Okay I understand that each share holder has to fill separate form, but what if me and my mother(joint owners) want to add my father, same one person as nominee. In that case should we fill one form or two?

Very informative video on nomination, I have purchased flat in joint ownership and we both owners wants to nominate same person as a nominee. Hence please inform can we filled single triplicate form which has both the owners name and signature. Will it be valid? Or we both owners needs to fill seperate nomination form for singal nominee.
