If I owed 3 years and have been on a payment plan and are now at 80% paid...is this still worth applying for?

you forget to mention that the onetime forgiveness should be done by the end of you last payment for the debt.. or after you get the panelty forgiveness, they will continue charging interest/fees after that forgiveness
This is true, though if you request the one-time forgiveness once for a year, you can request it again for that year once you've paid off the debt in an IA and get those additional penalties abated. IRM 20.1.1.3.3.2.1(14).
