I am from india would like to i have and spelling mistake in my mothers name, she has all her documents with name Sangeeta but it has to be Sangita , as we need to apply for her Passport with " Sangita" not Sangeeta but she don't have any proof for Sangita name spelling but i have made and affidavit stating the same will that work while apply for Passports??
As shall we ask to mention her name as Sangita on the basis of affidavit. As her adhaar or any other documents has the name of Sangeeta.

If you are a US citizen by birth or naturalization, and if your name has been changed; you don't need to provide a name change affidavit when you apply for OCI. You only need to submit a copy of the court order issuing the name change

My minor child's (US CITIZEN) first name has been changed legally and his existing OCI card needs to be updated with new name. so how do I populate the "Previous Name" under Personal Particulars in the form ? The checklist says " In case of change of name, previous name must invariably be mentioned in the OCI application, failing which the OCI application is liable to be rejected"

I believe after you access the form online to update the name, under the services for re-issuance of OCI Card, there is an option called "Change of Personal Particulars". Choose that option, then click next, and it'll take you to the form.
And in the form, I think they don't specifically ask you for the previous name. At the bottom of the application page, there is a column that says "Reason for Availing the Service" and in that column is where you mention the name...something like;
"Change of Personal Particulars
previous name - First and Last
new name - First and Last".
I think this is how you do it, though I'm not sure. If you still need help, I'd probably recommend you get in contact with the Embassy/Consulate of India near you. I do understand how overwhelming this is....... hopefully, this helps!

Thank you for the information -- it is really useful .. can you also add an example when the the scenario is split of name . Ex: OldName is Dilip Kumar as Given Name with Surname blank -- and new name : Dilip as First Name and Kumar as Surname? -- since both names are same , what to be put as old name and new name?
They have asked to submit this Affidavit even in case of marriage which just need adding husband's surname in passport. Looks like they have used confusing language so call center calls can beyond 5 minutes and it can lead to charge in credit card.

There isn't a field for signature at the bottom of the affidavit just a field for old name - is there where I sign? 2. The bottom of the affidavit has the name and address fields twice. Hi Suja. Thanks for the video. For my wife, we applied for surname change due to marriage and included the marriage certificate. But they asked us to also submit the affidavit. So, I guess it is not true that you don't require affidavit for surname change due to marriage.

@@jssaini1998 Yes I went to local Notary and ask them to basically that both names belong to me and explain your situation in that letter, I also notarized my DL and Passport copies and sent to Indian Consulate. that worked.

My Current Given Name is Ashish Nigam , and i want it split in given name and Surname , what will be old name and new name ( both spellings are exactly same ) , as affidavit template does not have separate columns for first name and surname

My daughter had the name as per the birth certificate in her Indian passport however, we decided to change her name and we got it changed in her birth certificate in India and currently, her passport is expired. Pls. let me know what Public documents can be provided as evidence for new name. In school, I don't think we can change the name to new name without changing in Passport first ?
You can provide driving license, newspaper advertisement clippings.. for school your newspaper advertisement should be enough
Name Change from Pannakumari to Panna was done in us passport and pa driver license Id without any court document or affidavit. Why VFS or Indian consulate need any document (Affidavit). I hope no need for news paper clipping. In Gujarat where Mr Modi is it very common to add kumari, kumar, bhai, bahen, kant, lal etc. I hope while US government understand and why not our own Indian government and staff understand this small facts. VFS is so so customer unfriendly and confusing. Looks like scam to me.

If your only changing the last name after marriage then not necessary but looks like your removing part of given name , in that case affidavit is needed

2. I need to change my address also due to merging of my location to municipality.
3. Also my passport got expired by 9months.

Hello Suja - I sent application for the renewal for my daughter after 20 years of age. One of the thing is that my daughter is missing middle name in her original OCI Card. In her US passport she does have middle name. Here is the response from VFS Global.
OCI Card Applicant’s name on the US Passport does not match with OCI card. Please provide Affidavit for change of name. Must be notarized.

2 people are witnesses. witnesses can be other than close relative like friends .

I changed my name during US naturalization, I basically changed some names to initials. For e.g Lakshmana Siva Rama Krishnan was changed to Lakshmana S R Krishnan. Do I need a name change affidavit. If affidavit is required can I use the US Issued Name change form as proof.

We just changed name on gazzete in india for my son name .we have to renew his passport from here .in school also he s having his passport name .He s in VPK (4 years)now .how to proceed for name change in passport?
