    Dear Shark Tank Applicant,
    Congratulations on making it to the Personal Video Submission round for ABC's Shark Tank. There are two documents attached: 1) Application Packet and 2) Background Form. You must DOWNLOAD, PRINT, and fill out COMPLETELY both documents. You must hand write the documents in black or blue ink. Do not print out double-sided and do not staple the pages. Use paperclips or binder clips. If you have any questions about any of the documents please call us. Incomplete forms will only slow down the process and could affect your chances of getting on the show. Within the Application Packet are instructions on how to make your 5 minute personal video submission. I just want to add that this video is your only chance to impress everybody so make it great. Wow us! Dazzle us! Be memorable! In addition to the Application Packet, Background Form and Personal Video please include a recent photo of yourself, a copy of your photo-ID, a picture or drawing of your product/idea/business/etc. and samples of your product if applicable. Once completed send all of this to the address below as soon as possible because once we've found enough good people we are done casting. If selected to come to Los Angeles for the show you will need to be available for up to 5 days in October.
    HOME TAPE SUBMISSION REQUIREMENTS
    If you are applying as part of a team of collaborators, each collaborator must complete and submit his/her own Application Packet; however, collaborators may appear together in a single audition video.
    BE SURE TO ANSWER ALL OF THESE QUESTIONS
    1. What’s your name, where are you from and what do you do for a living?
    2. What is your business/product?
    3. How much money do you need from investors and what percentage of your company/idea/product are you willing to give up (e.g., 10%, 40%, etc.)? Be specific. Make sure the amount and percentage you are asking for matches what you’ve written on the application.
    4. How will the money be used?
    5. Please describe your product/business.
    - What is it? Is it patented?
    - How or why does it work? (Do a demonstration if you can.)
    - What’s the market for this and why?
    - How did you come up with the idea?
    - Why is your idea the next best thing?
    6. How much have you invested in your business/product? What would happen if you can’t get your business off the ground?
    7. When did you start inventing or become an aspiring entrepreneur? What influenced this decision?
    8. What does your business/product mean to you?
    9. What is your ultimate goal for your business/product?
    10. What has been your biggest challenge so far?
    11. What do your family and/or friends think of your business/product?
    12. Is there anything else about you or your business/product that you would like to share?
    *Do not include Marketing/Advertising in the amount of $ you're asking for
    *Don't say "invention" - say product or business (if applicable)
    *Have "Infomercial" energy, and please stand up. Do not sit down while taping.
    *Feel free to brag about any accolades you've received

    The product only ended up in my golf bag (still used to this day) but it was a great experience both in learning more about product manufacturing, patent application process as well as visiting the studios in Hollywood :)
