Ninja Nerds,
During this lecture you will be able to learn the EKG/ECG basics. Follow along with Zach as he will teach you the placement of ECG leads including the bipolar limb leads, augmented unipolar limb leads, and the precordial (chest) leads. Upon the completion of this video you will understand how an ECG/EKG machine generates charges to view the heart in a transverse and frontal plane, assisting in the identification of pathology. We hope you enjoy this lecture and be sure to check out all of our social media pages and ways to support us below!

To clear up any confusion with the upward deflection of T wave. So the repolarization vector we said is moving upward and rightward basically the exact opposite direction of the mean QRS vector. If you remember the mean QRS vector was positively charged because it was a depolarization wave and we said that when positive charges approach the positive electrode of lead II it causes an upward deflection. Well in the same way the repolarization vector is moving upward and right ward towards the negative electrode of lead II. The repolarization vector is a negative vector and it’s approaching the negative electrode of lead II so if that’s the case negative charge approaching negative electrode causes an upward deflection just like the positive charge moving towards positive electrode do. That is how you get the upward facing T-wave. Hope that helps Ninja’s!!
Thanksss!! Was so confused
that makes more sense now. thanks!
man , your videos are so great , wish you make more medical lessons , it really helps
Thank you so much for the clarification !
I'm just concerned about the propagation of action potential from the AV node down to the His- Purkinje fibers .
Actually I have 3 questions !
First : Does the depolarization wave start symmetrically & simultaneously on both the anterior ventricular wall & the posterior ventricular wall ?
Second : Does the repolarization wave follow the same course of the depolarization wave in the ventricles ?
Third : If the QRS complex of the ventricular depolarization overlaps the atrial repolarization wave ; why the atrial repolarization wave does NOT show in the EKG in case of 3rd degree heart block ?
Why you don't explain all medical topics
I'm still in shock at how clear this man just made ECGs
Ethan Whitt
I hope you had a nice day, hope you have a nice evening

Anyone know his genetic lineage ? No way this guy came from typical asshats
Oh yes I finally understood ECG in my third year of nursing school thanks to him WOW
Left ventricle is THICCC..😂
Seriously , when I begin to make a sum , I will surely donate to this guy❤️
Me too
Ya the THICC got me bad 😂😂😂
Sameee
I laughed out loud at once🤣
He truly deserves it
I’ve spent so much time trying to understand EKGs. Read many books, took many classes, spent a lot of money...and still didn’t understand until this man CLEARLY & EASILY explained it FOR FREE.
I can’t express my gratitude. You are a saint.
Exactly, I wished our teachers put in more efforts to bring out the concepts in a concise and more relatable way. Besides saving some time, us studdents would love education more :) Love you Ninja Nerd:-*
@@ishitasangwan5478 I can relate 100%

Me too! So So grateful to the ninja Nerd team!
Same here. It was such a relief I felt when I finally understood it. Thank you so much ninja nerd!!!
I love how you repeat each stage again, it gives us a chance to catch up each time if we didn't quite understand it or memorize it the first time.
The first 10 min blew my mind. I had to take a break. Most of us have never had a teacher as knowledgable as Zach. He is truly brilliant with an amazing mind. Thanks for sharing your brain with us!
Malay Mukhi so what buddy
Malay Mukhi man the diagram was invented by the inventor of EKG :)), not by. Dr Najeeb
SO TRUE! Amazing content :D
Well I understood
Actually !....very nicely elaborated explaination
I have an exam on Tuesday and was so worried because you didn't have a video on this .....but you uploaded this just in time. NOT ALL HEROES WEAR CAPES .
My exam on 15 Dec and it's help me a lot of understand EKG THANKS
vid-- Swedish identity? You tried, and use-- EKG monitors-- as DOD.
I'm not a doctor or med student, I just had an EKG today as a "routine" of being inducted into Medicare and officially being classified "old". I am a nerd who knows electronics and wanted to know more about how the graph was created and what it was depicting. I certainly learned all about how the pulses are detected and why there are so many wires, thank you for the well done explanation of why you hook someone up just so. It now makes me think about why the machine had so much noise that they had to run it 4 or 5 times and it apparently indicated that I was having a heart attack - well, I have my non-professional opinion about a bunch of long leads running around you, inside a typical commercial building complex, that are trying to detect pretty faint signals. The apology for the length of the video was unnecessary, it covered a heckuva lot!
I am in actual tears b/c I just sat for 6 hours in a nursing lecture and learned more from this guy on the internet for free than from my professor with a PhD in cardiac nursing
honestly by this stage, I should probably get my medical degree from ninja nerd not my college

Haha omg so true
100% Agree.
Same here🙋🏻♀️
I am a first year intern in emergency medicine. I some how got by in medical school just by memorizing EKG tips and tricks. As an intern now I know I needed a deeper understanding and you just blew every previous attempt of learning this out of the water. The first 30 minutes showed me that on a cellular level I never knew what was going on from a physics standpoint. Thank you so much, I could cry right now
yes that's very true, I do know a natural herbs doctor his name is dr moses herbal treatment on RUclips channel who can get you cure from meniere's disease or any other diseases permanently, as a Christian you also know that natural herbs comes from GOD I can give you his contact info if interested ok. 💯💯💯💯💯💯💯💯💯

You deserve all the good things in the world man, thank you for all the effort you always put in your explanations and how amazingly and naturally it all comes out
Mind-blowing how great teachers like this guy make really intimidating concepts a walk in the park. It’s really the prowess of a teacher that makes the difference, isn’t it? You can be as dumb as a rock, but if the teacher is this good, it won’t mean shit. Just imagine a world where every university professor is like this. We’d be invading Mars much sooner than Elon ever imagined. Lol. Thank you so much, professor. 👏🏻🙌🏻
I have made it through several cardio classes and NEVER has this been explained so clearly. You have a gift man! Its not that you tell us things we don't already know, but that you can string together these facts and explain the why behind it all.
I'm only 14 minutes into this video and I'm like WOW!! Very clear, concise and speaks my learning language!! God bless you my dear...I am so grateful for your gift of teaching :)
THANK YOU!!! I love how Zach would go over the previous concept from the previous picture each time to reinforce it. Best ECG training ever!
I have never been so grateful for someone in my life as I am for you. Thank you, for your explanations and the passion you show
True...
I honestly want to see teachers watching this guy for a bit
Absolutely sir 🙏 👏
Some teachers do watch this guy j/s
my lecturer brought me here😊
Our teacher don't teach us in class and told us to watch this channel
They’d say, if he can do it anyone can or some other bullshit. This is VERY unique and should be studied so others can follow
For real, I cannot believe how easy, simple and comprehensible you made this seem. Huge props
Nerd made me look like i never went to medical school...... what an underrated super intellectual hero.
I have been a nurse for 6 years and this is the most comprehensive and understandable explanation of an EKG that I have ever heard! I love that he breaks it down so well! Great job!
"Isn't it SO cool?" Yes! it is very cool, now that you have cleared up what my medical school professor couldn't! Thank you!!!!!
Point🤣👏🏻

Thank you so much Zach! ♥️
You cleared stuff that med school could never teach me. So glad that I came across your channel.
I decided to look at the videos before taking my notes. I'm glad I did! Now when I go back to take the notes it is going to be so much more effective in my comprehension. Thank you so much Zach!! I'm preparing for my Ecg class at work for my first new grad nurse position in telemetry. Your lecture will make it much easier as I study through the 250 page book !!
I can't believe it! We just saw that last week in college and as always i do, i searched that topic on youtube to find one of your great lectures and became upset because there wasn't one yet but just tonight, I got a notification from your channel just about EKG basics! I can't tell you how happy I am rn! You're my biggest assistant at medical school and i always reccommend your channel to my friends in college. I don't usually comment on videos but I really thank you with all my heart. You make life easier for me and for many other students. Please continue uploading such videos. Lectures are more fun and memorable with you than ever!

I was going to write a comment to thank him for what he does but after reading your comment, I felt like it conveys all the gratitude i have for all the helps he provides.
"You guys gonna hate me by the end of this "

I spent around 4 hours trying to understand ECG, I was looking into my book, various videos, internet pages... But I didnt understand a thing until I watched your video. Thank you very much good sir!
Just wanted to add my gratitude for the incredibly clear and thorough explanations in this video. You are an excellent teacher.
Just wanted to say that because of this video, it helped me get a 90% on my cardiac module in paramedic school! Thank you!!
I swear i watched my doctor’s 2 hour lecture and didn’t understand a word. Now i get the whole thing you’re amazing thank you
I knew I wasnt going to understand my course's lecture either so I went here XD
same here!
I'm an ECG instrument engineer,... and it's great to get the biology aspect from NINJA NERD to augment my understanding.
.- - GREAT video as always,... Time just melts away.

I work as a RN on a CV IMU. Been trying to understand the Leads and precordial leads and what they really mean in relation to the flow of the impulse. Never made sense for me and I attended a 3-day EKG course at my organization. Zach breaks down and explains the foundation of EKGs. Having been an instructor in the Navy, I am absolutely in awe of his teaching ability. Wish I had watched his videos sooner. Plan on going over the videos again and mastering EKGs before I take my CV certification. Zach, thank you. You are a blessing.
I work in a EP/Cath lab. I've been a cardiac RN for 18 years and I've taken SO many Ecg classes. You just gave the best instruction of this material I've ever seen!!
This was such a good lesson, I have a lot of trouble staying focused for an hour straight in class, here it was a pleasure. Thank you professor!
Seen many videos on basics over the years but this one is so comprehensive n covers exactly what is needed n how it should be learnt. Fantastic.!
Absolutely amazing. I enjoyed this lecture so much and found myself involved and answering your questions excitedly, like I was watching Dora the Explorer or something! Well done. Eases the worry of this paramedic student going into cardiology in 2 days!
Seriously? Two semesters of stressing EKGs and you just cleared it up for me in 20 min like ta-dah....light bulb. Bless you. Bless your videos. You are a wonderful human!!!

I stopped to Look at the comments and see if anybody else noticed when he said that 🤣🤣🤣 HE’S THICK
Again a massive thanks for quite "simplistic" approach to such a very complex topic. Being IT professional and not a medical student or professional, it was very helpful to understand the mechanics of ECG and how wide and exhausting the "view" is, by using just some electrodes and all the work done on the ECG machine side to express in a form of wave that can be quickly and effectively analysed by a professional. Huge kudos for Zach and the team!
Thank you so much for doing all of this. I'm currently, actually UNDERSTANDING what I simply parroted 2 years ago in order to pass the exams 🙏 you're a treasure 🥺💟
I just want you to know that I watched this entire thing and when I showed up to class the next day I blew my professor and classmates minds when I was able to explain where the depolarization occurs with each wave. He never even taught us why there are negative and positive deflections. Our textbooks barely even goes into this much detail unfortunately. Without you, I would've never known. Thank you from me and ever other medical provider that will be able to advance and help those in need from the knowledge you're providing us for free! (Donations will be headed your way shortly.)
lol bitch u sly
MythicBlade fo real tho they call me the queen of cardiology and all I did was watch this video 😂😂😂 (to be fair, I did share it with them but they never watched it)
I'm on cardiac ICU right now in residency and just wanted a refresher. You explain this better than any cardiologist or cardiology fellow I've ever encountered.
Such a clear and concise lecture. Good stuff for those like me (at some point) who read and read and only got more and more confused. Looking forward to more lectures especially on clinical application.
I trust this guy so much that without my lecturers even teaching me i straight away jump into these videos so that i am crystal clear in my concepts from the beginning. And i am glad i found these videos so early in my medlife ! Huge thanks to all the ninja nerd team ! Love ya guys ! U deserve so much more !
OMG I NEEDED THIS SO MUCH ! I'm getting a pen and paper and will start, WHAT A LIFE SAVIOR !
I think im in love with this man. I have my med school admissions exam in a few days and this was so unclear to me in the book and now i had a huge lightbulb moment. Thank you!
I was confused all through Paramedic school and you just made the light bulb come on explaining this. You're Awesome!!!
You are awesome Zach. Continue to create the revolution in teaching medical-related topics. Thank you for these lectures! 🤩☺

Great lecture! Blown away by your attention to detail and by your obvious joy in teaching this topic :) Thank you, Zach!
perfect time .
always recommended your channel to my friends.
I am a nurse with a poor college education. And I really feel happy because I am learning stuff that I should know but no one was capable of teaching me in a simple way. Thank you
This is the best explanation of EKG basics I have ever seen. It sure beats route memorization .All critical care nurses should see this.
Fantastic. BEST... ECG breakdown ever... have been struggling with understanding the wave formation in each lead. Thank-you so much. you did an amazing job
This is just one of the most amazing physiology videos I've watched so far. It was long, but it was completely worth it🥰👌👌 Thank you to the moon & back for such amazing effort. 🤗
3 hours at the University and NOTHING ,then you come and in just 1 hour I feel like BAM !!! AWESOME WORK 🙌 KEEP IT UP 👍
Never thought the day would come when I would actually be able to understand ECGs. I used to memorise without understanding, and after a while I would forget. Now I have a solid foundation. Thank you!
Your approach to teaching is quite commendable. You're one of the best out there!
You might be the best teacher I've ever had, for 53 minutes I thought to myself " why they don't.... you know... just explain it this way....." this was A MA ZING.
miles better than the lectures I've had at university - barely had a clue til watching this
One of best ECG talks Ive ever seen. thank you
this is so amazing my attention didn't waver even for a second and the entire content of the lecture has just embedded in my brain!
thank you soo much sir!!
This is the best explanation I could have ever imagined to get!
THIS IS EXACTLY WHAT I NEEDED FOR JANUARY! Thank you so much!
This was THE BEST explanation of the EKG I have ever heard. It gave me a much better understanding of what sections of the heart the different leads are capturing so I, as a clinician, can interpret the EKG better (until I am more experienced reading them like the Ninja Nerd and I will be a Champion)!!....THANKS EVER SO MUCH TO THE NINJA NERD-YOU ARE THE BEST!! :)
You are gifted! this is the best explanation to ECG i have EVER SEEN!!
I have an exam in 10 hours on CV physiology and was struggling to understand ECG/vector analysis from my professor's lectures. I know, I shouldn't have procrastinated but I am so glad I found this!!! Everything just clicked. I'm pretty sure you just saved my grade. I know whose videos I'll be watching for our next block :) Thank you so much!!!
This video should be played in every medical college in the world
I moving from my country and I am reviewing cardiology , EKG and general medicine ... Ninja Nerds YOU ROCK !!! so clear explanations made me love EKG again : )
I literally had no idea I could understand basic EKG that much & after watching this full video, I find myself confident and satisfying with my knowledge on EKG
The best EKG video yet one of the most underrated channels. Ironic. Thanks for this! Made me appreciate Cardio esp EKG even more. :) more power and God bless!
Basically my notes, hope I did well xD
When negative moves to positive, there is a positive deflection (QRS complex)
When positive moves to negative, there is a negative deflection (Inverted QRS)
The flatline that comes immediately after the P wave is when the electrical current hits the AV node AKA PR Interval which holds the impulse for 0.1sec.
The dip after the flatline, right before the QRS complex is a negative deflection (Q wave).
The bundle branches depolarize from left to right. Essentially because of the anatomy of the heart, the conductivity is moving top to bottom or negative to positive AKA positive deflection, then from bottom left (positive) to right and up or positive to negative AKA negative deflection. When it reached the apex, it ended at a positive point. Now it's moving from a positive point (Apex) back to the right side and up a little which is a negative point, hence the positive to negative. This negative deflection is the Q wave or septal depolarization.
The purkinje fibers are depolarized which then causes the myocardium to contract, making the ventricles contract or a big positive deflection AKA R wave. The R wave is depolarization at the APEX of the heart.
Lead 2 is located at the left foot. The net conduction points south east, if our body was a compass. The LV generates more conduction because it's thicker than the RV. Therefore, the net conduction leans a little left instead of the middle.
As stimulation is moved outward at the R wave, the current must go up, and when we go from down to up thats a positive to negative AKA negative deflection. This negative deflection is the S wave which is the plummet after the R wave on an EKG. The S wave is the depolarization at the BASE of the heart.
ST segment is the time at which the ventricles are still depolarized(no extra movement=isoelectric), it hasn’t repolarized yet.
The negative charge is now moving back up and away from the positive Apex, it causes a positive deflection AKA the T wave.
12-Lead-EKG gives a 3D view of the heart on a 2D plane.
3-Leads are bipolar limb leads: Lead I Lead II Lead III -
(Have negative and positive electrode for each) | Looks at the heart in a frontal plane
| Coronal view or front and back view
3-Augmented Unipolar Limb (AUL): Leads AVF(Foot) AVR AVL -
Lead I is a neg node on the right shoulder and a pos node on the left shoulder, that imaginary line that connects the 2 is Lead I - Looking from the left/lateral heart.
Lead II another neg node on the right shoulder and a pos node on the left leg. The imaginary line is from the right shoulder to the left leg AKA Lead II. - Looking from inferior to superior.
Lead III another pos node on the left leg and a neg node on the left shoulder. - Looking from right/lat heart/inferior.
Einthoven’s Law: Lead I + Lead III = Lead II
The AUL has one node per part of the body.
What the AUL does is switch the charges on the nodes to see what’s happening on one side of the heart and computes it on the EKG.
AVR shows what’s happening on the right side -> creates a positive charge on the right node/arm while the left arm and left leg are negative.
AVF shows what's happening from the inferior view -> creates a positive charge on the left leg node while the left arm and the left arm are negative.
AVL shows what’s happening on the left/lateral side -> creates a positive charge on the left node/arm while the right arm and left leg are negative.
6- Precordial Chest Leads: Lead I Lead II Lead III Lead IV Lead V Lead VI - Transverse or horizontal section
Right 4th intercostal space V1
Left 4th intercostal space V2
Left 5th Intercostal space mid clavicular V4
V3 goes between V2 and V4
Left armpit (anterior axillary line) 5th intercostal V5
Left middle armpit (mid axillary line) 5th intercostal V6
Precordial Chest Leads don’t need negative charge leads with the positive ones, just positive leads (unipolar)
V1/V2 picks up interventricular septum conduction/posterior
V3/V4 picks up anterior ventricular wall conduction
V5/V6 picks up left lateral part of the heart conduction
V7/V8/V9 for posterior
boi, you make my life easier, virtual hug from malaysia
God bless you again virtual hug from india 🙏🏽

In 10:03 it’s actually the PR segment, not interval. Segments are just the straight lines, while the PR interval includes also the P wave.
interval in the sense of actual delay in conduction no recording in the paper just a straight line - it makes sense in his order and your's also makes a point like in the books
