I live in CA and these are some weird sewage systems. It is not like that here. Access points are usually outside.
There are a few here that's outside as well.
What part of the Us are you in? Why there are always 2 inspection for any drain? Always cast iron, never plastic. How hold these drain are?
New York , 2 for a trap under, sometimes pvc, and some are ove 100 years old.

What's the point of the trap in the sewer line? It seems most of the time it's the cause of the clogs.
It prevents gas, rodents, bugs from getting into the house and stops the clog from getting deeper. Makes my lob a bit easier sometimes 😉

Cast iron House traps clogging for over 100 years or more now.
they clogged far less with higher flow toilets and idiots not throwing "un" flushable wipes(all wipes), dumping grease and everything else known to man down the drains.
even peoples diets changed and there was likely no toilet paper then. also no tree's near the homes/lines, in which most of that idiocy was planted far later on by fools. reap what you've sewn?

i never understand why in the US you have this weird setup of the two caps, am guessing that its some sort or ubend/trap but it seems to cause more issues than its worth, we in the UK don't have anything that like, ours goes straight out to the sewer line
The whole house trap is a leftover in the code from when each fixture did not have its own trap. Most of these are older buildings.

Matter of fact, indoor running water only started to be common after 1900, and most still had an outhouse, or a chamber pot with someone to empty it. (hired staff) The problem to solve was the early running water generally was only for a sink so had no trap. When toilets because common then everything was tied into the same waste pipe. NOW you have the stink at the sink! That's why code started calling for a whole house trap, which also kept sewer rats from crawling back into homes.
