A Failed Defect Detector and the Train Derailment at East Palestine

  • Defect Detector had a bad day and gave multiple hot axle alerts.
    Hear the defect messages and find out why the defect detector went haywire.
    At the end of the video I will take you inside the detector cabinet and you
    can watch the detector as a train passes. There have been no future errant
    messages since this video was taken.
    Also will give you my thoughts on the horrible Norfolk Southern train
    derailment at East Palestine, Ohio. It is so very sad to see events
    like this happen. It is not good for the community nor for the railroad
    industry.
    That was an eastbound NS general merchandise freight train which derailed on
    main track 1. As a result of the derailment, 38 rail cars derailed and a fire ensued
    that damaged an additional 12 cars. There were 20 total hazardous material cars in
    the train consist, 10 of which derailed,

    The NTSB issued a preliminary report on what happened at
    East Palestine. Report was issued February 23. Here is a link to the community tab

    Dave, I have confirmation that it WAS indeed a burned off journal. Inside wheel, and what's causing all the sparks is the wheel is being pulled against the brake beam and causing the wheels to slide at high speed. The bearing was trending for 3 detectors prior, the dispatched failed to notify the crew. Like you had said this all started right after the Salem detector and by the time they got to the E. Palestine detector it was beginning to derail. They went into emergency before the engineer could react, and that's all she wrote. This information comes directly from NS supervision at Conway yard, relayed to my my supervision.

      I have no clue what NS rules are as far as trending bearings go, ie.,
      how much of a bearing temperature increase is allowed between detectors, let us hope dispatch followed NS protocol on that.

      @@MaidensWorldOfWax Electronic detectors and their transmitter supposedly send information on any anomaly with the train to the engineer by shortwave radio. Number of axels back from the loco if any errors and total axels if all was OK at the end of the train.

      I bet the people with the 3 initials in the offices on the top floor told dispatch to ignore the alarms and get the train to the destination Because it was running late.
      Precision railroading is going to kill a lot of people

    Very informative. Years ago for a couple of mornings I heard a screeching noise. Didn't know the source. I happened to be up earlier another morning and heard it approaching from a distance. Got dressed and walked 1/8 mile to the end of our street to the rail line. Train was coming out of the yard 2 miles away. Sound was very loud. Got the numbers from the car and called the railroad. Apparently they took care of the problem. How it went undetected for several days is scary.

    Wow Dave, I learned more from you in 10 minutes about the Ohio NS derailment than the 2 weeks of so-called reporting in the news, NS press releases or NTSB reports, and even though speculation, you explained enough for me to understand what I see there now, thank you!

      The weird thing is that they enact new regulations to supposedly change things for the better but end up doing nothing. There has been a lot of talk about improved braking technology but this latest accident is most likely nothing to do with that. To put in perspective on a 150 car train there are 1200 wheel bearings. Only one failure most likely lead to this. The other thing is a safety system that gives to many false alarms will be ignored.

      You cannot return to the 3x5 mile downwind area safely for a long time or soil must be removed. Children, gardens and pets will be exposed to a cocktail from incomplete combustion and soot.

      @@SovereignTroll working on a solutions video, corn and lavender can pull contaminates from the soil, they're used to reclaim mining land that's been wrecked. Various fungus and bacteria can break down contaminates into non toxic elements. Activated charcoal can be used in your soil to neutralize (for lack of better word atm) the toxins as well, holding them in the little crevices until they break down or some worm, fungus or bacteria breaks them down first. Restoring the land can be done and rather quickly implementing these techniques. The water is what I'm more concerned about. I need to research more about this and the natural path from the area affected to give a respectable solution.
      The fear-porn click bait videos without solutions is no help to the community and much of it isn't accurate. It's very frustrating. Seems there's an effort to bolster distrust in public servants and businesses which do themselves no help by their corruption and greed. This building up, unchecked will only bait us into destroying ourselves from within. Not a good thing. We got to build and fix things, not burn ourselves down so the enemy can swoop in and finish us off.

    Working at a plant that builds and tests locos for a living, we here can appreciate the complexities of these various safety systems, and the fact that sometimes the sensor itself is the problem.

      So now I'm wondering if the electronic brake systems that there's been so much attention focused on would have made any difference, since the derail wasn't apparently even verified until after the train had already stopped. 🤔
      My understanding is that it would auto apply brakes to cars that are *behind* the derail, to prevent them crashing into the ones ahead of them.
      But then, aren't air brakes designed to "fail-safe" and apply braking if they lose air pressure anyways? 🤔

    Back in the late 1970's when I was in junior high school we were sitting at a crossing in town and one of the cars had a flaming wheel box. As soon as we got home my dad called the local police department and reported it to them so they could call the railroad.

    Hi Dave!
    BTW, those tank cars derailed in accordion fashion because they are equipped with shelf couplers.
    Designed to keep cars from coming apart and having a coupler puncture the tank car.

      great video. kinda worrisome that a space heater not working caused a issue. there are way more reliable means to maintain certain temps than that. with redundancy. probably cheaper too! subscribed ! look forwards to watching more on your content

      @@ccrx6700 I do Electronics as a job also I would guess it got cold and you had water between 2 spots on 1 of the green board's. Once the temp got up the water went away. Fault now gone.

    Thanks for sharing your thoughts with us, Dave. I've heard all sorts of crazy claims, including the idea that Norfolk Southern is derailing trains on purpose for some mysterious, nefarious purpose. Glad to have a dose of sanity and reason.
    Not surprised that a computer would malfunction with the heater off -- they do tend to be sensitive to temperature and moisture. :)

      Your very welcome MB. Thank you for the very nice comment. I listened to several news broadcasts and these reporters don't have
      a clue about railroading. Conspiracy theorists seem to abound
      in this day and age don't they. These events are very sad and
      cast a dim light on the entire rail industry. Very, very sad to see
      this happen. We really appreciate your watching and writing in
      my friend.
      As of yesterday NTSB confirmed a wheel bearing went bad
      and the train went into emergency after the car derailed and
      air hoses came apart. The axle with the bad bearing did skew
      and press against the brake beam which applied the brakes
      as it was going down track before the derailment. So I was
      right in that the car wheels were sliding causing the sparks.

I am a railfan and subscriber to your channel and do not believe the wreck was intentionally caused by crew or maintenance. I DO believe we are being constantly lied to by those who we pay and expect to be truthful. The word "secret" is repugnant in a free society. JFK

    Thanks for the teaching on this. As you said you are not this expert on this incident, but your understanding into what MIGHT have happened is still highly interesting.       that most folks don't know about with detectors and derailments.

    tragedy! I too am a former rail-
    roader and feel sorry for the
    people getting flack while just
    doing their jobs, whether train
    crew, investigators or whoever.
    I’d hate to see anyone lose their
    job in a knee jerk reaction, instead of waiting for all the info
    is in! I also feel sorry for NS as a
    whole, or maybe railroading in
    general, as this is going to be
    very expensive!

    I found this to be educational...with the wreck in Ohio people for some reason have been more vocal about what railroads transport and the safety measures they take. Accidents happen but catastrophic ones are hard to get over.

      "for some reason" come TF on dude. You know the reason. And the railroad unions have been warning of staff shortages and cuts leading to less maintenance getting done, and less people for car checks to find problems.

      The economic devotion to Profits before People seems to become evident as the Investors appear to be the only folks the Government defends?
      Shareholder values seem to have wandered beyond the limitations of human ability?
      Hollywood could not have written a more stupid script......

    Thanks for sharing yes scary stuff no matter how hard we try it all happens sometimes. And hot box detectors like axle counters, scotch blocks etc all can have intermittent faults...we have a Junction here in the State of Victoria in Australia called Newport near Melbourne, it used to be an old interlocking manned frame signal-box, looked like it was straight out of England in the 1800s it never failed now its been up graded with electronics, every time it rains it faults out all signals back to danger tracks down etc....we now call Newport "malfunction junction"....lol...thanks for sharing...regards

      We have 4 electric switches, 3 work perfect all the time, every
      time it rains the 4th one shorts out and needs to be reset. Had
      a guy from Union Pacific tell me they had a similar problem with
      a new electric switch and finally just had to put a whole new
      one in. Malfunction junction... I like that. Always appreciate
      your visiting and hearing from you my friend from down under.

    Dave,
    Great video as usual! As you and I have spoken about many times before. Things happen on the rails no one could ever guess why. So much technology has been Intergraged over the years some for the best some not so good! Nothing and I mean nothing will ever replace the human eyes the human touch or smell. Those are all useful on the railroad. You can see when something is not right you can feel when something is not right and you can smell when something is not right. There is something about my sight feel and smell that's not right about the NS derailment....like you I was not there I was not at the throttle something caused the train to go into e brake busted hose or in the cab. As your video proved defeat degectors are not always 100 percent...as bad as this sounds I hope that was the case and the in cab crew didn't ignore the warning
    ..time will tell...let the cards fall as they may either way NS has a major clean up

      After working in this industry for decades I am going to take a slight exception to the comment "nothing will replace human eyes'. I get what you are saying and I agree...somewhat. But humans make mistakes. They don't pay attention. They run red lights. The speed. They cut you off in traffic. They don't look back over their train. They don't check their airplane before taking off. With trains exceeding a mile in length more dependence on technology becomes vital.

      @@cdavid8139 I agree with what you are saying too. However, we have become so dependent on technology that it has made us lazy. In my final years with norfolk southern. All I had to do was rely on trip optimizer and let it go. More knuckles were broken while running TO than any other time in my career...first thing that's asked was from the dispatcher...was train in trip optimzer...once I said yes they said ok.... had I said no paperwork meetings and all kinda mess would have been in play. You can listen to a locomotive feel it beneath you and know what's going on.... guages are good they make it easier. You can smell a scorched pan of potatoes and not have to stir the pot so to speak. We are a computerized world like it or lump it we have taken so much of the human element out of every day life. Railroads are no exception. Given Dave's situation in his latest video. They had 4 people pk the ground as the train went by ...don't think for one minute a class 1 would have done that...only after a derailment

On the other hand you know for a fact that there are trainmen (and truckers and sailors) who are not 100 percent dependable. How many times when you were running did you find yourself nodding off or look over and see your conductor paying no attention to what was going on around them. Somewhere out there a blend of technology and common sense works best

      I just mentioned in another video about planes encountering more close calls that the human condition is what it is and functions poorly in boredom. I mentioned in yet another video thread that the timing of this incident immediately followed the spy matter. Just how much of rail load contents is available on line or is it all internal? Who is this administration allowing into our country? This aspect CANNOT be dismissed. Can these sensors be temporarily defeated and no one be the wiser?

      From inside sources, the dispatcher ignored the trending hotbox alert. The bearing failed after passing a detector and once it was a hit it was too late, no way to stop in time.

      Thank you Eric. NTSB has recovered the truck set that first
      derailed and it is being sent to a lab for investigation. What we
      did find out today, new info. A bearing failed, axle skewed and
      was pressing against the brake beam which acted like the
      brakes were applied and the wheels were sliding on the rails,
      so I was right in part of my speculation. Thanks so much for

      Thank you fireball, don't know about expert, but have come to
      find out the truck set wheels were in fact sliding on the rails
      causing the sparks. A wheel bearing had gone bad and skewed
      the axle so it was pushing against the brake beam holding
      the brakes on. That new info just came to light this afternoon
      by NTSB.Always appreciate your watching and writing in my friend.

    Thank you for explaining about the train going into emergency and the cars ending up jack knifed in a zig-zag fashion. I had wondered about that since seeing the first reports of this. If I understood you correctly, you mentioned being about 50 miles away from the derailment in PA. I hope you are now where close to being in the down wind path of the smoke. I feel really sorry for those people, not just in East Palestine, but anyone within 50 to 100 miles down wind is likely to have toxic fall out on their property. And, anyone along the Ohio River for many miles is likely to see the results of contamination. You be careful and stay safe.

      the past were written in blood. Hopefully new and improved
      safety regulations will now be made for the industry.

      It also sounds like the people running these trains have to be A. Brave, lol, bc I ain't getting out and walking the entire length of a train, on track in the middle of the woods, alone at night lol!
      And B. They also seem to have to be extremely patient... Because this man said "at walking speed" 😳 THAT must make for a LONG night at work!!!
      Given the people running the train that night wasn't rushing or that they they did everything they could to prevent this or even didn't know there was problem....and I don't even know if they lived, I haven't looked that much into it....I definitely appreciate the ones that do their job to a T even if it means traveling at a walking pace, for safety sake, and not getting home for probably HOURS later than they were supposed to.
      We live next to a railroad track where a train comes through often and there are a LOT of children living in just my apartment complex alone. I can't imagine that happening right outside my back door!! It's very scary, but it's also comforting to know there are guys out there that absolutely do everything right to prevent accidents from happening and communities from having to deal with utter destruction.

    Glad you put in the caption comments. I know you were NOT assigning blame, because I've watched enough of your videos that I think I know you wouldn't do that. But adding the captions certainly helps to clarify that. Like explaining that it wasn't the engineer that threw the train into emergency, but rather the separation of the brake line.
    Good job to hear from someone knowledgeable and that you don't second-guess what happens. If only more news outlets would do the same. I've heard so many networks speculating about everything from unions, to regulators, to who-knows-what. Before all the facts are known.
    Just glad nobody was killed, and hope there won't be any long-lasting effects of the spill.

    Thank you very much for this very interesting and informative video presentation which is very much appreciated by the people. We are grateful for talking about the situation in Ohio, though I am sure that there will be a major enquiry into what happened. It shows the importance of these hot axle / hot wheel detectors. Just a little warm air seemed to sort out the problem of your detector. I am aware that electrical items do not like things too cold or for that matter too hot. It is always reassuring to hear those words, "no defects".

    We had a similar situation last month with a hot wheel on an eastbound train, we have multiple hot wheel detectors along the ROW and the dispatchers are always monitoring it, the hot wheel caught the attention of the dispatcher and let the train continue on until it passed the third hot wheel detector and notified the train crew to set out the car in town.

      Apparently new info out Jason, this bearing went bad which caused
      the skewed axle to press against the brake pads which sent the
      wheel sliding along the rail. However also, this bearing had
      been trending for 3 detectors back from E. Palestine. Since
      the detector did not give an alarm, engineer didn't get a message.
      However NS had it set up so a warning message was sent
      to dispatch about it trending. We do not know what NS protocol
      is for the amount of temp increase allowed for each succeeding
      detector, nor do we know the bearing temps at each detector.
      Hope and pray dispatch followed NS rules.

Back when I was railroadin hot boxes were checked immediately but reading the comments it seems the procedure has changed and the crew may not even be notified of a developing issue until apparently it's too late. Seems a terrible practice to delay Inspection

      getting hot, engineer did not get a message because an alarm
      was not sent by the detector. However detector was programmed
      to send a warning message to dispatch of a trending hot bearing.
      We do not know what NS protocol is for how much of a temp
      increase is allowed between detectors nor do we know the
      exact bearing temps that were recorded. NTSB will find that
      info out. Appreciate your watching my friend.

    Dave, I know exactly where that hot wheel (and dragging equipment) detector in Salem is. Visited the very same spot with an NS signal technician a few years back when I was doing some consulting work for one of their suppliers. You guys aren't the only ones who have to constantly deal with issues in those detectors. Given the environment that those sensors have to endure it's a wonder they work at all. What the NS guys really hated about them was when it snows they constantly have to run around clearing the snow off of them to keep them from throwing faults when a train passes over.

      The detectors have a camera and a noncontact thermopile like the kind in a forehead fever thermometer inside. If dirt or snow or water gets in, they are not gonna work properly.

    Nice explanation Dave. I don't know about NS, but I know BNSF has gone to "talk on defect only" on their wayside detectors (as a railfan that was a disappointment because I used to hear the detectors when taking pictures so I could predict when a train was near my location). So it is possible that the defector was inoperative and didn't announce a defect - and if NS has that same rule, the crew might of not known there was an alarm. From the news reports I've seen the NTSB seems to be on that and they've pulled the detector's local logs, so when the final report comes out it will say if the detector was malfunctioning or not.

      @@JetSetDiva In BNSF's case, the "talk on defect only" does announce "Detector Working" when a hi-rail truck goes over it, presumably to let the track inspector or work crew know the detector detected them, but again, it doesn't say if each of the various elements are working.
      It's been a few years, but I think the move was more to cut down on the radio traffic or have the detector "walk" over the dispatcher/crews than anything.

      NS on the Fort Wayne line has message given to the engineer
      when their is a critical alarm, not a warning. Trending warning
      message are sent to dispatch. Also on our detector there has
      to be a minimum of 5 axles cross before it will give any message,
      that is to prevent MOW equipment from setting it off. However
      it still reads MOW bearing temps, if there was a hot bearing
      on an MOW equipment then it would announce that.

    According to the reports coming out now from the ongoing investigation it was a bearing that failed. Not dissimilar to the one you had on you're coal train some time back. Camera footage showing it in the "late stages of bearing faleure" before derailment. And they are sending what's left off for metallurgical testing to try to figure out why it failed. Basicly the root cause was a mantinence issue, failed mechanical part. There's a growing debate about federal regs that were scrapped years ago that could have minimized the severity of the derailment, aswell as questions about exactly why the clearly hazardous cargo wasn't requiered to be handled as such by NS via regulations. There seems to be a lot of blame being put on the govt by individuals online (partisan politics) and even consperacy theories propagating. All of which I find shameful. Real lives are being impacted by this, dead pets, risk of serious cancers down the road, water and land contamination. And the hyper politicization helps none of them.

But the responsibility lies in the decisions that made the derailment worse via opposing/lobbying against electronic breaking mandate, running much longer trains, even axle builders buying cheap Chinese bearings that don't hold up as long or as well compared to US produced.

    Dave, nothing worse to do than diagnose an intermittent problem. I would think there's a way to look at a history report in that computer and see exactly what the readings are from the infrared cameras. That way you could at least tell which of the 4 cameras is flaky. Interesting stuff and thanks for educating us.

      100 trains then starts to write over them. Our guy was on phone
      later that day with tech from progress rail, although I don't know
      what they came up with but detector has worked perfectly since.

    Dave I want to tell you something buddy- you’re doing a great service to everyone by putting together this video. It’s much appreciated!
    East Palestine is truly a catastrophe, but the government’s response to intentionally ignore the residents health and safety concerns along with denying assistance like FEMA, is a tragedy also.
    Those people were sacrificed by the “controlled” burn and forsaken by their government who seems to value Ukrainian lives more.

      the events that happened. This is a very high profile case with

    At the end of the video I was laughing so hard at the defect detector! Especially when turning a space heater on fixed the issue.. That's one those gremlin situations! Interesting to see those situations don't just effect the model train world..

    Great video Dave. I never saw the inside of a detector before. I see Progress rail services manufactures them. Guy that worked in the rail mill with me for awhile got a job there in the 1990s.I think that company is in Cleveland Ohio.

    😉Thanks for the Video, I appreciate all of them.
    One thing that bugs me about the tanker derailment is that it has been common practice to put two or three inert unhazardous train cars between every two tanker cars carrying hazardous materials in case of derailment. In the overhead cluster camera shots of those tankers it didn't appear that there were ANY buffer cars!? That obviously wasn't a very good idea was it?🤔

    Given this, I wonder if the defect detector was giving off the hot axle alert so many times that at one point, people just assumed a train was fine when in reality (as was the case in what would have soon became East Palestine) there truly was a defect. I think this may be a big reason why the crew likely didn't know that there was a defect until it was too late, but we'll have to wait on the official NTSB report for that.

      What happened at E. Palestine was a tragedy. We feel so sorry for all
      involved, the residents and the RR. We will await for the official
      NTSB report. They will investigate every single detail of all
      the events that happened. This is a very high profile case with
      all the lawsuits filed and environmental concerns. A lot of
      money will be riding on their findings.
      So very sad it happened. In the past a lot of RR safety rules
      have been written in blood sadly. Hopefully this tragic incidence
      will bring into play a new, better set of safety regulations for
      the rail industry. We certainly do appreciate your watching
      the video and for writing in my friend.

    Dave, thank you very much for your explanation of how the bearing and wheel sensors work. I also appreciate your estimates of what happened at East Palestine and POSSIBLE causes keeping it simple for even an old sailor to understand.
    The computer running your detector shares something in common with a lot of other computers ... it's sensitive to temperatures it has to operate in. Does it display similar problems on very hot summer days with no A/C? I wouldn't be surprised if it did.

    Great video Dave! So much of the news reporting has been incorrect, from my limited knowledge of railroading. East Palestine is a short drive from home, but I am glad I am upwind and out of the affected area. Great demonstration on defect detectors!

      Yes sir I will. NTSB has recovered the truck set that first
      derailed and it is being sent to a lab for investigation. What we
      did find out today, new info. A bearing failed, axle skewed and
      was pressing against the brake beam which acted like the
      brakes were applied and the wheels were sliding on the rails,
      so I was right in part of my speculation. Thanks so much for

    Great video - I learn something new each time you put out a new video. Regarding the detectors: how far apart are they usually placed? Does a track with a lot of curves usually require more detectors than an alignment that is predominantly tangent? Thanks for your response.

      1 detector. NS typically puts theirs 20 or so miles apart, some
      other RR put them closer or further, it's all up to the engineering
      department of each RR where they decide to put them. Thanks so

    Good stuff Dave, I was a telecom tech for the Santa Fe, Union Pacific and BNSF Railway in Los Angeles for 25 1/2 before retiring in 2021 and one of my duties was to main the voice and data radios for our Santa Fe Springs hot box detector which was about eight miles from Hobart Yard in Los Angeles. It was an interesting site because about eight years ago they went from having double track to triple track at that location and the data radio was only capable of handling inputs from two tracks. I had to rescript the radio get it to handle three track inputs and make some configuration changes the Progress Rail Services computer and install a multiplexer box to allow the system to handle three track inputs. It was really cool to see and hear it work once all the parameters were properly tweaked.

      @@ccrx6700 The detector at Santa Fe Springs California is the one I used to maintain the vhf hi band voice radio and the vhf low band data radio in and sometimes it would hiccup and give squirrels reports to passing trains.

    I appreciate everything the engineers and other vital workers such as yourself go out and do such an amazing job. I know safety is always a number 1 priority. It is a shame the accident happened and I wish there was more that could of been done but we have car accidents and plane accidents out there too. Some are human error and some are just machinery related. The same people that complain about accidents happening in their hometown are probably the same individuals that run through a railroad crossing when the signals are activated.     A better heater might be one of those mineral oil-filled little radiators that have a thermostat and no fan to get clogged or burn out.

      was broke so the space heater was only temporary until HVAC

    There are five rail crossings in Salem. The video is taken after the fifth one going westbound. There are 12 more crossings before East Palestine, and some are state highways. Hell, two are main streets in Columbiana, and no one saw this rolling fireworks display. There are many open country places where the train could be seen from highways. It was dark, with no leaves on the trees. Puzzling, to say the least. Seventeen crossings and no eyeballs on the train.

    Great video. Your information is 5x what comes out of the media. It's unfortunate that Americans suffer while the News media covers it up because they have stake in the rail roads. Keep us up to date with the news. Luckily for us rhere are experts like you breaking things down into terms we can understand!     Thanks Dave for the insight. I have read many of the posts by professionals in the RR industry and their collective wisdom suggests a failure in on of the wheels. Personally, I thought it might have been a track failure. I see how much time and energy you put into maintaining your track in good condition, and wondered if poor track maintenance might have been the problem. Guess I was wrong, but you provided the information I learned about RRing to make a guess about what happened. Thanks :)

    I thought hotbox, but locked brakes makes more sense. I understand it was a 150 car train, which even before remote controlled distributed power was pretty standard for an Ohio Valley train. The was with caboose, and it was up to the train crew on both ends to look and sniff for stuff like sticking brakes.
    I look forward to getting a clearer picture of these events. Keep up the good work.

      Your welcome Dave. NSTB new info out today, was a bad bearing
      that caused the axle to press against the brake beam and cause
      the car wheels to slide along the rail creating the sparks. So I
      was right in part of my speculation. Dispatch got a message
      from several previous detectors that a bearing was getting
      warmer. We don't know yet any other info. Always a pleasure
      to have you visit with us and hear from you my friend.
