The human body doesn't know the difference between the hand of a lover or a molester. Especially the impressionable mind of a child. The shame an pain if not felt then but later on in adulthood is well portrayed in this film. Kiera Knightly, excellent work.
I don't think that being a masochist is a "mental illness", I see it as a sexual preference. Her anguish and repression, because her father was the one inflicting the action, is what pushed her hysteria. It is extremely difficult to guess what would be the face or voice of somebody recognizing that memory to another human being. She is re-enacting the rationalization of the memory here: it is very difficult to act something like that, there is no reference material. Those that think this is bad acting should express what they think should be the right emotion for such a strong and intimate moment.
People are wondering why Keira Knightley might have overacted or been more theatrical in approach here, but it's usually pretty dark to see someone very accurately portray a mentally ill person, especially someone who has severe traumas like this. I think this movie was meant to be something that was more accessible to the general public, so maybe she overreacted for it to reach the target audience(?)
There is another film about Sabina Speilrein which I think is much better acted by Emilia Fox and Iain glen. It is called the Soul Keeper and I highly recommend it. Also you can watch it for free on youtube.
