is tiredness, but I’ve been able to work through that one by working out more often, about 4-5 days a week. Still it would be cool to know if you’re having any of the side effects in a couple of weeks.

      I’ve experienced extreme exhaustion but I think that’s more attributable to long Covid. Gym routine DEFINITELY helps with overall energy and mood. I’ll keep you posted!

Is difficult to explain to anyone about tiredness because people think is because I’m lazy, and I’m not… I’m constantly pushing though it, as with everything some days are great with 1 training session, others are not so great. 😅

    No. Biktarvy does make you gain weight. It rounds off at 30lbs. I know a lot of people gaining weight from it

      I’m not claiming that my side effects are the same for everyone. I’m explaining why I chose to switch. and the side effects I listed are the common side effects that were discovered through clinical research. Just like the side effects on the side of a Tylenol bottle, it doesn’t mean any or most people will experience them

      Hi Raif. I am so attentive to your vlogs… you are a paragon to our Poz community.
      I was diagnosed in the mid eighties and have been extremely blessed while others have fallen around me. At 65 I won a second place award for body building but since i suffered a collision with a tractor trailer!!! and a hip replacement, abdominal surgery and lower spine fusion later I am ready to get back into the gym… I am now 76 ( whaaaaat how’d that happen??).
      Since the three years of surgery recovery I have been depressed and gained weight and am at 184 now. Can’t seem to drop anymore even though I eat diligently … I’ve been on Biktary for quite a few years and am looking forward to hearing more about your switch to Dovata
      Thank you for your educated and unabashed and informative disposition.
      ( next time you sign a HIPPA FORM think of me…. I was formative in litigation to get that into law !!)
