I think the song is about how she feels about her life & what she has been through. I don't think it's about relationships, it's about her inner self. As a Cancer survivor & someone who also had problems with my voice after throat surgery,I can really relate to this.
She did not have cancer again with her throat. She had cancer when she was first going to tour the USA at a very early age. The throat problems were an operation to remove a salivary gland on her tongue which resulted in paralysis for a year where she had to learn to talk again.
Yes I had written the same the other day but my comment was somehow deleted by RUclips lol. Seems the person in his live chat has misled him, not intentionally of course! Also this song was written before her vocal cord paralysis not after as was suggested to him as well. I do believe that what happened to Deltas salivary gland was a common long term complication of her cancer (Hodgkins Lymphoma is known to do this).. but as you say her cancer was cleared when she was very young.
