I think it's a bunch of audio loops overlayed depending on rpm / load , gear etc. Basically you will need some audio loop samples for turbo flutter , rev at load , no load , starter sound , and overlay these with each other properly with varying sampling rates and also varying volume to get that realistic effect.
The only cars that had V4 engine are Lancia Fulvia and Zaporozhetses (ZAZ-965, 966/968 and 968M). This doesn't sound like a V4 at all, it's a V8 flatplane or an I4.
