    That scene where House took his O2 mask off was so well acted and intense. First time I've ever heard him use that "everybody lies" phrase in a comforting way before either, like he meant "don't worry, everybody else lies too, don't let your secrets kill you"

    That issue of publication bias is pretty widespread; for quite a long time most journals rarely published negative results. Now, there are a few journals with this information (JNR, Positively Negative etc.), but it's still not that common.

      Haha I’ve seen him do so much crazy stuff that my threshold for calling out his insanity has gone up slightly. That being said though I still love seeing it as it’s a lot of fun to watch.

    I was spiked with GHB in a nightclub while I was at university, luckily it was merely as a tool to commit robbery rather than the sort of crime nobody ever fully recovers from. I had no idea what had happened to me or how I was somehow in an unfamiliar area of town hours after the last place I remembered being with everything of value I owned missing. I needed to make sense of it for myself and paid for a blood test that is how I found out what they did to me. On the bright side targeting someone that resorts to a lab to explain what happened to them was probably a bad idea as a prosecutor from the CPS called to ask for a copy of the report 6 months later.

    To be fair if I was sure I wasn't having sex my first thought when a doctor said "some sort of middle ground" wouldnt be im having sex without my own knowledge

    As I recall, brain damage starts 3 minutes after the oxygen supply is cut off. Obviously this is variable based on how well oxygenated the person was before the supply was cut off. For example someone who was already struggling due to COPD, lung infection, exposure to Carbon Monoxide leading to high carboxylate-haemoglobin &c will hit problems sooner.

    Very based and sensible approach from you this episode, really enjoyed the commentary on the breaking down of the "victim mentality" for minorities, since as a minority myself, i see it too often when i think it's entirely possible to be whatever you want to be regardless of skin color in our modern times. I thought it was such a good approach to back up what you were saying with references to a research study, that "Dartmouth Scar experiment", showing that a biased view on discrimination is a self-defeating philosophy. Also that jab at tik-tok was tier 1, my guy has all the correct opinions lol.

    On the topic of patient self exposure. I had the opposite trauma of when I was young, I once had a rash on my upper thigh, as soon as I entered the examination room, my aunt who took me there ripped my pants off for the doctor to examine. Not 5 seconds later, a previous patient of that doctor barged in for a quick question.
    And there I was, 10 year old dick in the wind, for an entire corridor of queuing patients to see. I still dream of it every now and then.

    I really liked the actress playing the sexsomniac. She was cute, but spicy - and formed a cool relationship with House despite his outrageous (but correct) diagnosis. That look she gave him when he said "I sense unresolved issues" (not in this clip), it made me laugh my arse off.

    It is pretty wild how, in almost every single one of these episodes, amongst your initial guesses is Lupus, consistently, and without a hint of irony. As someone who knows nothing about it, what the hell even is Lupus? Does it just cause like every symptom under the sun? Like some unholy amalgamation of every disease ever created, spliced together in some hellish umbrella corp-esque underground laboratory in a mansion in the mountains somewhere?

    Amazing reaction! I've really learned a lot with this video, the episode itself left me with some questions on how all of it was possible, so i'm very glad you could enlighten me.       House's speech in the end is so very House and so satisfying. it reminds me of Alan Shore's lawyering in Boston Legal, if you've seen it - it's so masterful you don't even care about the consequences :D

    So, weird story relating to sexsomnia: a former co-worker of mine got arrested on charges of molesting his underage daughter. His defense to the police and in court was that he was asleep when it happened and that he had sexsomnia. Turns out, he somewhat confessed that he was indeed awake during police interrogation, and that was admissible in court. After one mistrial, he was convicted and sentenced to 30 years in prison. This episode always reminds me of following that trial while it was happening.
    Also, moral of this episode? Don't hide your medical history from your doctors. Actually, that's a takeaway from a fair percentage of House episodes, now that I think of it.

      Hi Echelar! Woah that story is insane. I imagine so many people would use this as an excuse to try and get out of accusations of cheating or crimes. Glad to hear he was put away for it and they found out eventually.
      I guess they will do an extensive assessment before letting that one get through, with sleep studies, thankfully!
      Re the moral - you are definitely right. The epilepsy was the key to getting this one solved. Great episode - thanks for asking me to include the clinic scenes. I think it makes the reactions so much better!

    Really enjoying binging your videos recently! They never fail to be both informative and entertaining. I work in Veterinary and have heard it is around 6 minutes before lack of O2 causes permanent brain damage. I imagine this would be the same in humans?

    The thing that really bothers me about drugs trials is that the bar is so low for a drug getting through. All the manufacturer has to show is that their drug is better than a placebo. They don’t have to show it’s better than a competitor or than the previous version, just better than no drug. Each drug could literally be worse than the one before it but that would not be shown by clinical trials as it’s not tested.

      Haha glad you enjoyed the theory. Looking back I think I got a little carried away but what’s life without some imagination?!
      Yeah so I usually give early access to members on videos that are being released soon but still set them to public soon after. I have actually recorded a decent distance ahead but my editor is the limiting factor for releasing them.
      I don’t want to have content behind a paywall permanently but having a longer gap between having episodes available to members and then to public could help to give you members more value.     The absolute prevalence of HIV being higher among heterosexuals despite the higher risk among homosexual males makes sense simply by considering the vastly different base rate prevalence among the general population. The reality is that cishetrosexuals are by far the dominant group in the general population so pretty much any condition that can possibly affect the dominant group will probably affect more of them if you actually look for it. It probably always has been that way and I am not even convinced that homosexuality really is a risk factor as we have failed to look for HIV as a disease in the vast majority of the population for reasons that have no basis in evidenced based medicine. Last I checked viruses still work on the cellular level and lack any ability to change their behaviour based on the choices made by over a kilogram of neurons they are unable to perceive the existence of let alone interact with at the level needed to respond to human choices lol.

    7:40 you said immaculate conception, but i think you meant virgin birth.
    Virgin birth is getting pregnant without sex.
    Immaculate conception is that Mary was devoid of original sin, as how could the woman that carried the messiah be tainted.

    I don't usually post corrections in the comments to a video, but considering this video is about education and in a friendly way pointing out how the show does or fails to do medicine... I'll share one friendly correction here if I may, Dr. Mezher. :)
    In the part of the show in the clinic, the woman patient denied having intercourse and that prompted a discussion of "immaculate conception". A very common mistake to make, but theologically speaking this is the incorrect term to use. Immaculate Conception means to be born without Original Sin and is, as far as I know, uniquely Catholic AND they only apply it to Mary, Mother of Jesus and not Jesus himself. Immaculate Conception is not virgin birth. Wild isn't it!

    It's been a while since I handled an ortho or Neuro case, I don't think it's fair to broadly ask tendon reflex speeds, as those can range significantly. Generally, unless it is a slower, extremity I was told 10 ms or above is concerning. And if I recall I think something less was average.

    Symptomatic toxoplasmosis would still be a zebra in human, sure the parasite itself is not uncommon as evidenced by the fact that a significant portion of human adults have antibodies against it floating about. Including me apparently as I provided a blood sample to a study testing that question, I for one certainly have never experienced any clearly indicative symptoms of exposure, perhaps it was one of those times we all have where we feel a little off colour like we've got a mild case of some mildly annoying nasty was it but as far as I am aware it rarely produces a diagnostically identifiable presentation in human right?

      My mic is pretty good, it’s a VideoMic NTG, but it is camera mounted at the moment. Due to your comment I have now bought a boom pole to get it closer to my mouth and that should improve the quality drastically. You should feel the difference in the videos released approximately 4 weeks from now.
