Our whole medical system is a mess! Profit driven instead of patient driven puts us all at risk. Corporate "culture" pretty much everywhere is awful. I see burnout in all different fields. I am not in health care, but I am on call pretty much all the time, and it wears on me. It's good that you are prioritizing your mental health. Hopefully you will land somewhere that feels like a better, safer, fit.

I definitely understand and empathize. It sounds like we practicing as NPs around the same time. I started back in Sept 2018. I felt burnt out as well and quit my NP job back in February. I was getting increasingly anxious and more depressed going to work. I knew it was time to go. The clinic I was at was run by chiropractors. I took a crisis ICU RN position but I was quickly reminded why I left bedside. I agree with a previous comment that some nurses are brutal. Right now I’m trying to figure out my next move.

I really appreciate your take/insight at 6:55. This is the nature of being a physician. We absolutely care about patients, but our caseload is insane, and we have far too many patients and are spread too thin to be able to spend the kind of time that we would like to with each and every patient. I think you have a unique perspective because you've seen both sides (bedside/provider role). I dunno what the solution is, but I sympathize with what you are saying. Being fast isn't from a lack of empathy, it's from outside pressures and for-profit bullshit.

Hey Laurie, I’ve been a Family Practice NP for 2 years and see an average of 15 pts per day in clinic. I have considered urgent care, but friends I know who do it see so many pts per day, BUT aren’t dealing with the number of complaints or going over labs, refilling meds, etc like we do, so I think it’s less stressful, depending on how busy the clinic is. My typical patient encounter is: “I’m here for lab results, BUT I’m having migraines, abdominal pain, and back pain for the last few weeks. Although we want to address all these concerns in one visit, we cannot be thorough this way, and trying to explain this gets challenging, because the pt wants everything addressed in one fifteen minute visit. That’s the most challenging thing for me. Does going over labs and reconciling meds get old? Sure, but it’s the nature of primary care. I’ve been wanting to do nursing homes for awhile since you can make your own schedule and not be tied to a clinic all day. If you want to call in sick or leave half day, your pts have to be cancelled/rescheduled which can be challenging, too. Psych NP is another good choice. They’re paying high 100’s to $200k and visits are quick/lucrative career. It’s just not for me, though. Definitely try other specialties and find your niche. Hospital medicine is stressful and I can see how it’s affecting you, mentally. You got this!

Maybe taking some time off will be good, but remind yourself why you wanted to be a Nurse and Nurse Practitioner. I know NP is different from being a nurse aide ( you have way more responsibility). But take some time off and remind and remember why you started in the Nusing field. That will help you everyday you work. I constantly have to remind myself that. And I became a nurse aide was to care for people, the way my mom was not cared for. I hope this helps at least a little bit. God bless

Currently an NP. I've been practicing as an FNP since 2017. Worked primary care and inpatient ( peds office, PACU, nursing home, childrens hospital and now inpatient acute pain management) sometimes multiple jobs at once. I'm going back to school now for CRNA. NP burnout is real and you have to figure out how to get a job/position that allows you to live your life. We have to move away from living to work and more towards working to live. Sometimes that even means leaving healthcare. For me, even though I love the Healthcare field, specifically nursing, I contemplated leaving a few times. Covid is/was stressful and put a toll on a lot of us. Do what's best for YOUR mental health because your patients will be no good if you aren't, no matter how good your heart is.

Keep in mind that your career isn’t forever but great memories last a lifetime so how you enjoy this time in your life does matter. Nursing homes are the worse to be NP. Some hospitals only care about the amount of patients you see! The more you see the better! That’s the biggest burnout because we as nurses want to spend as much time with our patient to find the root problem in their health.
I think you should try a specialty that you have never thought about. For example, cardiology, pulmonary, dermatology etc.

I don't think getting your acute care certification is a bad idea at all. It may set you back a bit in terms of time, money, and maybe a few other sacrifices, but if you'll be happier overall and can see a fulfilling and long lasting career out of it then I think that it's worth it. I remember you saying before that you prefer the hospital setting. You may deal with lots of stressors in the hospital too but the fact you'll be in a setting you find interesting and exciting may help to keep you going and not burn you out nearly as fast. Plus the stressors you'll face there will be of a different type that you may be able to work through easier.

I think you've reached your optimal LEVEL!! Don't give up! Remember your vacation time. Also 45 patient's a day is OVERLOAD! I'm not an NP but I love nursing. Especially case management and insurance. I just don't like to work from home! I would love a side job.

I have been working as an NP for 7 years. Prior to that I worked as an RN in the hospital for 7 years. Prior to that I worked as a CNA in a nursing home for 4 years. I am OVER healthcare. I work in a clinic that schedules 6-7 patients an hour. We also take walk in patients. The sad thing is this is my "dream job." I care about helping people. This is my 3rd job as an NP. I have been here for 5 years. It's the same whatever you go. Healthcare in the US is completely broken. I'm currently considering my options ☹

Im an RN working agency, getting sent to different LTC and skilled facilities. It is sad seeing what goes on, patients don’t get the proper care they need because nurses have so many patients on our assignment. 25:1, sometimes 30. It’s ridiculous 🤦🏾‍♀️🤦🏾‍♀️🤦🏾‍♀️ God help us. I was so burnt out after a year...i just finished my last day last week of that job, switching to a clinic/slight urgent care setting. Call, charts, it felt like I never had a day off. Now even after i'm done...I'm up at 3am (usually got up at this time to start charting)...I got another job getting paid per hour BUT i won't be taking home any work with me and I no longer have to take call (was on call 24hours/7days a week every 4-6 weeks on top of being on call 6am-6pm on weekdays).

I watched this video the other day and several things went through my head as I am starting school this fall and I know the demands on an ARNP are high so I am a little more nervous. OK as for your situation there are a few things I could suggest to help remove the pressure your under 1. just do wound care and go to a wound care clinic to work, there are several around me in Florida and that is all they do is wound care. 2. Just do wound care in skilled nursing and cut out the rest 3. Just be an ARNP and no wound care at all you have overdone it and loaded yourself down with too much work and burnt yourself out. Everything is to be taken in small doses in life too much of anything is never a good thing, you did wound care (hard enough on its own), regular visits, and hospital visits that is way too much for anyone.

You don't have to stay there if they don't listen to you. I know a few colleagues( NPs) that got burned out quickly in the nursing homes. Don't compare yourself with anyone and don't try to excuse their mismanagement. In my nursing home we had a wound doctor, nurse practitioners and several doctors that rounded. They are giving you a job for many people. You need to calmly tell them what you can or cannot do and how many patients you can see in day. You're documenting or lack of it can bite you later if you're rushing. You need enough time to document, review charts, etc. They need you, and they don't know how much you can take. You need to present it in writing, how many patients you can see on a day. Or else that you will not be working with them. The management may not understand the NP and treat you as a nurse; yet you have the responsibility of diagnosing, prescribing etc that shift RNs don't
