Rick consoles Negan on his idea for a united DC with a powerful economy (which is necessary for the future enemies by the way!!), and a flabbergasted Negan is caught off guard by an upward swing by Rick. It's not a great wound, but deep enough to sever part of the jugular. As such, Negan immediately breaks into panic and tackles Rick (he's a lot heavier in the comic, Negan is) before snapping his leg. It's all he can muster before passing out, and medics rush to save Rick who insists that Negan be saved first since he's bleeding out. By a miracle they manage to, but at the cost of Rick having an improperly healed fracture. Hence the cane and improper walk of older Rick in the show when Carl dreams of it.

People are critically raving it because of the changes it went through, but it seems like it may just be more of the same with a new can of paint. Negan actually escaping is such a weird change to make from the comic, and while I hope they do something nice with it, I feel like at this point it's par to expect the less-than-stellar.

I was talking more from a writing standpoint, though. AMC's show has, at least in my opinion, trampled all over its own feet in the past with these deviations from the comic. I just doubt Negan's gonna have the same dramatic swings he did in the original media, maybe returning to Alexandria later on with next to no context to raise suspense since it's a TV show and all. Who knows.
She never got to experience or get to know the dark evil side of negan. She just sees him as a bad guy and judges him based on how he acted since he met her and from what i've seen, hes only been nice to judith since he met her.
He got his throat stitched back together and I guess the cut wasn't deep enough to fatally kill him in seconds. Had Rick decide not to spare him while his throat was slit he would have indeed died.
