ATV Wreck | Case Breakdown

  • 🚑 🚨 You and your EMT partner are just finishing lunch on a hot Summer day and are dispatched to a rural area in your first-due area for an ATV accident. You are approximately 20 minutes from the area of the incident. Dispatch reports the patient is a 54 year old male who lost control of his ATV and is lying unresponsive in a field that can be accessed easily by your ambulance.
    The dispatcher also states that the caller is the brother of the patient and was also riding his ATV behind the patient and witnessed the patient begin to swerve his ATV erratically before overturning and being thrown approximately 10' from the ATV. He was found by his brother to be bleeding from the head, have snoring respirations, and unconscious.
    You arrive and find the patient with his brother at his side. He is unresponsive with a GCS of 1-1-1, snoring respirations, a strong radial pulse, and warm skin that is pale and clammy. Your partner provides manual cervical immobilization and performs a jaw thrust that improves the snoring respirations. Rapid Trauma Assessment (RTA) reveals a hematoma to the occipital region of the head with minor bleeding, pupils equal in size and reactive to sunlight, shallow respirations, global abrasions, and the remaining RTA is unremarkable.
    You obtain a baseline set of vitals that include a RR of 12, HR of 64, BP of 154/90, SaO2 of 95%, and FSBS of 45 mg/dL (2.5 mmol/L). The brother is a poor historian on the patient's medical history, but is able to state he has a history of heart problems, hypertension, and diabetes. He is unsure of medications he takes and does not know of any allergies, but states he is certain he takes some type of shot prior to his meals.
