    Festoons are a fascia-related issue. The lack of blood flow and dehydration cause the fascia to become rigid, creating a vicious cycle of reduced oxygen and nutrient supply due to poor circulation. The solution is myofascial release (deep massage) of the affected area. Once blood flow is restored, natural healing can occur.

    Oculoplastic surgeon here. Great video about a very common problem: festoons. Super detailed explanation. One point: Lower lid blepharoplasty ( LLB) does NOT remove festoons. LLB removes the orbital fat that is protruded out of the orbit and this is not even near any festoon. One is at the level of the lower lid, the other is located on the cheek, away from the orbit, anatomically. Tightening the lids skin is almost always performed with LLB and still does NOT affect any festoon. However, what I recommend to my patients is to add a CO2 laser of the lower lids and face. This is in fact what is the best approach for the combination of LL bleph plus festoons in my specialty. Also, about the salt, I recommend using half Nu-salt and half table salt in a shaker. It works well if salt is at the core of the festoon.😉

      Thank you for your valuable input. I had co2 11 weeks ago full face. For some reason I have what looks like edema in that festoon area but only on one cheek 🤦‍♀️. Is it too early to expect this festoon area to look better ??

    A friend had the issue of fat deposit under one eye and the other eye was normal. She pretty much tried every thing you have talked about and eventually, she underwent an under-eye blepharoplasty procedure. Excellent results.

    I majored in photography and you are so right. Lighting is everything. You can have perfect skin/makeup and if you have bad lighting the photo won't look good. Vice versa you can have no makeup and not so great skin and if you have the perfect lighting you can look great!

      @rajanimdskincare Dr. Rajan, I am about to buy your new LED mask. I was just wondering if there is a risk of it causing melasma in my skin. I never had melasma. My skin is just a little bit darker under my eyes and around my mouth/chin. I have a light med olive completion (darker during the summer). I also have some freckles on my cheeks. I just heard of other people saying that LED/NIR in general made their melasma worse or caused them to have it. Can you please shed some light on this (no pun intended) on this? Or direct me to a video of yours if you already spoke about it. Thank you! 🙏

      @@dliz4019 my festoons started to look huge the very day we switched our household lights to overhead recess lights. I can see every single dent and bumps on my face. I look horrible and sad part is, those lights are everywhere.

    I remember the exact day when they appeared. Same bathroom, same mirror, same lighting than always. I trained myself to be a backslipper many years ago and I hadn' t had a salty dinner at all. Simple: one day, sooner or later, your face makes "the dubstep drop". I was 50. But you know what? As a goth, I love the way how Miss Gravity has displaced volumes in this area. The undereye was a convexity and now is a concavity, I adore hollowy purple dark circles. The malar mound was a concavity (Willem Dafoe style) and now is a pointy convexity, especially when I take a photo at 45°. Looks like Maleficent' s prosthetics.
    Lower third of the face volume redistribution? Nah, thank you🙅🏻‍♀️😌. RF at the office to the rescue, Zygo Bros!

    I want to share my story as a cautionary tale. I started getting filler under my eyes about 10 years ago. About eight years ago I feared I was being “over-filled” and stopped. I waited for the filler to dissipate and when it did, I was left with “edema” pouches, particularly pronounced on one side, which very much appear as malar bags (same area). I have tried almost all potential solutions (short of surgery) including a course of micro needling which a plastic surgeon thought may help-it did not. Now it is something I just live with. Some days it is more pronounced but generally better if I sleep on my back. I wish I’d left my face alone!

      Filler often does not dissolve in the body. It must be reversed with an enzyme. In cases like this I inject a dissolver and re fill with PRF. Consider seeing and expert in dissolving.

      @karenfriesen280 I have it, too, and am convinced it's from filler. Mine is also better when I sleep on my back and more pronounced on my right side than left. I just started using a face roller from Sephora so I'll see how that goes. I have used the hyaluronidase in the past and thought it went away, but then later added more filler.

      Thanks for responding. I go to a pretty highly rated derm clinic so it’s disappointing they haven’t been able to offer a good solution. I’ve also tried the rolling with little success. Mine too is more pronounced on the right side. I still get Botox but have sworn off fillers. Yeesh!

      I was going to get under eye filler…..but I dont think I will get it now. I will look at other solutions instead. Thanks for sharing your experiences. I hope you manage to find a solution that works for you

    Thank you for covering the malar groove and bags! I'm in my early 40s and have the groove developing but no bags. I caught my face in the department store mirror the other day and was shocked to see my mid-cheek groove even under soft, diffuse light. I looked so old! 😭I plan to get PRF injections and skin tightening treatments. Hoping they help reverse my face by 4-5 years. 🙏

    I use Restalyne Eyelight in my office. It comes in a half syringe size and you use half on each side. The amount of product is minimal and we get a lovely result.

      AGREE 💯, there are actually three different types of mirrors, that makes a difference too.
      I'm pretty much on my own, 😱 but SADLY made the effort to travel eight hours to be with family.
      So weird, I look in the mirror (especially one's I'm not use to, and I THINK 🤔 TO MYSELF, YUK, DO I ACTUALLY LOOK LIKE THAT😮.
      but, people kept saying where are your wrinkles 😂.
      I'm thinking are they blind🙄😮.
      Not sure, but I did stop having photos taken about 10 yrs AGO, as made me depressed at what I was seeing 😢💓

    the worst, to me, is side lighting, for imperfections.
    Great shirt. Black, pure white and the cool based primary colors suit the Winter color pallette so nicely! Brightens the countenance. Winter pallette is about contrast,sparkle and flair. This is also why shinier fabrics look great👍

    I recently underwent quad blepharoplasty on my upper and lower eyelids and have noticed a sunken tear duct causing dark circles under my eyes. My surgeon assured me that it looks good and advised patience until the sutures come out and the swelling subsides, but I am still experiencing these issues. I would greatly appreciate any guidance or advice you can offer, doctor.

    Hi Dr.Rajani, I recently went through a terrible situation, where I had a sudden eye emergency, and had to have a vitrectomy surgery after having my face down for seven days when I finally was able to put my face up, I noticed this what I think is a festoon under my left eye not the eye that I had the surgery on and I’ve been very upset about it . I’m hoping that it’s going to go away. I’m going to a ent Dr. hoping I have a sinus infection and I’m also going to a dermatologist this week , I also showed it to my retina surgeon but he did not make a comment. I’m going to ask him if it’s a clogged tear duck, but I will try some of these nonsurgical suggestions and hope it will go away because it wasn’t there before this whole incident with the eye surgery and the face downing and I read that festoons are permanent😭

    I went to a plastic surgeon at Cleveland Clinic who does blephs and told me she doesnt know how to treat festoons. I am glad there are home remedies. Shes full of it

      I like skiing in the winter and there are no big mountains. I grew up on the east coast. Also I like windsurfing and kitesurfing. The west coast is closer to maui. Maybe one day I ll move back.     Yup got mine after filler 🤦‍♀️. Had co2 four weeks ago and the cheeks look worse 🤦‍♀️. Maybe in time????! What do you think doc? Use tretinoin too 🤦‍♀️ I’m so upset
    Update four weeks out and doc said it’s fluid from co2 treatment and too early to try to do anything other than lymphatic drainage techniques and jade roller, heat then ice.
    Doc what do you think ? Do you ever have a client get fluid trapped after co2???

    I have mild festoons . I got them as soon as i got filler under my eyes . Needless to say , i don’t do fillers . I don’t do lymphatic massage but i use a foreo and i think it does exactly what a lymphatic massage is supposed to do . I’m trying hifu and rf microneedling ( with regular microneedling ) and will consider surgery if this doesn’t resolve it

      Me if anything has worked for you ? I did co2 and now I seem to have a small pouch of fluid near my cheek. My doc is saying festoon.
      I Didn’t have this before. I think it might be migrated cheek filler I have been doing lymphatic drainage, jade rolling, icing and safflower oil for the past 6 weeks Nothing helps. What are the things that your doing please if you don’t mind sharing

    I just had co2 fractional laser face neck and chest and in two weeks I can say I absolutely love my skin. $3500 even pretty good tightening over and under my eyes. Hopefully it gets even better in a couple months??
    Sadly I can say I have more like edema festoon i think was from filler on one cheek (looks like a small puffy pocket) and co2 seems to be exasperating the look of it. Will this go down? Is it too soon??
    One last question about co2 in the 2nd to third week is all the tightening done? Or does more come as time passes? I’m so impatient doc 😂

      Cameras aren’t as good as eyes at colour. When you’re surrounded with really dark, cAmeras show your face like its glowijg
      And the opposite is true when you’re surrounded with white

    Doc can you subsize it with a needle then the same day do co2? One co2 isn’t working for me unless 12 weeks is too soon to tell ?? Mine is minor and only one side of my face

      IKR?! I had a guy friend in high school ask if I'd been crying, ugh. Worst reaction was being on low carb for a while, had a big special occasion night with lots of pasta and cake. Went into work the next day, and a colleague literally gasped and was ready to take me to the ER because she was convinced I was having an anaphylactic reaction to something.
      Yeah, not embarrassing at all to just say, nope, my eyes basically swell shut after too much white flour/sugar! I also have a systemic lymphatic issue, so as usual, our smart Dr. Is right on the cause for aesthetics, causes and solutions!

    Are festoons/malar bags the same as puffiness around the upper cheek area? I have those & most people think I’ve been crying because I have puffiness around the upper cheek area. I’ve been a sun abuser for the first 30 years of my life & that’s probably what contributed to it.

    I m positive that didn't have those malar edema situation before and you are right it looks more prominent with overhead lights. But unfortunately those lights are everywhere. Now what can I do? I am suffering with those without fillers.

    I have mild festoon under the eyes but more towards the cheek. Looks like fluid.
    I just had co2 full face three weeks ago. Love my skin but the line/lumps are still there she was aggressive there Is it too soon to tell?

    Doc I have a question
    I had co2 9 weeks ago and have what looks like festoon edema on one cheek. If I ever so slightly tug the skin upward the edema seems to disappear a bit. Do you suggest to do thread lift ? How would you handle this fluid looking lump? I’m tired of rubbing and doing lymph draining. It’s exhausting to make it disappear for a short period of turn throughout the day. Thanks in advance and I’m 60 for reference

      It's just a laser like halo or pixels. Its good if you have an expert operator

      @@CapeRN Oh No …. I hope it’s just an allergic reaction and it goes away…. I went to the urgent care and the doctor told me to put preparation H just a small amount there and also ice pack but I am going to the ENT doctor and also a dermatologist next week because this happened all of a sudden after suffering the eye emergency on the opposite eye.

    Bad lighting and angles are your FRIENDS.
    “Bad lighting” can never invent a new failo or problem. It can only exaggeeate existing ones.
    So “badlighting” is a great way to find out what your aesthetic issues are.
    And until you look good in bad lighting, you don’t look good.

    Thank you for everything you teach us Dr. Rajani! I’ve had under eye HA fillers done almost 3 years ago, developed festoons quickly thereafter (one side faster than the other), and they never went away since. Do festoons go away when the filler’s dissipated (which I assume would’ve happened by now), am I stuck with them forever unless trying the remedies you are mentioning? Thank you

      Filler may take 10 years to dissipate. I dissolve it therefore.
