With terminal illness, she has amnesia. Can't remember him at all. As they are looking through a bunch of photos, she doesn't remember anything except for the big lollipop with a smiley face on it(웃음 사탕). As the bills pile up, he gets desperate to make money for her surgery and does illegal things just to get it done. She gets worse and they have to operate and as that's happening, he turns himself in to the police. The song pretty much describes how much it hurts now that she is no longer there(as in amnesia and literally dying) but like a fool he is smiling and will continue to smile for her. He misses her but she can no longer see him as the same person, he calls for her but there is no reply. Even though it hurts so much, he will smile. He loved her and only her more than anything in the world but she is no longer there.
i stayed in hospital cuz of my broken ankle for 4 days. last day all i had oily hair,some moustache,bunch of black dots on my nose,long and half printed nails... thanks to fair..
