      OK...I have watched soooo many videos and researched so much about diets, weight loss and fatty liver disease.....I think I FINALLY have some sanity to my health situation. I started on the Keto diet....more accurately pseudo-Keto....so much to learn and then there is NO definitive information.
      When you mentioned people living to over 100....My family came from the Carpathian Mountains of Eastern Europe. They normally lived into their 90's and 100's. I grew up in the US as a second generation living and working on a family farm....much the same way as was done in the old country. We worked hard from sunup to sundown as kids. We didn't have much like suburbanite kids (I considered them spoiled lazy snobs). BUT we ALWAYS had good food to eat. We grew vegetables for sale in town and snacks for us kids were a salad at night or just some cukes or tomatoes, maybe in vinegar with pepper. One thing I did that maybe now that I'm older has caused fatty liver in me is that I drank a gallon of REAL cow's milk every day. Cold, sweet, rich delicious milk. After leaving the farm I learned to deal with the watered down "whole" milk from the stores.
      But now, how do I reverse this issue of fatty liver and the weight I've gained over the past 60 years? I'm not a vegetarian but if vegetables were the only thing I could eat...as long as I could eat any and all of them....I'd be very happy till I'm well over 100. But I'm thinking that the sedentary lifestyle this past 7 years and same eating habits and diet are the culprit. But is that correct? Or is their something else I should be doing? Like maybe finding a doctor that would give me some specific and accurate guidance, instead of a two word diagnosis!

I have had a few bad weeks culminating in feeling off colour for a couple of days....one day of eating wfpb and I already feel better..an object lesson for me to avoid packaged food and sugar!     That was such an outstanding info packed interview. She is wonderful backing up what she says with science. Thank you so much. For the Turmeric try taking with 1/3 (unsweetened home made) almond milk 2/3 water with a dash of of fine black pepper and a dash of ground pepper and a 1/4 teaspoon of EV olive oil which may help with the absorption issue.

    This is the best interview I've heard on this subject, thank you!
    I think computers are the largest contributor to NAFLD. Even more than diet, although I think both diet and exercise are necessary for a healthy life. I say this from my computer as I sit here, overweight, and with NAFLD. I'm going for a lunch hike. I'm going Green Mediterranean diet from Keto as well.

    What a fabulous interview! I love it when a WFPB doctor interviews another WFPB doctor. You always ask the right questions. I am intrigued about the role of some medications and plant fats on fatty liver. I will have to listen to this interview a couple more times. Thank you so much for all the info and making me smarter. People have to understand how our body works and what fuel is needed for optimal health. (I also love your podcasts on "Self principal".).

    As a doctor, she’s wrong. Saturated fats are bad ONLY if you were having lots of processed carbs, then the resulting sugar and insulin spike makes them lay down in the wrong places.
    Most people with insulin resistance cannot handle processed and refined carbs. These Carbs trigger insulin resistance, which triggers an inability to handle carbs, and it’s a vicious cycle. Fibre plants, protein and fats are good for you.

    My ancestors lived into their 90s never went to a gym or lift weights. They walked everywhere.
    After dinner mom would have us hold hands and we would go for a walk down the hill and back. We lived in the mountains. Walking on flat land doesn't do much for anyone. It helps but walking up hills in nature is the best.

    I found this interview very interesting.
    A decade ago, I used to have a high level of ALT(GPT) enzyme, up to 88. It went down to normal values after I started running, lost a considerable weight, and eventually switched from being vegetarian to vegan. Unfortunately, a couple of years later I stopped running, and gradually gained back the weight. But the ALT(GPT) marker stayed fine, even after 8 more years, when I was diagnosed with T2D. I wonder if it is possible that being vegan, avoiding the saturated fat in cheese, has made the difference.

      That’s great to see your improvement. The beauty of a Whole Foods plant based diet is in its nutrient dense calorie poor foods. In addition, it is naturally low in fat. All of these things lower our chances of getting fatty liver.

    Ah, this was a helpful video to watch after just having a tense discussion with a brother that has a fatty liver, and he's a high protein keto aficionado. He gets flustered and reminds me that he has a bachelor's in animal science and made terrific grades. We don't agree on world views and he disagrees with not eating meat. Our father is prediabetic and has had high blood pressure for 20+ years. I say complex carbs aren't the problem, even some fruit, and they dismiss me completely.

      It’s always best to rely on science. There are lots of well designed studies that show how plant based diets are helpful in fatty liver disease. At the end of the day, all we can do is to help people see the truth. We can’t force anyone to change. The best way to convince others is to lead by example.

    All other experts seem to suggest that saturated fat is only bad if it's combined with alot of carbs/sugar. Historical data also shows the increase in NAFLD cases correlate with the increase in consumption of added sugar (though I admit it correlate even better with increased prevalence of obesity, which can be due to both sugar and fat consumption.) 🤔

    You need to get a perfect fatty liver reversal program stay with it religiously it can take up to 4 years but man will you feel the difference

    Awesome interview! So glad to find your channel and this video helped me understand much more. Meeting with Dr tomorrow for my wife and will discuss her issues with high cholesterol, a1c and insulin resistance. Have you any experience with the metabolic problems in a person with Myotonic Dystrophy MD1 ?

      Hi Mike. Thanks for your support. As you know MD1 can affect many systems including muscles, eyes and of course the endocrine organs. The big thing here is that there can be significant insulin resistance. Its a good idea to work with an endocrinologist. There is a lot of complexity here as there are high insulin requirements to overcome insulin resistance. Insulin can lead to increased fat stores which can then compound the problem. One of the first meds used is metformin as can sensitize insulin and has been around for decades.

    Thank you doctors! I appreciate this information so much! I was told I have very early fatty liver. Do not ever drink alcohol, eat mostly plant based diet with fish few and far between, use only whole grain foods, lots of vegetables, nuts, grains and fruits. I only use olive oil, some avocado, and coconut oils. I don’t use much sugar and cook all our foods, including bake most of my whole grain bread. I do have polysystic ovaries, and I am overweight because I had two bad accidents and can’t workout as I would love to. I am working on losing the weight and have lost some already since I have consciously cut down my carbohydrates.

      You can also do intermittent fasting to help with some weight loss and improving your gut. Start by slowly reducing your eating windowto 8-10 hours a day and fasting (water only) outside of 8-10 hour window.

    This doesn’t make any sense for me. I’m thin, no diabetes, exercise, eat well and have fatty liver with elevated liver enzymes. I don’t have any weight to lose.😢

      @bonniek7228 I totally understand what you're going through. I have the same exact issues. Can't gain weight, low blood sugar, low BP..so it's a mystery why the liver is fatty.

      Check out TOFI (Thin outside fat inside). It appears to be the condition for some individuals. I know, it's confusing and sometimes, I feel that we should stop watching health related RUclips videos, as the confusion is increasing 🙄.

      had the same problem. I believe it's because your body makes to much triglycerides and it goes straight to the liver . No fault of your diet .

    Introducing a small glass of daily wine from being a teetotaller helped reverse my fatty liver as it sensitised me to insulin. I went off alcohol as I’m undergoing pregnancy attempts and fatty liver and prediabetes is back

    I dropped around 50lbs on a wfpb diet prior to getting my fatty liver diagnosis. I was getting my gallbladder checked in an ultrasound where my Dr saw the fatty liver. all my liver labs were in normal range, and my alt is 36 tho she recommends under 30 in the video.

    Get off of all refine oils this causes high inflammation in the body causes great pain (use real extra virgin olive oil) (use avocado oil) both of these also help in the fatty liver reversal

    I went to my GP bc I was thirsty constantly for over a month and when the the blood work came back it showed ANA titer 1:160 homogenous pattern hep2 ANA positive. So I was wondering if I could have AIH or primary biliary cirrhosis. But the reason I’m listening to this video is bc back in 2020 I took a 23&me test that included dna health screening that showed an increased risk of developing fatty liver disease. I didn’t think too much about it but in the process of trying to figure out what might be going on remembered. Which lead me to read an article online on Pubmed Central that talked about NAFLD patients could but rarely did present with a titer as high(1:160) and the higher the titer the more fibrosis and risk for already having NASH. If I do have nafld I’m not overweight 112 lbs. and never eaten a lot of meat and been a vegetarian almost a year now. After listening to another one of the good kind Dr. Hashimi ‘s informative videos (a recent short actually) on the benefits of taurine in mice and monkeys too (I think(?) now wonder if my taurine levels may be low as a result of not eating meat. Right now it’s a wait n see game but wanted to say thankyou to both these doctors.

    This is so confusing, there are so many differing opinions on the best diet for fatty liver disease, who is right that is the question? I have severe fatty liver disease but I only weigh 96-98lbs, so hardly in the overweight or obese category? my HBa1c is very low 32 (uk measurements) i am not able to exercise much and I rarely eat meat, or eggs so what is the right thing to do diet wise? Answers and advice would be gratefully received! Thanks in advance.

      There are lots of causes for fatty liver. Without knowing your full medical history, its a little difficult to answer. I would strongly recommend working with your doctor to see what might be contributing to your fatty liver. I am glad to see you minimizing meat. Focus on whole grains, fruits, vegetables, legumes. Every little step counts towards exercise :)

    This is most useful, thanks! I am sort of addicted to listening to plant based medical advice, and originally got my information from the likes of Mcdougall, Pritikin, Campbell and many others. I'm not quite so addicted that I go to support groups of diseases I don't have like Tyler Durden in Fight Club! =D
    On the Starch Solution, fixed my cholesterol problem 4 years ago and lost 50 lb, and came off blood pressure medication with doctor supervision, and that same med was controlling resting heart rate which was previously high. All were fixed but blood pressure is a bit high at 115/83 and I need to exercise now. All the other things are fine 4 years later and I am losing more weight and getting in shape.
    I'm busy working on getting some ab definition for vanity since now I can and before on the SAD diet didn't have a chance.

      @@joanneclark8256 Diastolic at 83 is high. However, I am higher now even, at about 135 / 90.
      The Starch Solution book defines the diet and it is no meat, dairy, oil or processed foods but by it's definition of 'processed' allows whole wheat pasta and whole wheat flour products like bread.
      I assume that a very large reduction in dietary saturated fat caused my total serum cholesterol to drop. That nutrient tends to be very low on a whole food-plant based diet.

    Dear Dr Gray. Please let me know the evidence on which you are relying when you say we know that dietary saturated fat increases fat in the liver and fibrosis and scarring. Which papers do you rely on? You also refer to studies showing that saturated fat in the muscle makes us less able to uptake glucose and store it as glycogen in the muscle. And how do we know that the saturated fat in the muscle comes from dietary saturated fat rather than carbohydrates? Again which papers are you relying on? Thankyou.

    My guts do not read lables and do not recognise the difference between simple and complex sugars/carbs, both will increase blood glucose and insuline driving the long term insuline/glucose roller coaster. To bunch fruit and vegetables into the same group and red and processed meat into another is so wrong and gives missleading results. There are no mechanisms causing sat fat consumption to cause sat fat accumulation in cells and liver. Very few people can follow a -500 kcal/day regime in the long run, much easier to scrap the carbs, they are as such not needed by any process in the human body.

    Hmm I am confused I thought I was told that Choline can help pull the fat out of the liver. You mentioned it about 20 minutes in and made it sound like a bad thing. So maybe I am not understanding something.

      Choline can increase TMAO that is linked to an increase risk of heart disease. People who eat most plants don't have this issue as the gut bacteria is different. The concern is mostly in folks who eat a lot of meat. Hopefully that clears it up

    Now I have all kinds of questions. Like doe Milk Thistle have any effect on a fatter liver? Also does intermittent fasting help? Does drinking lots of water help?

      Intermittent fasting is a wonderful tool and it can be used as part of a healthy lifestyle. Lots of interesting data on milk thistle. But for fatty liver, you really need to focus on weight loss.

    you mentioned keto in this video and I have been doing keto this year ever since I found out I had a fatty liver last year and I have lost 50 pounds this year I do try to eat as many good fats as I can but i do eat some bacon, by some I mean 2 maybe 4 tops. I also maintain a calorie deficit. does it really matter the kind of fat I eat if I am losing weight and I know for a fact my bmi and overall fat percentage has gone down quite a bit?in faster time then what would have otherwise happened. A major part of ketone production happens in the liver so by definition wouldnt that process stop fat storage?

    Thank you for this video I’ve been having to push my doctor because I just feel very uncomfortable in my abdomen no major signs but my family everybody in my family has heart disease diabetes kidney issues and I’ve been trying to eat clean whole food plant-based for 17 years and I am still not feeling comfortable and still considered obese ain’t no processed food And I feel miserable I exercise daily and I can’t seem to lose the weight and my doctors don’t find anything wrong with me so I finally insisted and asked for an abdominal scan and that’s where they found Nash it would’ve gone and diagnosed if I wasn’t so adamant because of my eating style I didn’t show any issues with diabetes or heart disease but my body fat BMI is 36 and my sister has had a gastric bypass and has heart disease and everybody on my dad side died of heart disease so when I had to give my grandmother her insulin needle when I was a kid I realize whatever they were eating or doing I didn’t wanna be sick like that so I’ve always been on a journey to figure this out 17 years ago I had an 8 pound fibroid attached to my uterus nobody picked up on anything except the doctor asked me if I was an alcoholic as my liver was smashed flat and then she just said it was probably the 8 pound five by pushing against it and that was the last time I had any discussions about liver so I asked for a referral to mayo clinic in Jacksonville looking forward to an appointment but this is interesting because I’ve been doing whole food plant-based trying to stay lower carb trying to make sure I’m getting the right balance and I’m still having so much discomfort in my abdomen

      I am so sorry you are going through so much. I would recommend working with an Obesity Medicine Specialist to help you with weight loss. We have so many new treatments that can be used along with your plant-based diet, exercise, sleep and reducing stress.

    Would buying a glucose monitor help? I’m not overweight and have a healthy bmi. I’m exercising, eating correctly and am not losing weight. I feel like I might have a blood sugar issue that is being missed.

    Would you recommend calorie restriction to someone recovering from an eating disorder? Can fatty liver be caused by weight gain in eating disorder recovery?

      When someone has an eating disorder, its important to work with a therapist or other professional. Its key to not make any drastic changes but focus on small, sustainable changes in the long run. Life is a journey and we have to enjoy the ride :)

Question - do you know if Vitamin D can cause kidney stones (I've had too many and never again if possible!!) and if so do you recommend a level to stay below with D3 supplements? (ex. 4,000 daily is safe? I have a deficiency I'm working to fix) THANKS

      Hi Linda. Not a simple answer as it depends on the situation. Vitamin D increases the absorption of calcium from the gut. However, Calcium in the food we eat binds to oxalate in the gut and reduces oxalate from going into the body. When I see my patients for kidney stones, I have them start with a 24-hour urine test (kidney stone panel). The most important things I am looking for are what is the urine output amount (should be 2+ liters in 24 hours), are they spilling excess calcium, oxalate, uric acid, or sodium. Are they low in citrate? I need to know what type of stone it is as some stones form in an acidic environment and others in alkaline. Lastly, need to know how much magnesium is in the urine as well.

    You don't want to abuse the rapid weight loss and shrink too much (you want to shrink to your normal body weight) and get off of the rapid weight loss-
    (Just use it as you need it- if you abuse yourself with sugar and carbohydrates you shouldn't put in your body)! These kind of foods gave you the fatty liver and other problems in your digestive tract

      I am so sorry to hear about your father's illness. He will be in my prayers. Liver cirrhosis is generally not considered to be reversible. However, we can try to prevent any further injury to the liver with exercise, low-fat plant-based diets, proper sleep.

      @@waidal-madhajy2946 Yes, that is what I put my patients on. However, its a low-fat, whole-food plant based diet. Another words, stay away from processed foods like chips, cookies, candies sodas, diet sodas etc

    Do not drink any fluids or liquids with high sugar and fructose corn syrup (fructose corn syrup damages the liver just like alcohol)

    Use the rapid weight loss to shrink where you want to be and then maintain yourself was a ketogenic diet you never have more than (5 g )of sugar in a (24-hour period) that's all the human body requires! plus, it's better to eat some natural fruit then refined sugars and you never want to use sugar substitutes and you substitute sugar with a natural organic (stevia) which has many benefits to the human body
    Because the human body has insulin release trouble we need to get your insulin releases back to normal so we want to keep the sugar out of the diet and out of your body you need to be checked for (cellular inflammation) this causes the thyroid to act up along with a fatty liver and other digestive issues

    I just recently found out that I have a little bit of fat in my liver but I've had high liver enzymes for about 4 years now but I also have osteoporosis that I'm treating naturally I'm going the natural way, when I go grocery shopping I used to only focus on bone strengthening foods but now that I know I have this fat in my liver I know I need to focus on liver healthy foods as well
    How do I find a balance between eating for my bone health and eating to lower fatty liver??

      I think its important to build your foundation on primarily whole-food, plant based diet. Then add on other foods as needed. No reason to make it too complex. If you want to save your bones, you have to add resistance training. This is incredibly important

      @@SELFPrinciple I mostly eat a plant-based diet I also recently increased my protein intake a lot of my proteins to come from plant source, I started drinking milk thistle tea because I heard it's good for a liver health, I've always exercised my whole life last year I started doing strength training but the problem was that I was exercising and under eating that is why I have osteoporosis for years of under eating and over exercising it was only until I was diagnosed with osteoporosis in March of this year that it was a wake-up call for me to eat more , my hotel life I only focused on calories not nutrition but now I tried to focus on getting enough calories and getting enough nutrition, but what about dietary fat? I know fat is essential bones in need the vitamins from fat and the rest of the body but what about the liver? I know it shouldn't be this difficult but it kind of is because I'm worried that foods that are good for my bones may not be good for my liver, but I don't want to starve my bones of nutrients in order to save my liver this is difficult ☹️😓

    I've been on Starch Solution for two months.
    I lost 21 pounds. I’ve had a fatty liver for years. Bad
    news is, my blood tests last week showed,
    ALT, GGT, CRP, cholesterol and Alc were all
    elevated and now my doctor wants me to go
    low carb. I'm mad and depressed about all
    this. I have an ultrasound soon to see if my
    liver is worse than the previous ultrasound I
    had a few years ago. Why would this diet
    raise everything???? I was feeling great and
    everything. Now I'm scared. I don't like low
    carb either. Anyone have this happen while
    on starch solution? Is it from too much fruit? I hear fruit can raise cholesterol, which is a bummer.

      Make sure you are not consuming coconut, a source of saturated fat. My cholesterol went UP when I changed to a vegan diet, eating dairy substitutes multiple times a day. Dropped 60 pts when I went WFPB, no oil, salt or sugar.

      Try more Non starchy veggies, a more Nutritarian approach (Dr. Joel Fuhrman)! Also more Legumes/Beans/Lentils, less white potatoes and white rice would be a good shift!

      @@karlhungus5554, I still have a fatty liver, but had to go back to low carb, which gave me much better test results. CT scan still shows a fatty liver though, but liver enzymes, cholesterol and other things did improve.

      ​@@Kimberli1966 Thank you for taking the time to respond. I'm sorry to hear the fatty liver persists, but I'm glad to hear your low-carb diet did improve other factors. I followed low-carb for over five years and had some things improve and other things worsen. No diet affects everyone the same way, obviously.
      There's a supplement called TUDCA that I'm taking for gallbladder support (hoping to gradually dissolve a gallstone) that is alleged to help the liver, too. On PubMed there's a paper that concluded: "TUDCA attenuates the progression of HFD-induced NAFLD in mice by ameliorating gut inflammation, improving intestinal barrier function, decreasing intestinal fat transport and modulating intestinal microbiota composition." Note: HFD = High-Fat Diet.
      Now, it's a mouse study and only 16 weeks in duration. So, it's hardly a reason to start taking TUDCA on your own. But, if you're interested in knowing more, maybe do some online research and print out some summaries (from reputable sites like PubMed, Mayo Clinic, Cleveland Clinic, etc.) to share and discuss with your doctor. Either way, I wish you well!

      Beans are a good source of protein and fiber. They also have complex carbs. So overall, they are good for your health including your liver. For eggs, we generally promote moving towards more plant based diet as its better for the environment and your health.

    The terrible bias here is that the answer is complex plant carbohydrates, regardless of the question, regardless of the evidence that a well formulated ketogenic diet including fatty meat is good for metabolic health. WHAT, that can't be! It interferes with what we already "know".
    To make statements like "we know that saturated fat causes fatty liver" is a completely unsubstantiated statement.
    Get off your dogma and study the real science without prejudice.
