You need to have two sturdy chairs or boxes that stop the bar 3 inches over your chest. If you havent got a power rack or cage..Then you can go all the way with CONFIDENCE. My numbers shot up like this. Because I was no longer scared of.the weight coming down on me. And I was adding a rep or two every workout.

Don't need to hit failure all the time... I.e. hit heavy weights (4-8 reps) without comprising the form.

You are sawing through your rotator cuff each time you bench at 90 degrees. You are doing a guitine press. Try 30 degree power lift bench press.
