Without radiograph, ask for the patient symptoms, if it is pain or just sensitivity and if it subsides or not. But in the case of pedo patients symptoms are not completely reliable.
If there is pain which does not subside even after the removal of stimuli, it is mostly irreversible pulpitis, can go for pulpectomy. If the pain subsides after removal of stimuli it is reversible pulitis, can go for direct pulp capping or pulpotomy. If there is no pain or only sensitivity, try indirect pulp capping or restoration alone depending on the depth of carious lesion.
BUT IN ANY CASE, A RADIOGRAPH WILL HELP IN DIAGNOSIS BETTER AND WILL LET THE TREATMENT PLAN BE MORE APPROPRIATE.
