It made them one of the scariest antagonists in videogame history, because they bring the audience a true sense of fear and anxiety as every crucial step might be perfectly calculated by an extremely dangerous and challenging enemy, almost god-like, who takes literally zero effort and has plenty of time to kill and mutilate the player. It even might fit with the concept of psychological studies about the "Uncanny Valley" on character design. Yeah 1990's CAPCOM, you had truly passionate artists.

RE3 was terrifying, the sounds the zombies made was unique from all the other games that used the same generic zombie noise. Till this day, that damn music that plays when he appears chills my blood. This towering monster and the loud blaring horn section sets the scene of how evil this bioweapon is.
The only game that i remember having anxiety was re3. I believe it was the fact that playing it the first time, you never know when or where he appears, plus you keep saving ammo/herbs for him and the bastard appears when you are not expecting.
