Have seen also Bane + SEE to die to Queen Ami. Currently the only really guarantee counter is JMK + CAT with ignore taunt datacron. But i have also seen QGJ+JKA+KAM cheese to kill at least Obi with JKA AOE.
Btw i think CAT would kill the Master QG if you played manually and let CAT stack enough dmg before using her leap, auto was just using leap too often and let him regen. Also i was telling you that aggressive clicking is probably reason for that error. From programmer point of view if you click too fast it might send duplicate requests to the server and server then thinks client is not in sync so tells it to restart. ;)
